[/box] Click here to access grievance/appeal forms Click here to submit a grievance or appeal online The Patients’ Rights Office was created in response to California legislation requiring each county mental health director to appoint patients’ rights advocates to protect and further the Constitutional and statutory rights of mental health care recipients. What the Patients’ Rights Office Does: Represents Consumers at Inpatient Psychiatric Hospitals – Patients’ Rights Advocates travel to psychiatric hospitals throughout Los Angeles County and represent patients who are involuntarily detained. The Advocate prepares consumers for their certification review hearings. The hearings are held to determine if the treatment facility has sufficient cause to hold the patient without his/her consent. Advocates also represent consumers at medication capacity hearings (Riese hearings). These hearings are held to determine whether or not an involuntarily treated patient has the ability to consent to or refuse psychiatric medication. Responds to Inpatient and Outpatient Psychiatric Grievances and Complaints – Patients’ Rights Advocates investigate and respond to grievances and complaints about inpatient and outpatient mental health providers. The complaints can come from consumers, family, friends, and even staff. Advocates also provide other advocacy and mediation services to consumers involving outpatient providers. Provides Residential Advocacy – Patients’ Rights Advocates, along with the State of California Community Care Licensing and the Ombudsman, investigate complaints about licensed board and cares and unlicensed residential facilities where consumers live. They also provide training on residential law and rights to consumers, family and providers. Advocates for Minor’s Rights – Patients’ Rights Advocates specialize in minor’s rights and parental rights/responsibilities. They investigate complaints regarding minors, monitor facilities providing services to minors to ensure compliance with laws and regulations and represent minor’s at hearings. Responds to Long-Term Placement (IMD) Consumer Concerns – Patients’ Rights Advocates work directly with consumers living in IMDs, providers, families, and interested parties. Provides Training & Consultation – Patients’ Rights Advocates provide training and educational presentations on patients’ rights and mental health law to consumers, providers and interested parties. Locates Missing Persons – Assists families and friends to find missing mental health consumers without violating consumer’s privacy or HIPAA regulations. Provides Medi-Cal Beneficiary Publications and Forms – Distributes information explaining Medi-Cal mental health services and grievance forms in the threshold languages. The Patients’ Rights Office Mental Health Client Resource Directory – provides an overview of resources in the Los Angeles area including but not limited to mental health, medical, legal, self help, and housing. It was developed as a tool to be used for self empowerment and advocacy. Feel free to download it and to distribute it to anyone who can benefit from the information it contains. Select Page Home About LACDMH Strategic Plan 2020-2030 LACDMH Strategic Plan 2020-2030 TRANSFORMING THE LOS ANGELES COUNTY MENTAL HEALTH SYSTEM The LACDMH Strategic Plan focuses on the change we need to drive across the system, a system we break down into the three key domains where we interface with clients. The first domain, Community Services , represented by the green circle in the figure, signifies the North Star where we will always prefer to provide services and opportunity; half of the Strategic Plan focuses on community and ways in which proactive and therapeutic resources can be built up across the County. The second domain, the Crisis Care , represented by the yellow ring, includes the intensive care resources needed to help individuals in crisis who are falling out of community. The third domain, Re-Entry Initiatives , is represented by the red ring, where our broad portfolio of re-entry resources (including compelled treatment) is deployed to help clients who have fallen out of community into the “open-air” asylum of the street, the “closed-air” asylum of the jail, and the personal asylum of deep isolation. The people and processes that guide our work across all three domains create our ever-present department Infrastructure , which is represented by the square and circular lines. As LACDMH advances its mission, it is entirely committed to playing a key role as partner and contributor to the County’s broader vision for addressing critical challenges and helping communities thrive. To this end, LACDMH has aligned the goals of its plan with the County’s Strategic Plan to ensure a cohesive response. We invite you to learn more about each of these domains below. Crisis Care Crisis care, reflected in the yellow ring, is defined by our interactions with clients experiencing crises and includes real time services as well as facility-based treatment for rapid stabilization. With a strong crisis system in place, numerous bouts of homelessness and episodes of incarceration (the institutions of our day) can be avoided. Proper function in this domain will require the addition of new staffing capacity, including emergency outreach and triage division reinforcements, and a significant expansion of urgent care, crisis residential (including STRTP), sub-acute and acute treatment beds that together create an impenetrable guardrail around communities to keep our most vulnerable clients out of harm’s way. In short, this portion of the LACDMH Strategic Plan focuses on intensive treatment . Re-Entry Initiatives Institutions refer generally to the “open-air” asylum of the streets or the “closed-air” asylum of the jails, neither of which is an acceptable place for engagement and care, let alone habitation. Given the epidemics of homelessness and incarceration in our County, we must be sure that we have in place a robust portfolio of re-entry initiatives designed to care for individuals languishing and subsisting in the institutions and that we provide clear, resourced pathways back to the community. For children, “Institution” refers to prolonged or repeated child welfare involvement and juvenile probation. Mental health services provided include assessments, case management, crisis intervention, medication support, peer support and other rehabilitative services. Services are provided in multiple settings including residential facilities, clinics, schools, hospitals, juvenile halls and camps, mental health courts, board and care homes, in the field and in people’s homes. Special emphasis is placed on addressing co-occurring mental health disorders and other health problems such as addiction. The department also provides counseling to victims of natural or manmade disasters, their families and emergency first responders. The Director of Mental Health is responsible for protecting patients’ rights in all public and private hospitals and programs providing voluntary mental health care and treatment, and all contracted community-based programs. The Director also serves as the public guardian for individuals gravely disabled by mental illness, and is the conservatorship investigation officer for the County. Wellbeing and Recovery Outcomes Our aim is to help our clients and families to: Achieve their recovery goals; Find a safe place for them to live; Use their time in a meaningful way; Have healthy relationships; Access public assistance when necessary; Weather crises successfully; and Have the best possible physical health. Service Recipients DMH’s services to adults and older adults are focused on those who are functionally disabled by severe and persistent mental illness, including those who are low-income, uninsured, temporarily impaired, or in situational crises. Services to children and youth are focused on those who are seriously emotionally disturbed and diagnosed with a mental disorder. They include wards or dependents of the juvenile court, children in psychiatric inpatient facilities, seriously emotionally disturbed youth in the community, and special education students referred by local schools and educational institutions. No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income. Select Page Home Court Programs CARE Court Information and Resources CARE Court Information and Resources CARE Court – A New Mental Health Resource Fostering Recovery and Empowerment People in our community experience severe mental illness and may not receive regular help, often resulting in homelessness and substance use. Their loved ones and other caregivers often struggle to find lifesaving support and assistance. Los Angeles County’s Community Assistance, Recovery, and Empowerment (CARE) Court, which helps people with untreated schizophrenia and other associated psychotic disorders receive treatment and services for their health and well-being. Participants can receive many kinds of support to promote their recovery and well-being, which may include counseling, medication, housing options, social services, and others. Rather than cycling through jails and emergency rooms, CARE Court gives vulnerable individuals and those who care for them another path to access key services that can help keep them safe. Family members, roommates, clinicians, and others can petition the court to seek approval for this program. The Los Angeles County Department of Mental Health (LACDMH) is pleased to offer CARE Court as part of an extended menu of programs to provide mental health assistance for those who need it. CARE Court is currently accepting petitions (required forms for this process are available on LA Court’s CARE website ). Residents and family members can also access other LACDMH programs, services, and resources here , by calling our 24/7 Help Line at (800) 854-7771, or calling/texting the 988 Suicide & Crisis Lifeline . Service providers who would like to refer a client to CARE Court may do so using this form . Who is Eligible to Participate? The CARE program is meant to help adults experiencing specific types of severe, untreated mental illnesses. To be eligible for the CARE program, participants must be: 18 years or older Diagnosed with schizophrenia, bipolar disorder 1 with psychotic features, or other psychotic disorders Currently experiencing behaviors & symptoms associated with that severe mental illness Not receiving ongoing voluntary treatment Please take the time to learn about eligibility BEFORE moving forward with the official petition process. If someone you know needs help, there are many resources out there to keep them safe and support their well-being. How Does the CARE Process Work? STEP ONE – Referral The process begins when a family member, roommate, clinician, or other eligible person files a CARE Act petition referring someone who needs help to the CARE program. STEP TWO – Assessment The court appoints a legal representative and will ask to include a client-chosen supporter to help the potential participant navigate the process. A clinical team will meet with the client in the community to determine whether they are eligible to take part in the CARE program. STEP THREE – Developing a CARE Plan If a person is deemed eligible for the CARE program, a CARE team (that may include clinicians, case managers, people with lived experience, and others) works with each participant to create a personalized treatment plan (that can include housing options, medications, counseling, and other services) and help with supportive decision making (which could include topics like a Psychiatric Advance Directive—a legal document that can be used by a person with mental illness to say what their treatment preferences are in case of a crisis). Review hearings will be scheduled periodically to assess the participant’s progress and ensure services are offered quickly. STEP FOUR – Completion Participants receive services for up to one year, which the court can renew for an additional year, if needed. At 11 months, CARE Court holds a one-year status hearing to determine whether the plan’s services and resources should be extended for up to 12 more months. STEP FIVE – Next Steps The participant can continue with treatment, supportive services, and housing options in the community to support long-term recovery, even after they are no longer a CARE participant. Their supportive-decision-making materials can also stay in place for any future use. How Can I Start the CARE Court Process? The process begins when a family member, roommate, clinician, or other eligible person files a CARE Act petition . A judge determines whether the potential participant meets the criteria for the CARE program. If the participant is accepted into the CARE program, their CARE team works with them to develop a plan that will provide services tailored to their needs. The plan will provide the individual services for up to a year, which can be extended by a year if chosen or needed. No one could have predicted that the Lunar New Year’s Eve mass shooting in Monterey Park at the Star Ballroom Dance Studio would be another in a tragic wave of mass shootings in our state, preceded by a mass shooting in Central California just days before, and followed by horrendous incidents of gun violence in Half Moon Bay, Oakland, San Diego, and the Beverly Crest neighborhood of Los Angeles this past weekend. This has all occurred alongside the tragic loss of Black lives at the hands of law enforcement, most recently Keenan Anderson, Tyre Nichols, and Anthony Lowe, Jr., as we collectively observe Black History Month. This is not the new year message I had wanted to share. I wanted to convey a cheerful message about both the calendar new year and the lunar new year. But these recent events deserve space and conversation. A growing body of research tells us that mass shootings and traumatic events are associated with a wide variety of adverse psychological outcomes in survivors as well as proximal community members. There is also evidence that these events affect people who are indirectly exposed. In other words, these events affect us all. In the short term, they may increase our fears, reduce our sense of safety and general sense of well-being. And these adverse effects are magnified when there have been multiple trauma exposures, and vulnerabilities in the community, or variables that increase identification with the victims such as shared race, religion, geography or ideals. I think it is immensely important in times like these to pay attention to how you are feeling. Our impulse is to just push through, but we also need to honestly acknowledge how we are doing and what we might need in this moment. These recent events have affected everyone differently, so it is important to take the time to check in with yourself, and check in with those around you. Maybe you are not sleeping well or you do not have much of an appetite. Maybe you are feeling a bit anxious, angry, sad, or even numb. Talk to someone—loved ones, a professional, the anonymous 9-8-8 crisis line . You don’t have to face this alone. And at the end of the day, we are stronger together, and together we can get through this. I want to give a very special thanks to the Psychiatric Mobile Response Team staff who had been on site since the Monterey Park tragedy at the crisis center set up at the Langley Senior Center. They worked long hours providing kindness, care and mental health resources to the families of victims and community members. Thank you, Winnie Wong, Alice Xiao, Henna Cho, Venus Ngai, Garrett Horne, Carlos Pineda, Maria Martinez, Cesar Urizar, and their fearless leader, the unstoppable Miriam Brown. They have all been amazing. We also have a whole team of staff from across all service areas who have taken shifts to be at Langley this week to provide support as the seniors return to the center. According to Chinese tradition, this year of the rabbit is likely to be calm and gentle, bringing energy that will help those looking for a more balanced life. These metrics reflect our commitment to transparency, accountability, and timely access to care for Medi-Cal beneficiaries. Data Points Responses 1. Senate Bill 43 was passed and signed in 2023 to significantly update California’s conservatorship laws for the first time in more than 50 years. The bill broadens the authority of the Lanterman-Petris-Short (LPS) Act, which governs involuntary detention, treatment and conservatorship of people with behavioral health conditions. The law, which goes into effect in January 2026 in Los Angeles County, expands the definition of “gravely disabled.” Gravely disabled now includes people with a mental health disorder, a severe substance use disorder, or a co-occurring mental health disorder and severe substance use disorder who are unable to provide for their basic personal needs for food, clothing, shelter, personal safety or necessary medical care. SB 43 also expands the array of testimony that can be submitted into conservatorship proceedings, requires counties to consider less restrictive alternatives when conducting conservatorship investigations, and expands state reporting requirements. What is the Lanterman-Petris-Short (LPS) Act? The LPS Act is a 1967 California law that governs involuntary detention, treatment and conservatorship of people with behavioral health conditions. The act established a system of short-term involuntary holds in designated facilities and set requirements for the path to temporary and permanent conservatorship. What is the Definition of "Gravely Disabled"? The new definition of “grave disability” requires specific criteria to be met. A person, as a result of a mental health disorder, a severe substance use disorder, or a co-occurring mental health disorder and a severe substance use disorder, must be unable to provide for their basic personal needs for food, clothing, shelter, personal safety, or necessary medical care. Examples include: Personal Safety: The inability to survive safely in the community without involuntary detention: Placing oneself in harm’s way of traffic. An individual incapable of defending themselves against ongoing victimization due to a lack of awareness of their vulnerability that presents an imminent danger to the individual. Severe impaired judgement resulting in risky situations that threaten the person’s own life or those of others. Necessary Medical Care: Due to mental health and/or severe substance use disorder, involuntary detention is necessary to prevent serious deterioration of an existing physical medical condition which, if left untreated, is likely to result in serious bodily injury: Pregnant women so severely impaired with a mental illness and/or severe substance use disorder, neglecting prenatal care, follow-up care, homeless, excessive substance use, often victimized and placing fetus at risk. Signs of significant malnourishment (loss of weight or dehydration) that place the individual’s health at serious risk. Wound care and infection issues that are likely to lead to loss of limb or life if not treated. Connecting Factors That Identify Grave Disability Legal criteria, plus “cause and effect” connecting them. Select Page Our Services Veterans Children Transition Age Youth Adults Older Adults Countywide Services Disaster Services Public Guardian Home Our Services Child Welfare Division Child Welfare Division The Child Welfare Division (CWD) was created as a direct result of the Katie A. lawsuit, which alleged that youth in contact with the County’s child welfare system were not receiving timely mental health services to which they were entitled. CWD provides oversight to ensure that youth who are in or at-risk of entering the child welfare system receive specialty mental health services in an individualized and timely manner. In 2017, the Continuum of Care Reform (CCR) Assembly Bill 403 was passed and reformed the child welfare system. Youth who must live apart from their biological parents do best when cared for in a committed and nurturing family environment. CCR ensures services and supports are tailored toward the ultimate goal of maintaining placement stability and permanency. Select Page Home Our Services Older Adult Services Older Adult Services GENESIS Older Adult Program Full Service Partnership Mental Health Service Providers Mental Wellness Series   Welcome To Older Adult Services Mental health is essential at every stage of life, and there are LACDMH services in place to address our population throughout the lifespan. Older Adults age 60+ receive mental health services in our DMH directly-operated and our DMH-contracted adult clinics, countywide. Services in these agencies typically involve screening and assessment, case management, individual/group/family treatment, and crisis intervention, as needed. These services are available to help Older Adults manage their mental health as well as improve their quality of life. LACDMH’s directly-operated GENESIS Countywide Older Adult program is specifically for Older Adults age 60+ who are living with severe mental illness and are homebound. GENESIS offers field-based outpatient mental health services to those unable to come to a clinic due to impaired mobility, frailty, or other limitations. It reforms behavioral health care funding to prioritize services for people with the most significant mental health needs while adding the treatment of substance use disorders (SUD), expanding housing interventions, and increasing the behavioral health workforce. BHSA guides counties to look at their whole behavioral health systems of care in their community planning process. It also enhances oversight, transparency, and accountability at the state and local levels. Additionally, the Behavioral Health Services Act creates pathways to ensure equitable access to care by advancing equity and reducing disparities for individuals with behavioral health needs. It is one part of Proposition 1. The second part of Proposition 1, the Behavioral Health Bond, authorizes $6.4 billion in bonds to finance behavioral health treatment beds, supportive housing, community sites, and funding for housing veterans with behavioral health needs. The QI Unit establishes annual Work Plan goals, monitors Departmental activities for effectiveness, and conducts processes for continuous improvement of services countywide and in collaboration with other Departmental Divisions. The structure and process of the LACDMH Quality Improvement (QI) program is outlined in Policy and Procedure 1100.01, “Quality Improvement Program.” The QI Unit works to ensure that the quality and appropriateness of care delivered to consumers meets or exceeds local, State, and Federal service standards. The QI Unit is organized and implemented in support of an organizational culture of Continuous Quality Improvement (CQI) that: Fosters hope, wellbeing, resilience and recovery Reduces disparities Promotes consumer and family involvement Enhances cultural competency Integrates the treatment of mental health and substance use disorders with physical healthcare The purpose of the design and implementation of countywide QI activities is to ensure an organizational culture of continuous self-monitoring through effective strategies, best practices, and activities at all levels of the system. The QI team works to engage and support the Regional Quality Improvement Committee (QIC) members in QI processes related to the QI Work Plan, specific Performance Improvement Project (PIP) activities, and additional QI projects conducted at the regional level. Services Mental health services provided include assessments, case management, crisis intervention, medication support, peer support, psychotherapy and other rehabilitative services. Services are provided in a variety of settings including residential facilities, clinics, schools, hospitals, juvenile halls and camps, mental health courts, board and care homes, in the field and in people’s homes. We also provide counseling to victims of natural and man-made disasters, their families and emergency first responders. No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income. Our services to adults and older adults are focused on those who are functionally disabled by severe and persistent mental illness, including those who are low-income, uninsured, temporarily impaired, or in situational crises. Services to children and youth are focused on those who are emotionally disturbed and diagnosed with a mental disorder. They include wards or dependents of the juvenile court, children in psychiatric inpatient facilities, seriously emotionally disturbed youth in the community, and special education students referred by educational institutions. The Director of Mental Health is responsible for protecting patients’ rights in all public and private hospitals, programs providing voluntary mental health care and treatment, and all contracted community-based programs. The Director also serves as the public guardian for individuals gravely disabled by mental illness, and is the conservatorship investigation officer for the County. Mental Health Services Act The Mental Health Services Act (MHSA), created by the passage of Proposition 63 in 2004, has expanded the partnerships and capacity of the mental health system in Los Angeles. DMH continues to work with a diverse group of community stakeholders to effect the historic expansion of mental health services funded by MHSA. Select Page Cultural Competency Unit Cultural Competency Unit Cultural Competency Committee Cultural Competence Glossary of Terms Cultural Competence Resources Home Cultural Competency Unit Cultural Competency Unit The CCU promotes a collective sense of shared responsibility for the delivery of services that address health inequities and meet the needs of Los Angeles County’s cultural diversity inclusive of racial, ethnic, linguistic, age, gender identity, sexual orientation, socioeconomic status, degree of physical and cognitive ability and disability, spirituality and religious beliefs, and lifestyle choices among others. The CCU is housed within the departmental Anti-Racism, Inclusion, Solidarity, and Empowerment (ARISE) Division and has reporting responsibilities to LACDMH’s executive management, the California Department of Health Care Services (DHCS), and the Los Angeles County Board of Supervisors (BOS). The CCU functions are based on the following premises: Cultural competence involves honoring the cultural and linguistic preferences of persons receiving services from LACDMH in order to support their process of healing, recovery, resiliency, and wellbeing. LACDMH clinical and administrative programs are most effective when they operate as an integrated network to provide services that take into consideration the cultural and linguistic needs of the communities served. The development and delivery of culturally and linguistically responsive, accessible, appropriate , equitable and effective services require collaborative approaches with constituents, community-based organizations and the community as a whole. Becoming a culturally competent, inclusive, and responsive system of care is a developmental process. It involves strategic interventions that focus on eliminating mental health disparities and address service gaps. It requires listening, learning and connecting LACDMH with the voice of constituents, their family members, and communities. CCU Functions The CCU is actively involved in assisting the LACDMH practitioners, provider sites, clinical and administrative programs, and management to integrate cultural and linguistic competence in the service delivery framework. The most salient functions of the CCU include: Enforce ment of Federal, State and County regulations pertinent to cultural and linguistic inclusion and responsiveness . Development of policies and procedures (P&Ps) that highlight the essential role of cultural competence in the provision of relevant and meaningful services. Monitor ing of service delivery from a quality improvement perspective to ensure that services are planned, delivered, monitored , and evaluated for cultural and linguistic relevance and appropriateness. Develop ment of mechanisms to collect, organize, and track LACDMH data pertinent to Los Angeles County demographics, service utilization , and departmental interventions to reduce mental health disparities. Conduct ion of system-wide cultural competence organizational assessments. Development of the LACDMH annual Cultural Competence Plan report in accordance to Title IX and DHCS’ Cultural Competence Plan Requirements. Coordinat ion of the cultural and linguistic competence content for triennial Medi-Cal Audits and annual External Quality Review Organization System Reviews. Develop ment and deliver y of cultural competence trainings and presentations at various departmental venues, LACDMH-sponsored conferences, cross-county and State level summits, community-based organizations, and the community at large. Responsiveness to inquiries and requests from executive management, DHCS, and the BOS. Provi sion of technical assistance to LACDMH executive managers, Service Area and Countywide Chiefs, as well as clinical and administrative programs. Provi sion of administrative consultation and support for the Cultural Competency Committee (CCC). Interfa cing with various stakeholder groups such as CCC, consumer groups, and Quality Improvement Council to ensure that their voice is included in the CCU projects. The CCU Program Manager/Ethnic Services Manager is the lead for the LACDMH Language Assistance Services and is actively involved in the Departmental Language Access Plan. The CCU staffing includes the two LACDMH Sign Language Specialists who facilitate American Sign Language (ASL) services for clinical appointments such as psychotherapy and psychiatric appointments , departmental cultural events, and departmental stakeholder groups. The Unit consists of the following Teams: Medi-Cal Certification & Credentialing Manager: Norma Cano, Psy.D. Certifies the Department’s directly operated and contracted Medi-Cal SMHS providers in accord with all applicable County, State, and federal requirements. Credentials directly operated practitioners providing SMHS. Designates facilities to accept patients on involuntary holds under LPS act. Policy & Technical Development Manager: Jennifer Hallman, LCSW, MPA Develops and revises policies, forms, manuals, and bulletins associated with the provision of Medi-Cal SMHS by directly operated and contracted providers. Oversees the adherence of providers to Medi-Cal requirements and provides direction and training. Assists in the development and refinement of the Department’s electronic health record system (EHRS) and uses data from the EHRS to assist in oversight and consultation activities. Serves as a liaison between the County and the State Department of Health Care Services (DHCS) including participation in DHCS policy workgroups. Oversees County’s Network Adequacy and Access-to-Care performance. Responds to DHCS audits of the County’s outpatient and inpatient SMHS. Initiates DHCS waivers of licensure for unlicensed contracted provider clinicians. Provider Support & Review Manager: Nikki Collier, LCSW Provides training, consultation and technical assistance regarding the documenting and claiming of Medi-Cal SMHS with an emphasis on contract providers. Reviews the clinical records of contracted providers to identify areas where support is needed.
