Fevers Flu and cold symptoms Vomiting or diarrhea Sore throats and earaches Headache that doesn’t go away Chronic pain Sprains, strains, and minor fractures Wounds needing stitches Many other conditions are severe enough to require a same-day appointment, especially those that cause acute symptoms, like a urinary tract infection or skin infection. When should you go to the emergency room? Services Provided Physical/Check Up Well Man or Well Woman Exam An exam on a healthy person with no symptoms to look for hidden disease and give advice on healthy behavior. Getting the right preventive services at the right time can help you stay healthy by preventing disease or by detecting a health problem at an early stage when it may be easier to treat. Does Not Include: Medication refills, evaluation of new symptoms, or management of chronic problems. Insurance Coverage: Covered by most insurance. Not covered by Medicare, but may be covered by secondary insurance if Medicare patient has that. Follow Up/Chronic Care Visit A visit to review chronic problems, adjust medications, check laboratory monitoring, or refill prescriptions. Administrative Physicals Camp physicals, school physicals, work physicals, insurance physicals—this type of visit requires the doctor to fill out a form. Services include whatever is necessary to fill out the form. Does Not Include:  Medication refills or evaluation of new problems. Insurance Coverage:  Sometimes covered by insurance, but often not. In cases where a form is needed a short time after an appointment (i.e., child had an annual Well Child Exam to update immunizations and three months later needs a form filled out for camp), the doctor may be able to complete the form without another visit. However, there is a fee to fill out the form and that fee is usually not covered by insurance. Coding for Your Insurance The difference between medical services is sometimes hard to understand. As a physician practice, EMC does not control these differences. Our reimbursement is based on the coding system that insurance companies use to describe medical work. For example the coding system classifies a physical exam and medication refill as two separate services. Think of it this way: you wouldn’t expect to make a restaurant reservation for three people, and then bring a fourth person for free. Or have a plumber come to fix a leaky sink and install a dishwasher for no extra charge. The same is true for medical services. Frequently Asked Questions Can I combine services? For your convenience, the provider may combine two types of services at one visit — a Well Man Exam and a chronic care visit to refill blood pressure medication, for example. Many factors determine if we are able to do this at a particular visit, how well a medical condition is under control, a new more urgent problem is present and the general health of the patient is all a consideration. However, because these are two different services, there would be two separate charges. We may collect two co-pays at the time of your visit and submit two charges depending on your insurance benefit. How do you decide what to focus on during an exam? Providers are taught to examine literally thousands of physical signs—from knee reflexes to heart sounds. When conducting an exam, the doctor relies on your personal medical history in combination with any current symptoms or problems. Sometimes, specific symptoms such as a heart murmur or rash will be the actual diagnosis. Your medical history is always the first important consideration. How do you decide what tests and services I should have? Providers are specially trained to evaluate the usefulness of tests and preventive services and help you decide which ones are right for you. When you carry extra weight, your heart has to work harder, and your joints begin to wear down from the added stress. Being overweight also has a widespread effect throughout your body as fat cells produce hormones and other substances that regulate your metabolism and influence organs. When these biochemicals get out of balance due to extra fat cells, your risk for chronic diseases such as diabetes, heart disease, and high blood pressure increases. Why is it so hard to lose weight? If you’ve struggled to lose weight, or you lose weight only to regain it, you’re not alone. Many factors influence your ability to lose weight, and they’re not easy to overcome. For example, many people overeat when they’re stressed, and they struggle to break the cycle. Hormones also work against your best efforts. One hormone, leptin, promotes fat burning. But many people who gain weight develop leptin resistance, which means your body doesn’t respond to leptin adequately. As a result, you continue to store fat rather than burn it for energy. Another hormone, ghrelin, makes you feel hungry. After you lose weight, levels of ghrelin spike by 23%, making it hard to fight the hunger. If you can stick with your diet for a year, your ghrelin levels return to normal. Does professional weight-loss support help? If you use a mail order (3 month supply) service (ie. Express-Scripts , RX Solutions ), contact them at least 1-2 weeks prior. Ask your pharmacist to send us a prescription refill request by fax . Once received by us, your physician may authorize a refill, if appropriate. If you have not been seen by your physician recently, refills may be denied, especially if you have high blood pressure or diabetes. Blood tests and physical may be advised. Please allow 72 business hours for responses to all refill requests. We do not refill any controlled substance medication without an office visit. We apologize for any inconvenience.  Welcome The issue of "medication refills" is often a sensitive one for patients. When people visit the office for a medication refill, it's really a follow-up on the medical condition, and intended with our patient's best interest in mind. A prescription medication is one that has the potential for complications and we believe the prescriber has a responsibility to supervise the patient.  As such, we ask that you schedule an appointment to have your prescriptions filled. In some cases, we deem there to be too much risk to approve refills without seeing you first. This can even be true of maintenance medicine. At your scheduled office visit, Dr. Kashani will discuss appropriate monitoring intervals for your medications, and will make sure you have enough refills until the next planned office visit. The initial visit takes some additional time because of the new electronic agreements. You will have the opportunity to check-in in advance if we have your e-mail, via a link sent to you 2-3 days before your scheduled appointment. The intial log-in takes about 20 minutes, but after that it's much much faster. Appointment Length Appointments are officially either 15 minutes or 30 minutes, which is what the insurance will cover. We meet with patients longer if needed. We take your health seriously and health can be complex. As such, we like to limit appointments to discussions of one or two issues in order to adequately cover each issue. Waiting Times We’re sorry if you ever have to wait! We consider your time just as important as ours, so we try hard to get scheduling right. We do find that at least one contribution to increased wait times is when patients bring multiple health issues to an office visit. Please try to limit your visit to the most pressing issues, and schedule a second appointment for additional concerns. All of that being said, we never truly know what patient needs will be or what emergencies may arise. And we need your help! If everyone arrives on time and is prepared for their appointment, It makes it run more smoothly for everyone else. Thanks!   Photo ID and Insurance Card All patients will be required to present a valid driver's license or photo ID, their current medical insurance card and co-pay at every office visit. Patients without verifiable health insurance will be responsible for the payment for that visit. Refill Policy The issue of "medication refills" is often a sensitive one for patients. When people visit the office for a medication refill, it's really a follow-up on the medical condition, and intended with our patient's best interest in mind. A prescription medication is one that has the potential for complications and we believe the prescriber has a responsibility to supervise the patient. As such, we ask that you schedule an appointment to have your prescriptions filled. In some cases, we deem there to be too much risk to approve refills without seeing you first. This can even be true of maintenance medicine. At your scheduled office visit, the provider will discuss appropriate monitoring intervals for your medications, and will make sure you have enough refills until the next planned office visit. When your medicine bottle shows: "No refills remain" or "Contact your physician for a refill," these should serve as a reminder that you need to come in and be seen. Some medications are best monitored with laboratory testing in addition to an office visit. These include cholesterol lowering medication, blood pressure and diabetes medication, and thyroid supplements. If you think you may need lab work prior to the office visit, have it drawn at least 48 hours before the appointment so that we can use the appointment time more efficiently. It’s always exciting to take a long-overdue vacation or journey abroad. But remember: to get the most out of your trip, there are many things your provider will need to know to help you the best advice possible. Here are a few tips to help you prepare for your trip: Plan ahead Some vaccinations take time to begin working. If you’re traveling abroad, research the vaccinations you will need and when you should receive them. Also, think about the following questions before you visit your provider: When are you traveling, and how long will you be at each location? Where are you traveling? Which countries are you visiting? If you are visiting more than one country, in what order will you visit these countries? Where within the country or countries will you be traveling? Are these destinations urban or rural? Where will you be staying (sleeping) during your trip (such as hotel with air conditioning, screened cabin, open-air tents, or with family or friends)? What activities will you be doing during your trip (such as hiking, backpacking, scuba diving, sightseeing, visiting family or friends, etc.)? Have you traveled internationally in the past? Where did you go? When did you travel? What vaccinations have you had before? Do you have or could you get a written record of your previous vaccinations? When did you have these vaccinations? How many doses did you have of a particular vaccine? (for example, some vaccines, such as the hepatitis A and B vaccines or the measles/mumps/rubella (MMR) vaccine, require multiple doses for long-term protection) Have you had any allergies or reactions to vaccines? Do you have any other allergies (for example, medicines, foods, or environmental)? In particular, do you have an allergy to eggs, latex, yeast, mercury, or thimerosal? What is your medical history and current health status? Be sure to tell your provider about past illnesses and surgeries, chronic health problems, or other underlying medical conditions. What medications are you currently taking or have you taken in the past 3 months? Is your immune system suppressed? If you are a female, Are you pregnant now? Take extras Whether you wear glasses or contacts or you take prescription medication, pack as much as you need to get you through your trip – and then some. Not all medication accessible in the United States is readily available overseas. Pack spare glasses and extra medication in a carry on bag in case your luggage is lost or stolen. Prepare a medical summary. If you have a complex or chronic health condition, ask your primary care provider or other physician to prepare a medical summary of your situation. English is the international language of medicine, and most providers abroad will be able to read and comprehend your provider’s notes. Check your health insurance: Contact your health insurance provider to determine if it covers you overseas. For most tests, we offer same week appointments – no unnecessary traveling to testing facilities or waiting days for your results. Tests performed in our offices include: Echocardiogram Carotid Doppler Ultrasound Vascular Extremity Testing Abdominal Aorta Tests we refer out include: Nuclear stress test Stress echocardiogram Holter Monitor/Event Monitor EKG   Echocardiogram Echocardiogram is a Doppler ultrasound of the heart to evaluate heart function. Echocardiogram is also referred to as transthoracic echocardiogram (TTE), Doppler ultrasound of the heart, or surface echo. This test uses a painless wand that is moved over the chest and heart area and creates a digital image showing blood flow and structures of the heart.   Stress Echocardiogram Stress Echocardiogram, also called a stress echo, combines an echocardiogram with a stress test to show how the heart works during “stress” or exercise. The echocardiogram uses sound waves to produce an image of the heart on a monitor before and during exercise. Stress echocardiography helps cardiologists diagnose coronary heart disease. Carotid Doppler Ultrasound A Carotid Doppler ultrasound can measure the flow of blood within the heart and arteries as well as evaluate valve movement. Helping to detect blockages, an arterial ultrasound can screen for clots or blockages that might raise your risk for a stroke, peripheral artery disease of the legs, or an aortic aneurysm; a common and potentially fatal disease. Holter Monitor / Event Monitor A Holter or event monitor records the heart’s rhythm to diagnose abnormal rhythms of the heart. The device, which looks like a small box or pack is attached to leads on the chest and worn for a period of time to record the heart's rhythm. Extremity Testing Extremity testing can detect peripheral artery disease. An ultrasound painlessly creates pictures of the blood flowing through the arteries, showing any blockages that could potentially cause a blood clot or stroke. Your immune system identifies harmful substances that enter your body then destroys them. The first time it recognizes a dangerous substance, it produces antibodies so the immune system can react immediately the next time your body detects it. An allergy begins when your immune system reacts to a substance that’s harmless, labeling it as an enemy. Then every time that substance -- the allergen -- enters or touches your body, your body releases histamines, causing an allergic reaction. You can develop allergies to many different allergens. Some of the most common include: Contact allergens: Poison ivy, soap, latex Airborne allergens: Pollen, mold, dust Insect stings: Bees, wasps, hornets Pet allergies: Dogs, cats, pets with fur Medications: Many possible medications; penicillin is the most common Food allergies: Peanuts, milk, eggs, wheat, soy, fish What symptoms occur during an allergic reaction? Allergies often cause nasal congestion and ear infections in young children, which can make it hard to distinguish the symptoms from a common cold. Otherwise, the most common allergy symptoms include: Runny nose Congested nose Itchy nose, eyes, or mouth Swollen tongue or face Watery eyes Sneezing Skin rashes, hives, scaling, or swelling at site of insect sting Upset stomach, cramps, nausea, vomiting, or diarrhea (food allergies) What should you know about food allergies? One in 13 children have a food allergy, and the only treatment is to avoid the food that’s responsible. Peanuts, milk, and eggs are the most common food allergens in children, but eight types of foods are responsible for 90% of allergies. The other five foods are wheat, tree nuts, soy, shellfish, and fish. Food allergies can trigger a severe reaction, called anaphylaxis, that impairs breathing, lowers blood pressure, and may be life-threatening. Peanuts, tree nuts, fish, and shellfish are most likely to cause anaphylaxis. When should you get tested for allergies? You must get an allergy test to identify your allergens before you can begin treatment. Consider allergy testing when you have: Chronic sinus infections or difficulty breathing Shortness of breath, wheezing, or a tight chest No relief from over-the-counter medicines Allergy symptoms lasting several months Allergies that hamper normal life Asthma attacks that worsen or occur more frequently If you suspect you or anyone in your family has an allergy, call or email the office to book an appointment for an evaluation and testing. So, please come in for a free estimate where our staff will go over your medical records and explain in detail what you are still required from the USCIS.  SERVICES: Immigration Physical Examination Basic Physical Examination Vaccination status verification Tuberculosis Blood Test (Quanteferon) RPR (syphilis) lab test Gonorhea (urine) testing Certified and Sealed I-693 form Patient copy of I-693 form Vaccines or Xrays will be an additional charge. Additional services that might be require such as Chest X-Ray, blood titers and vaccinations are not included in the above price, but they are offered and can be completed at our office for an additional fee. If you have had all your vaccinations and do not have proof of receiving the vaccinations, we can check your blood titers to check for immunity. Please note that if the blood work shows you are not immune you will need to pay the additional fee for the vaccinations.  If you have a PPO insurance and have not had your yearly physical, we can use your insurance to cover the blood work, TB test and/or vaccinations. Your PPO insurance does not cover the cost of the  I-693 examination.  We do not accept HMO or Medi-Cal insurance for the immigration examination.   IMPORTANT NOTES: What the Patient Needs to Bring to the First Appointment: • Currently active Government issued ID with picture. Passport preferred. Driver’s license, Military ID, and Green Card also acceptable. • Medical Records translated into English by an official translator (if needed). These Immigration Physical Information Sheet records should include treatment history for Tuberculosis or any other infectious diseases and chest x-ray if you have one. • Immunization Record translated into English by an official translator (if needed). • Mental Health Records translated into English by an official translator (if needed). • The first visit may take 30 minutes to 1 hour or more for the first visit so please plan accordingly. The entire process can take 2-6 days depending on tests and vaccinations. Please complete the forms below prior to the first appointment. • If you require a translator, please bring one with you. Elite Medical Clinic will not provide you with one or reimburse for the cost. • Please bring Form I-693 to your exam and fill out section one in black ink. Do not sign section 2. The government requires this for completion of your immigration physical. Tuberculosis Update: 1. The Tuberculosis blood test is required for applicants age 2 and older. The results are usually ready in  72 hours after the blood is drawn. If the rblood test result is positive then a Chest X-ray will be required at an additional fee. We cannot accept TB tests from other providers. This must be performed by the Civil Surgeon. Applicants require proof of vaccinations. The civil surgeon will review your vaccination history to determine if any other vaccines are require and/or if you need any blood titers (laboratory tests that proves if a person has immunity or antibodies for a particular disease). Additional vaccinations and blood titers are offered at an additional fee. After the immigration physical examination is completed, Dr. Kashani will certify Form I-693 and the intake form (see below) with the test results of the immigration exam and vaccination history in a sealed envelope to present to the Department of Homeland Security.  DO NOT OPEN THE SEALED ENVELOPE. Turn in the envelope with your immigration application. A copy of your I-693 report will be provided for you to keep after results have been discussed with the examining doctor. The examination will only be valid up to a year after the immigration exam date. If it has been more  than one year, we will need to repeat another examination, blood and urine test as well as the TB tests.
