When you see a doctor or other health care provider, you may owe certain out-of-pocket costs , like a copayment , coinsurance , or deductible . You may have additional costs or have to pay the entire bill if you see a provider or visit a health care facility that isn’t in your health plan’s network. “Out-of-network” means providers and facilities that haven’t signed a contract with your health plan to provide services. Out-of-network providers may be allowed to bill you for the difference between what your plan pays and the full amount charged for a service. This is called “ balance billing .” This amount is likely more than in-network costs for the same service and might not count toward your plan’s deductible or annual out-of-pocket limit. “Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care—like when you have an emergency or when you schedule a visit at an in- network facility but are unexpectedly treated by an out-of-network provider. Surprise medical bills could cost thousands of dollars depending on the procedure or service. You’re protected from balance billing for: Emergency services If you have an emergency medical condition and get emergency services from an out-of- network provider or facility, the most they can bill you is your plan’s in-network cost-sharing amount (such as copayments, coinsurance, and deductibles). You can’t be balance billed for these emergency services. This includes services you may get after you’re in stable condition, unless you give written consent and give up your protections not to be balanced billed for these post-stabilization services. Certain services at an in-network facility, including a hospital or ambulatory surgical center When you get services from an in-network facility, including a hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers can bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed. If you get other types of services at these in-network facilities, out-of-network providers can’t balance bill you, unless you give written consent and give up your protections. You’re never required to give up your protections from balance billing. You also aren’t required to get out-of-network care. You can choose a provider or facility in your plan’s network. When balance billing isn’t allowed, you also have these protections: You’re only responsible for paying your share of the cost (like the copayments, coinsurance, and deductible that you would pay if the provider or facility was in-network). Your health plan will pay any additional costs to out-of-network providers and facilities directly. Generally, your health plan must: Cover emergency services without requiring you to get approval for services in advance (also known as “prior authorization”). Cover emergency services by out-of-network providers. Base what you owe the provider or facility (cost-sharing) on what it would pay an in-network provider or facility and show that amount in your explanation of benefits. Count any amount you pay for emergency services or out-of-network services toward your in-network deductible and out-of-pocket limit. California Law: California law generally contains balance billing protections similar to those under the No Surprises Act (as described in this Notice), except that the balance billing prohibitions also apply to services received in additional in-network facilities, including laboratories or radiology imaging centers. California also has an independent dispute resolution process to resolve claims-related issues, including disputes with your provider pertaining to receipt of improper balance bills, which can be initiated through the California Department of Insurance. It is a major graft procedure that uses the patient’s bone tissue or a synthetic bone material to create additional jaw mass. Let’s explore this topic in greater detail, break down why you may need it, and walk you through the procedure. We’ll also explain some of the outcomes and possible side effects. Let’s get started! Why would someone need a Sinus Lift? Sinus Lifting is most often used when a patient has previously lost a section of teeth, and they were unable to get dental implants before their alveolar ridge began to deteriorate. When they finally go in for dental implants, they lack the support needed to hold onto the implant. A Sinus Lifting is a surgical procedure that creates additional support by grafting new bone tissue into the jaw and helps encourage natural bone growth. Compared to other bone grafting procedures, a bone block is more invasive than a socket graft, which can be done immediately after removing a tooth, as with wisdom teeth. Additionally, if the bone deterioration is severe, you may also require a sinus lift procedure. Why do Tooth Extractions cause bone loss? After you have a tooth removed, you may suffer some bone loss if you don’t receive dental implants right away. The alveolar bone is prone to deteriorating if left barren for too long. This is because it helps support and holds onto your teeth. Without any teeth, the bone serves no purpose and will atrophy. Over time, your jawbone can also begin to deteriorate. As we chew, the pressure from our teeth helps strengthen the jaw muscles and jawbone. Without a tooth in place, that section of your jaw can also begin to atrophy. THE BONE GRAFTING PROCEDURE Before you undergo implant surgery, your surgeon will take x-rays and CT scans of your head and jaw to determine whether you have enough bone mass to secure dental implant placement. If you are missing bone mass, your surgeon will refer you to an oral surgeon to have a new section of bone grafted into your jaw. More often than not, your surgeon will use an autogenous graft, meaning they will take a block of bone from your lower jaw, hence the name “bone block” grafting. Without a doubt, this option can raise the cost of the bone graft procedure. However, by using your existing bone tissue, the risk of implant failure is greatly reduced. As another option, your surgeon may use a donor’s bone. ALLOGRAFT BONES Allograft bones are sanitized bone samples recovered from a human cadaver. After an organ donor dies, sections of their bone tissues are harvested and sent off to be used as grafting material. The grafted bone is less likely to be rejected because it matches human bone tissue. XENOGRAFT BONE Xenograft bone is another term for a bovine bone sample. A bovine bone graft is less expensive than an allograft or autogenous sample, but it comes with a higher risk of being rejected by the body. If your surgeon uses a native bone sample, they will begin by removing a section of living bone and then graft it into your oral cavity at the implant site using a set of tiny screws. Once the graft is in place, it will take up to four months to heal completely. Post Surgery instructions for Sinus Lifts After surgery, your surgeon will prescribe pain killers and a regimen of antibiotics to prevent infections and soreness. How can I tell if my Wisdom Teeth are emerging? For a few fortunate people, their wisdom teeth will emerge without giving any significant problems. For the vast majority however, emerging wisdom teeth will come along with significant pain, with 85% of people experiencing severe problems. In these cases, the expertise of an oral surgeon will be required to remove them safely. For those who have regularly scheduled dental checkups, it is likely that the surgeon will pinpoint when your wisdom teeth are about to emerge. This is usually spotted on an x-ray well before the wisdom teeth emerge, and allows your dental team to come up with a course of action. In some cases, however, your wisdom teeth will begin to emerge before they are spotted by your surgeon. If this is the case for you, any of these symptoms can be a tell-tale sign: -A throbbing sensation in the rear of your jaw -Gums that are swollen -Cuts or breaks in your gums -Earaches -Jaw pain It should be noted that there are other dental issues that share some of these symptoms in common. For instance, disorders of the temporomandibular joint (TMJ) can also cause jaw pressure or pain, earaches and headaches while tooth sensitivity may be experienced if you have a cavity. Why should I remove my Wisdom Teeth? For most people, the primary reason why their wisdom teeth will need to be removed is a lack of space in the jaw for them to grow in. If this is the case, allowing the wisdom teeth to grow in could create a pushing effect on the other teeth and cause crowding and overlapping. With teeth that are too close together, it can be a challenge to keep them clean, thus increasing the risk of tooth decay and gum disease. Another common disorder of wisdom teeth is impaction. This is when your wisdom teeth are stuck in your jaw and are unable to emerge or can only partially emerge. Both these cases increase the likelihood of infection and abscesses and thus necessitate an extraction. Sometimes your cuspids and canines don’t have enough room to grow in correctly and get impacted in your jaw. Since these are such important teeth for chewing and for your bite, it’s essential that they grow in correctly. In cases of impacted cuspids and canines, a grid and chain procedure is often the right option. A grid & chain procedure combines the talents of both your Bay Area OSM provider and an expert in orthodontics. Once your surgeon clears the way for your canine to emerge, braces are placed on your tooth to give it the necessary space to grow. How do I know if my canines are impacted? Most canines usually emerge around the age of 13. If they aren’t coming out, make an appointment at one of our Bay Area offices for a professional evaluation. Wisdom teeth are also known as the third set of molars. As these teeth are the last to emerge, usually during your late teens or early 20’s, there is little space for them to grow out naturally. As this set of teeth are not essential, they can either be helped out by an orthodontic device or they can be extracted. WILL I HAVE TO HIDE MY SMILE AFTER A TEETH EXPOSURE PROCEDURE? Although a tooth extraction procedure may make you feel conscious of your smile, there will be little to see from those looking on from outside. People often worry about unsightly bits of bone being exposed or metal contraptions filling the mouth. However this is not the case, with careful oral surgery, you will be able to continue to confidently smile. There will not be any unsightly exposed bone or socket visible. He received his undergraduate degree in biological sciences from Florida A&M University (FAMU). Dr. Osibin then completed dental school at the University of Michigan receiving his Doctorate of Dental Surgery (DDS) in May 2013 and was awarded the Chalmers J. Lyons Academy of Oral Surgery Award during graduation. Dr. Osibin was also a National Health Institute (NIH) research fellow who was published in the Journal of Dental Research for his work in stem cell isolation from alveolar bone. Upon completion of dental school, Dr. Osibin went on to be a clinical instructor and lecturer in the department of oral surgery at the University of Missouri – Kansas City (UMKC) School of surgery. He then went on to complete his Oral & Maxillofacial residency with UMKC, receiving several awards throughout his training including the Peer Leadership Award, top score on the in-service exam and induction into the Mosby Surgical Society for surgical excellence. While in residency, he completed several certifications including pediatric life support (PALS), advanced cardiac life support (ACLS) and advanced trauma life support (ATLS). Dr. Osibin is passionate about helping and healing the local and international communities having done several mission trips and volunteering his services to Jamaica and Honduras. Dr. Osibin’s interests include adult & pediatric anesthesia, dental implants, wisdom teeth removal, hard and soft tissue grafting, oral & maxillofacial pathology and reconstructive surgery, temporomandibular joint (TMJ) disorders and facial trauma. Our Bay Area OSM offices offer modern anesthesia services to ensure you have little to no discomfort during your procedure. Depending on the specific operation, your surgeon may provide one or more of the following anesthesia options. LOCAL ANESTHESIA Local anesthesia is an injection that numbs the tissue in the surgical area. Your surgeon may use this before a tooth extraction or in combination with another type of sedation for more advanced surgery. NITROUS OXIDE Nitrous oxide, which is also referred to as laughing gas, helps you relax while remaining conscious. You wear a small mask over your nose and inhale the nitrous oxide-oxygen mixture throughout your procedure. The effects wear off soon after your mask is removed. INTRAVENOUS SEDATION Intravenous sedation allows you to enter a dreamlike state, in which you’re not quite conscious or unconscious but floating comfortably in between. Your surgeon can customize the strength of intravenous sedation to meet your needs. GENERAL ANESTHESIA General anesthesia usually combines intravenous and gaseous sedatives to put you to sleep for the duration of a surgical procedure. When you have anesthesia, you will drift off to a comfortable, relaxed state or slip into unconsciousness and will stay that way throughout your surgery. Frequently Asked Questions WILL I WAKE UP OR BE AWARE DURING GENERAL ANESTHESIA? It is very unlikely that you will be aware of anything during general anesthetic. Allergic reactions are possible with almost any drug. You will be monitored throughout your procedure to see how you are reacting to the anesthesia. If it is noticed that you are reacting badly, then the operation can be stopped. It’s important to let your surgeon know ahead of the surgery if you have any allergies to any other drug. After a tooth extraction a socket preservation procedure is used to keep your jawbone healthy and strong. Also known as a bone graft, it is often completed at the same time as your extraction. A healthy jawbone not only helps you keep the full shape of your face, but can be done in anticipation for dental implant surgery in the future. Once your tooth is extracted, with it comes bone loss. Not only does part of your bone get damaged as the tooth comes out, without a tooth there to stimulate the bone, its density begins to diminish. Many times after a tooth extraction, your oral surgery provider will recommend a dental implant to replace your tooth. However, in order for a dental implant procedure to be successful, your surgeon needs to make sure your jawbone is strong enough to hold the implant. A socket preservation procedure is used to build back the density of your jawbone. What is the Socket Preservation process like? During this procedure your oral surgeon places a bone graft into the socket at the site of your tooth extraction. Bone grafts can come from the following materials: A bone source from your body Bone from a human donor Sourced from an animal (i.e. cows) Synthetic material Once the bone graft is placed into the empty socket, your oral surgery provider will cover it with a thin membrane and then stitch your socket shut with dissolvable sutures. It takes some time for your jawbone to grow around the graft so your provider will have you return to the office after a few months for scans to check your bone’s density. Is the Socket Preservation process painful? Since most of the time the bone grafting occurs right after an extraction, you’ll still be feeling the numbing effects of the anesthetic. Therefore, a socket preservation procedure is no more painful than the extraction that came beforehand. If you decide on a socket preservation after the fact, your surgeon will use anesthetic or sedation before cutting your gum to show the bone site for the procedure. There might be slight discomfort afterward for a day or two, but over the counter pain medication will most likely be all you need. Frequently Asked Questions CAN A BONE GRAFT PREVENT DRY SOCKET? Dry socket is a very painful condition that can occur after extraction. The blood clot that forms over the socket can sometimes get dislodged, exposing sensitive nerve endings beneath. A bone graft will be placed in the socket and that can help prevent the nerve endings from being exposed. WHY IS A BONE GRAFT NECESSARY AFTER AN EXTRACTION? It is common to experience some bone loss after an extraction. This happens particularly in the height and the width of the alveolar ridge. The alveolar ridge is the name given to the part of the jaw that is found under the gum line where both the upper and lower teeth attach. When we bite and chew, the force exerted on our jaw bone stimulates new bone growth. Shrinking can occur over time because the jaw bone no longer receives stimulation from the teeth in that area. It is well documented that when a bone graft is performed after an extraction, the bone loss is less. It is also a good idea to have a bone graft because if in the future you decide to have a dental implant, your jaw bone will be ready to support it. IS BONE GRAFTING NECESSARY FOR DENTURES? If you plan to use dentures after a tooth extraction, you may wonder if it is really necessary to have a bone graft. There are many benefits of having a bone graft, even if you are not planning to have a dental implant. Your jaw bone will be stronger after having a bone graft. This will give your dentures a strong foundation to fit on to. A bone graft will also help provide a smooth surface for your dentures, making it easier for the dentures to fix to. This will reduce any sliding that you could experience if you chose not to have a bone graft. Before being fitted for certain dental prosthetics, like dentures, you may need an alveoloplasty to contour your jaw so your prosthetic will fit comfortably. This procedure is also often performed after having multiple teeth extracted to remove any bone spurs and contour the jawline. Call or schedule an appointment online today. Why do I need an alveoloplasty? Alveoloplasty is a surgical procedure performed at the Bay Area OSM offices to reshape and contour your jawbone. It is often needed before being fitted for dentures so that they can fit snuggly. After a tooth is extracted, it will leave a hole. After the gum heals you will feel highs and lows on the jawline. This can result in poorly fitting dentures. The high points may rub against the dentures causing sore spots. Some who have been missing teeth for many years may have a reduced alveolar ridge and therefore not have enough of a jaw bone for the dentures to fit onto. Other times an alveoloplasty is used if you have bone spurs on your jaw, especially after a tooth extraction. It is especially important to help extraction sites heal properly and to make sure your gum tissue doesn’t become raised afterward. Apart from smoothing the jawbone, alveoloplasty can be used to speed up the healing process after multiple teeth are extracted. Usually, when a tooth is removed, the provider leaves the hole open and a blood clot fills the hole. The gum eventually closes over the hole. With alveoplasty, the gums are stitched together, closing the hole. This speeds up recovery and reduces blood loss because the socket is not left open. It may be important for some patients to heal faster, such as those with cancer. Before someone receives radiation treatment to the head or neck, any decayed teeth will need extracting. This is because during radiation treatment salivary glands can dry up and blood flow is reduced, making the decay worse. Once the decayed teeth are removed, the sockets need to heal before the radiation treatment can begin. Alveoloplasty can be used in this situation to speed up the recovery so that the radiation therapy can begin. Even if a single tooth is removed, it can be used. What is involved in an alveoloplasty procedure? Alveoloplasty can be done in conjunction with your tooth extraction, or after the fact, and is a minimally invasive surgery. First, your Bay Area OSM provider numbs the area with a local anesthetic and possibly provides additional sedation. Next, the gum tissue is cut open revealing the bone beneath. The bone is trimmed away and shaped using a series of files and a handpiece. Sometimes a dental drill can be used. While this is happening, the site will be irrigated to push away excess debris. Finally, your surgeon checks your jawbone for smoothness and your gums are stitched shut with sutures. What is the recovery like after an alveoloplasty surgery? Recovery after alveoloplasty surgery isn’t much different than after a standard tooth extraction. Right after the procedure your surgeon will have you bite down on gauze until all bleeding has stopped. Once home, be prepared to eat soft foods for a few days and stay away from anything overly hot, spicy, and citrusy. Do not eat any crunchy foods as they can reopen the surgical site.
