From its inception, the organization focused on providing educational opportunities and transitional employment pathways for youth and adults facing barriers such as limited English proficiency, unstable housing, unemployment, immigration status, and involvement with the justice system. A major milestone came in 2009 with the opening of our main office on Story Road. This expansion strengthened our ability to deliver comprehensive, community-based services in a centralized and accessible location. Our work emphasizes prevention, early intervention, and individualized pathways that help participants achieve long-term self-sufficiency. Annually, ConXión to Community supports hundreds of youth, families, and adults in Santa Clara County advancing its mission to promote equity, stability, and self-sufficiency. ConXión serves youth through afterschool academics, mentoring, and pro-social activities, while families participate in parent education, family engagement activities, and case management. ConXión also provides workforce and basic-needs support to unhoused or underemployed individuals by connecting participants to transitional and permanent jobs. ConXión keeps the community engaged by providing 20,000+ meals and/or supplemental food bag annually, along with critical hygiene services. These outcomes reflect ConXión’s long-term impact in strengthening families, supporting youth success, and helping individuals move toward economic stability. Through ConXión participants have access to the below. However, through our partnership referrals can be made to any of the 19 other FRCs. Fun and educational programs like music and movement classes and story times Workshops to support parents and caregivers Nutrition, health and wellness programs Connections to local resources including health insurance, food assistance, housing assistance, and baby supplies including formula and diapers Indian Health Center (IHC) The Indian Health Center was formed to ensure the healing and wellness of American Indians, Alaskan Natives and the greater community by providing high quality, comprehensive health care and cultural services. They offer our clients assistance by helping complete Medi-cal applications, workshops, and educational materials for health and wellness to empower our clients to make positive lifestyle changes. The IHC also provides resources for and participates in all Family Night activities. Silicon Valley Second Harvest Food Bank In addition to being a weekly distribution site, we access “shopping” days three times weekly, allowing us to provide food during off “food distribution” days to families in need. PATH Making It Home A PATH outreach case manager has office hours at ConXión every Thursday. PATH provides onsite, wrap-around supportive services to formerly-homeless residents in permanent supportive housing and field-based supportive services to vulnerable renters. All programs are designed to end homelessness in Santa Clara County so participants can rebuild their lives and move into the permanent homes we all deserve. UnidosUS UnidosUS is a nonprofit, nonpartisan organization that serves as the nation’s largest Hispanic civil rights and advocacy organization. These sessions will help parents and caregivers better understand child development, discipline approaches, effective communication, and how to manage emotions. Classes will be tailored based on the age of the child(ren) and the specific needs of each family, offering practical tools, techniques, and resources that can be applied at home. Parenting classes and workshops will be held weekly and will include specialized curricula for fathers, mothers, and combined parent groups. Each session will begin with a sit-down dinner to encourage shared family time and strengthen family bonds. To ensure families have consistent access to support, ConXión will offer rotating curriculum sessions twice per month and provide an ongoing weekly parent support group. We focus on families who are disproportionately represented within the child welfare and juvenile justice systems, throughout Santa Clara County. Cultural Brokers act as the “bridge” between Social Services and/or Probation Department. Families referred to ConXión are in need of support to get them through issues reported to Social Services CPS (Children Protective Services). Participants must be referred through Social Services. Our work emphasizes prevention, early intervention, and individualized pathways that help participants achieve long-term self-sufficiency. Annually, ConXión to Community supports hundreds of youth, families, and adults in Santa Clara County advancing its mission to promote equity, stability, and self-sufficiency. ConXión serves youth through afterschool academics, mentoring, and pro-social activities, while families participate in parent education, family engagement activities, and case management. ConXión also provides workforce and basic-needs support to unhoused or underemployed individuals by connecting participants to transitional and permanent jobs. ConXión keeps the community engaged by providing 20,000+ meals and/or supplemental food bag annually, along with critical hygiene services. Ollin means to move and act now with all your heart. To experience the Ollin factor, we have to get “All In”! OUR HISTORY ConXión to Community, formerly known as the Center for Training & Careers, was founded in 1977 as a 501(c)3 by community leaders committed to addressing systemic inequities impacting underserved populations in Santa Clara County. From its inception, the organization focused on providing educational opportunities and transitional employment pathways for youth and adults facing barriers such as limited English proficiency, unstable housing, unemployment, immigration status, and involvement with the justice system. A major milestone came in 2009 with the opening of our main office on Story Road. This expansion strengthened our ability to deliver comprehensive, community-based services in a centralized and accessible location. With increased capacity, ConXión broadened its programs to address the complex and interconnected challenges our participants face, including chronic truancy, substance use, housing instability, relationship violence, peer pressure, harassment, gang involvement, unemployment, and school disengagement. As our reach and impact grew, ConXión continued to evolve. In 2014, the organization refined its mission and vision to more clearly reflect its role as a community anchor and advocate. This shift emphasized our commitment to addressing root causes of inequity and challenging the over-criminalization of historically marginalized communities, while expanding pathways to education, employment, and long-term stability. Today, ConXión is serving Santa Clara County through education, workforce development, youth programs, behavioral health, and social services. ConXión remains grounded in its founding purpose while continuing to adapt to the changing needs of the community—providing culturally responsive, holistic services that promote opportunity, resilience, and self-sufficiency. Serving middle school and high school students from low-income, immigrant, and first-generation families, ConXión’s programs are designed to reduce barriers to education, increase school engagement, and promote positive youth development. Programs are delivered through school-based and community-based settings and include afterschool and expanded learning opportunities that provide academic support, tutoring, homework assistance, and enrichment activities. Youth receive guidance in leadership development, goal setting, conflict resolution, and decision-making, helping them build confidence and resilience while developing essential life skills. A core component of ConXión’s youth programming is individualized case management and mentoring. Staff work closely with students and families to address attendance challenges, behavioral concerns, and external barriers such as housing instability, food insecurity, or mental health needs. Strong partnerships with schools ensure alignment with academic goals and early identification of students who need additional support. Bright Futures Alcanzar Bright Futures Alcanzar program supports middle school 8th students’ ability to become socially, emotionally, and academically prepared for high school success. The after-school program will cultivate resilient, connected and thriving youth with evidenced-based programming that will assist with their transition into high school. Classes will be tailored based on the age of the child(ren) and the specific needs of each family, offering practical tools, techniques, and resources that can be applied at home. Parenting classes and workshops will be held weekly and will include specialized curricula for fathers, mothers, and combined parent groups. Each session will begin with a sit-down dinner to encourage shared family time and strengthen family bonds. We focus on families who are disproportionately represented within the child welfare and juvenile justice systems, throughout Santa Clara County. Cultural Brokers act as the “bridge” between Social Services and/or Probation Department. Families referred to ConXión are in need of support to get them through issues reported to Social Services CPS (Children Protective Services). Participants must be referred through Social Services. Serving middle school and high school students from low-income, immigrant, and first-generation families, ConXión’s programs are designed to reduce barriers to education, increase school engagement, and promote positive youth development. Programs are delivered through school-based and community-based settings and include afterschool and expanded learning opportunities that provide academic support, tutoring, homework assistance, and enrichment activities. Youth receive guidance in leadership development, goal setting, conflict resolution, and decision-making, helping them build confidence and resilience while developing essential life skills. A core component of ConXión’s youth programming is individualized case management and mentoring. Staff work closely with students and families to address attendance challenges, behavioral concerns, and external barriers such as housing instability, food insecurity, or mental health needs. Strong partnerships with schools ensure alignment with academic goals and early identification of students who need additional support. ConXión also emphasizes family engagement and cultural responsiveness, recognizing the critical role families play in a young person’s success. Through consistent communication, family activities, and referrals to supportive services, the organization creates a comprehensive support system around each student. Bright Futures Alcanzar Bright Futures Alcanzar program supports middle school 8th students’ ability to become socially, emotionally, and academically prepared for high school success. The after-school program will cultivate resilient, connected and thriving youth with evidenced-based programming that will assist with their transition into high school. These tools can help to increase diversity, promote cultural competence, and enhance organizational credibility and effectiveness of our programs. ConXión builds ongoing, permanent relationships for the purpose of applying a collective vision for the benefit of our community. ConXión offers participants assessments of basic skills, career interests and career ability. Most training programs also provide a two week assessment period that allows both the participant and the instructor time to determine if this is the right training for you. ConXión experience shows that case management services are critical to assist individuals overcome multiple barriers to employment. ConXión Case Managers are experienced in providing assessment and case management services to hard-to-serve individuals, meeting with them on a regular basis to monitor their progress, address any issues that arise, and make adjustments to their Individual Employment Plan, as needed. Staff has strong knowledge of and established relationships with a wide range of resource referral agencies throughout Santa Clara County. Case Managers identify other types of assistance that may be needed by participants, including transportation, uniforms or tools required for employment, and child care. Case Managers are responsible for ensuring that each participant makes progress towards the end goal of program completion, and then of employment, that the participant’s civil rights are not violated and that each trainee upholds ConXión’s standards and follows its regulations. We encourage our staff and program participants to get involved in the community. We sit on community and governmental Boards, Commissions, and Committees.
