Single or Multiple Implants Implants are versatile. If you are only missing one tooth, one implant plus one replacement tooth will do the trick. If you are missing several teeth in a row, a few strategically placed implants can support a permanent bridge (a set of replacement teeth). Similarly, if you have lost all of your teeth, a full bridge or full denture can be permanently fixed in your mouth with a strategic number of implants. Advantages Over Dentures or Bridges Conventional bridges and dentures are not fixed to the bone, and can therefore be unstable. This can make it difficult to eat or smile with confidence. Implants not only look more natural, but feel and act more like normal teeth, with a stronger biting force. And because they don't directly rely on neighboring teeth for support, implants don't compromise the health of your natural teeth. In fact, bridges are only expected to last seven to ten years, even less with root canals, whereas implants will typically last a lifetime. Post-Treatment Care Consider your replacement teeth to be the same as natural teeth. They require the same daily brushing and flossing, and the same amount of regular checkups. I am nervous about being unconscious. What levels of sedation are available to me? There is no need to be nervous about having sedation dentistry at our practice. We are licensed by the American Dental Association, and you will find that we are both respectful and professional at all times. Sedation dentistry is closely regulated by law, and there are three sedative states at which your dentist can administer your treatment: mild sedation, moderate sedation, and deep sedation. Mild Sedation — Anxiolysis is the lightest form of sedation dentistry and is often used for patients with mild anxiety, longer procedures, or more complex situations. Mild sedation is usually administered orally. You remain awake or very sleepy throughout the entire procedure and are able to breathe on your own, but you will feel a great sense of relaxation. Patients typically recover from anxiolysis sedation within a few hours after the procedure is complete. Nitrous oxide inhalation (laughing gas) is another form of mild to moderate sedation that results in relaxation during treatment. Moderate Sedation — Used for patients with moderate dental anxiety and for patients who need longer or more complex procedures, conscious sedation often refers to the use of light IV sedation. With conscious sedation, you will remain awake throughout your procedure, but will be in a deep state of relaxation. It is recommended that patients who receive conscious sedation have a parent, spouse, or friend accompany them to appointments because it can take several hours for the sedative to wear off and driving may be unsafe. Deep Sedation — Patients receiving deep sedation go between consciousness and unconsciousness during their dental procedure. Patients often have no recollection of the treatment and are unable to respond to commands even if they are awake at times during the procedure. It is recommended that patients who receive deep sedation have a parent, spouse, or friend accompany them to appointments because it can take several hours for the sedative to wear off and driving may be unsafe. How are sedatives administered? Inhalation — Nitrous oxide, or laughing gas, is the most frequently used method for easing mild to moderate anxiety. Recovery is quick so you can resume your normal activities immediately. Oral — An extremely common technique for dental sedation is oral sedation. It is easy and does not require the use of needles. You will be given a prescription pill which, taken about an hour before your appointment, will have you fully relaxed by the time you arrive at our office. And never give your child a pacifier that's been dipped into anything sweet. - Start brushing baby teeth as soon as the first tooth appears. Routine baby dental care should also include massaging the gums with a clean gauze pad. When all teeth have erupted, floss at least once a day to help prevent the buildup of dental plaque. - Wean your baby off thumb-sucking if he or she is still doing so by age four. Otherwise, it can cause overcrowded or crooked teeth. - Consider a combination of fluoride treatment and dental sealants, thin plastic coatings applied on baby molars to keep dental plaque from accumulating. Be sure to schedule your routine checkup. It is recommended that you visit the dentist every six months. At what age should I start taking my child to see the dentist? The American Academy of Pediatric Dentistry (AAPD) recommends that children first see a dentist as early as six months of age and no later than one year of age. During this time, your child's baby teeth will be coming in and your dentist can examine the health of your child's first few teeth. After the first visit, be sure to schedule regular checkups every six months. How often should I see the dentist? Children, teens, and adults should all see the dentist for a regular checkup at least once every six months. Your doctor will help determine how often you should visit the dentist for regular checkups. What is a cavity? A cavity is a small hole that forms inside the tooth because of tooth decay. It is also recommended that you use a soft-bristled toothbrush and toothpaste that contains fluoride when you brush your teeth. You should spend at least a minute on the top teeth and a minute on the bottom teeth, and remember to brush your tongue; it will help keep your breath smelling fresh! When should I change my toothbrush? Your toothbrush will eventually wear out, especially if you are brushing your teeth twice a day for two to three minutes each time. Your dentist recommends that adults and children change their toothbrush every three months. Gum disease is one of the top reasons for tooth loss in adults, and because it is virtually pain-free, many patients do not know they have the disease. During each regular checkup, your dentist will check for signs of periodontal disease by measuring the space between your teeth and gums. What causes gum disease? Gum disease is caused by a buildup of plaque (a sticky form of bacteria that forms on the teeth). If the plaque is not removed (by flossing, brushing, and regular dental checkups), it will continue to build up and create toxins that can damage the gums. Periodontal disease forms just below the gum line and creates small pockets that separate the gums from the teeth. Periodontal disease has two stages: gingivitis and periodontitis. Gingivitis — This is the early stage of gum disease, when the gums become red and swollen, and bleed easily. At this stage, the disease is treatable and can usually be eliminated by daily brushing and flossing. Periodontitis — If left untreated, gingivitis will advance into periodontitis, and the gums and bone that support the teeth will become seriously and irreversibly damaged. Gums infected with periodontitis can cause teeth to become loose, fall out, or be removed by a dentist. Certain factors can increase a patient's risk of developing periodontal disease, including: Smoking or using chewing tobacco Diabetes Certain types of medication such as steroids, anti-epilepsy drugs, cancer therapy drugs, calcium channel blockers, and oral contraceptives Bridges that no longer fit properly Crooked teeth Old fillings Pregnancy While it is possible to have periodontal disease and not know it, some symptoms can include: Gums that bleed easily Red, swollen, tender gums Gums that have pulled away from the teeth Persistent bad breath or bad taste Pus between your teeth and gums Permanent teeth that are loose or separating Any change in the way your teeth fit together when you bite Any change in the fit of partial dentures Treating Gum Disease Treatments for gum disease can vary depending on the severity of each individual case. The American Academy of Pediatric Dentistry recommends... Children should visit the dentist by their first birthday. It is important that your child's newly-erupted teeth (erupting at six and 12 months of age) receive proper dental care and benefit from proper oral hygiene habits right from the beginning. When New Teeth Arrive Your child's first primary or baby teeth will begin to erupt between the ages of six and 12 months, and will continue to erupt until about age three. During this time, your child's gums may feel tender and sore. To help alleviate this discomfort, we recommend that you soothe the gums by rubbing a clean finger or a cool, wet cloth across them. You may also choose to make use of a teething ring. When your child has finished teething, you can expect a total of 20 primary teeth. Your child's primary teeth are shed at various times throughout childhood. Permanent teeth begin erupting at age six, and continue until age 21. Adults have 28 permanent teeth (32, including wisdom teeth). Adopting Healthy Oral Hygiene Habits As your child's teeth erupt, be sure to examine them every two weeks, looking for lines and discoloration that may be caused by decay. Remember that sugary foods and liquids can attack a new tooth, so take care that your child brushes after feeding or eating. We recommend brushing four times a day for optimal oral hygiene: after breakfast, after lunch, after dinner, and at bedtime. Brushing can be fun, and your child should brush as soon as the first tooth arrives. When a baby's tooth erupts, parents should brush the tooth with a soft-bristled toothbrush and a pea-sized amount of toothpaste. For children younger than two, do not use fluoride toothpaste unless advised to do so by your dentist or other healthcare professional. We suggest reviewing proper tooth brushing procedures with your child. Flossing is also a part of good oral hygiene habits, and your doctor will discuss with you the right time to start flossing. If you notice signs of decay, contact your dentist immediately. Preventing Tooth Decay with Regular Checkups Tooth decay is caused by sugars left in your mouth that turn into an acid, which can break down your teeth. Children are at high risk for tooth decay for a simple reason: many children and adolescents do not practice regular, good oral hygiene habits. Proper brushing and flossing routines combined with regular dental visits help keep tooth decay away. Your child should visit the dentist every six months for regular dental cleanings and checkups. We recommend fluoride treatments twice a year along with cleanings to keep teeth their strongest. Tooth sealants are also recommended because they "seal" the deep grooves in your child's teeth, preventing decay from forming in these hard-to-reach areas. Sealants last for several years, but will be monitored at your child's regular checkups. In-Office Teeth Whitening The first professional option is commonly referred to as in-office whitening. This procedure is performed in a dental office. During an appointment, your dental professional will apply a whitening gel to the surface of your teeth. The entire office visit generally takes about 90 minutes and you will be able to see results immediately. At-Home Teeth Whitening Tray Whitening Tray bleaching is a type of professional whitening that uses custom-made trays and whitening gel only available through your dentist. The whitening gel is placed in custom-fitted trays that fit perfectly over your teeth. As the peroxide in the gel breaks down, hydroxyradicals help whiten stained teeth. Unlike strips, this process whitens all your natural teeth and may contain ingredients intended to provide maximum comfort. At-home professional tray whitening generally takes ten to 14 days, although you may notice results in as little as three to five days. Paint-On Teeth Whiteners There are many over-the-counter teeth whitening options, and one type of readily accessible whitening products is commonly referred to as a “paint-on” teeth whitener. Paint-on whiteners can be purchased over-the-counter and generally are less expensive and less effective than whitening strips or professional options. Paint-on teeth whiteners consist of a gel that is applied to the tooth using a small brush. Paint-on whitening gel hardens into a film that coats the teeth, and dissolves in the mouth. As with any teeth-whitening regimen, it is wise to consult with a dental professional before use. Teeth-Whitening Strips Teeth-whitening strips are thin, flexible plastic (polyethylene) strips with a thin film of hydrogen peroxide on one side. Whitening strip kits come with two types of strips: strips for the upper teeth and strips for the lower teeth. The whitening agent is applied by placing the strips across your teeth and gently pressing the strips to ensure contact with all your teeth. Teeth-whitening strips are typically worn for 30 minutes a day, twice a day. The duration of treatment will vary. Whitening Toothpaste and Mouthwash The least expensive options are whitening toothpastes and mouthwashes. Designed to have “whitening” capabilities, toothpastes and mouthwashes work to remove surface stains using mild abrasives. However, unlike the materials used in professional whitening procedures, these products do not lighten the actual color shade of the tooth. How Orthodontic Treatment Works Orthodontic appliances are made of metal, ceramic, or plastic. They may be removable or they may be brackets bonded to the teeth. By placing a constant, gentle force in a carefully controlled direction, braces slowly move teeth to the proper position. Gone are the days when a metal band with a bracket was placed around each tooth. You can now choose from brackets that are clear or metallic, and in some cases, the color of your appliance. Wires are far less noticeable, and the latest materials are designed to move teeth faster with more comfort. Today, any age is a great age to wear braces! Duration of Treatment Depending on several factors, including age, treatment method, and how minor or severe your case is, treatment time varies and can last between one and three years. A big factor to a successful treatment is you, the patient! The more involved and diligent you are, the quicker and more efficient your treatment will be. For children, receiving interceptive, or early treatment, can also help provide a quicker treatment time later on in life. Types of Braces Traditional Metal Braces Traditional metal braces are the most common type of braces and are more comfortable today than ever before. Made of high-grade stainless steel, metal braces straighten your teeth by using metal brackets and archwires. With metal braces, you have the option of adding colored elastics (rubber bands) for a more unique and colorful smile. Self-Ligating Braces Self-ligating braces are made from the same materials as traditional braces; however, self-ligating braces do not require the use of elastics, meaning fewer appointments and less friction being placed on the tooth. Self-ligating braces come with traditional metal, ceramic, or clear brackets. They are the same size as metal braces, but they use a specialized clip in place of elastics to help the archwire guide teeth into place. The clip helps reduce the amount of pressure being placed on the tooth and requires fewer adjustments because there are no elastics to replace. Clear (Ceramic) Braces Ceramic braces are made of clear materials and are therefore less visible on your teeth than metal braces. For this reason, ceramic braces are used mainly on older teenagers and adult patients who have cosmetic concerns. While they are visually less prominent, they do require more attention to oral hygiene, as ceramic braces are larger and more brittle than their metal counterparts. For these reasons, ceramic braces tend to be used more on upper front teeth rather than on lower teeth. Clear Aligners Clear aligners are a series of invisible, removable, and comfortable acrylic trays that straighten your teeth like braces. Not only are the aligners invisible, they are also removable; so, you can eat and drink what you want while in treatment. Plus, this helps to make brushing and flossing less of a hassle. The aligners are comfortable and have no metal to cause mouth abrasions during treatment. Gold Braces Gold braces are similar to traditional metal braces. Like traditional metal braces, gold braces are made of stainless steel; however, they are uniquely coated in gold. The gold coating may be considered a more cosmetic option than traditional metal braces. And, the cost of gold braces is surprisingly comparable to most clear braces. Lingual Braces Lingual braces are hidden behind the teeth and are therefore "invisible" when you smile. The ideal candidate for implants is a non-smoker who has good oral health, including a sufficient amount of bone in the jaw, and healthy gums with no sign of gum disease. Single or Multiple Implants Implants are versatile. If you are only missing one tooth, one implant plus one replacement tooth will do the trick. If you are missing several teeth in a row, a few strategically placed implants can support a permanent bridge (a set of replacement teeth). Similarly, if you have lost all of your teeth, a full bridge or full denture can be permanently fixed in your mouth with a strategic number of implants. Advantages Over Dentures or Bridges Conventional bridges and dentures are not fixed to the bone, and can therefore be unstable. This can make it difficult to eat or smile with confidence. Implants not only look more natural, but feel and act more like normal teeth, with a stronger biting force. And because they don't directly rely on neighboring teeth for support, implants don't compromise the health of your natural teeth. In fact, bridges are only expected to last seven to ten years, even less with root canals, whereas implants will typically last a lifetime. Post-Treatment Care Consider your replacement teeth to be the same as natural teeth. They require the same daily brushing and flossing, and the same amount of regular checkups. During dental bonding, a white filling is placed onto your tooth to improve its appearance. The filling “bonds” with your tooth, and because it comes in a variety of tooth-colored shades, it closely matches the appearance of your natural teeth. Tooth bonding can also be used for tooth fillings instead of silver amalgam. Many patients prefer bonded fillings because the white color is much less noticeable than silver. Bonding fillings can be used on front or back teeth, depending on the location and extent of tooth decay. Bonding is less expensive than other cosmetic treatments and can usually be completed in one visit to our office. However, bonding can stain and is easier to break than other cosmetic treatments, such as porcelain veneers. If it does break or chip, tell your doctor. The bonding can generally be easily patched or repaired in one visit. ↑ Back to top Bridges A bridge may be used to replace missing teeth, help maintain the shape of your face, and alleviate stress on your bite. A bridge replaces missing teeth with artificial teeth, looks great, and literally bridges the gap where one or more teeth may have been. Your bridge can be made from gold, alloys, porcelain, or a combination of these materials and is bonded onto surrounding teeth for support. The success of any bridge depends on its foundation — the other teeth, gums, or bone to which it is attached. Therefore, it’s very important to keep your existing teeth, gums, and jaw healthy and strong. ↑ Back to top Crowns Crowns are a restorative procedure used to improve your tooth’s shape or to strengthen a tooth. Crowns are most often used for teeth that are broken, worn, or have portions destroyed by tooth decay. A crown is a “cap” cemented onto an existing tooth that usually covers the portion of your tooth above the gum line. In effect, the crown becomes your tooth’s new outer surface. Crowns can be made of porcelain, metal, or both. Porcelain crowns are most often preferred because they mimic the translucency of natural teeth and are very strong. Crowns or onlays (partial crowns) are needed when there is insufficient tooth strength remaining to hold a filling. Unlike fillings, which apply the restorative material directly into your mouth, a crown is fabricated away from your mouth. Your crown is created in a lab from your unique tooth impression, which allows a dental laboratory technician to examine all aspects of your bite and jaw movements. Your crown is then sculpted just for you so that your bite and jaw movements function normally once the crown is placed. ↑ Back to top Dentures Dentures are natural-looking replacement teeth that are removable. There are two types of dentures: full and partial. Full dentures are given to patients when all of the natural teeth have been removed. Partial dentures are attached to a metal frame that is connected to your natural teeth and are used to fill in where permanent teeth have been removed. Just like natural teeth, dentures need to be properly cared for. Use a gentle cleanser to brush your dentures, always keep them moist when they’re not in use, and be sure to keep your tongue and gums clean as well. ↑ Back to top Extractions There are times when it is necessary to remove a tooth. Sometimes a baby tooth has misshapen or long roots that prevent it from falling out as it should, and the tooth must be removed to make way for the permanent tooth to erupt. At other times, a tooth may have so much decay that it puts the surrounding teeth at risk of decay, so your doctor may recommend removal and replacement with a bridge or implant. Infection, orthodontic correction, or problems with a wisdom tooth can also require removal of a tooth. When it is determined that a tooth needs to be removed, your dentist may extract the tooth during a regular checkup or may request another visit for this procedure. The root of each tooth is encased within your jawbone in a “tooth socket,” and your tooth is held in that socket by a ligament. In order to extract a tooth, your dentist must expand the socket and separate the tooth from the ligament holding it in place. While this procedure is typically very quick, it is important to share with your doctor any concerns or preferences for sedation. Once a tooth has been removed, neighboring teeth may shift, causing problems with chewing or with your jaw joint function. To avoid these complications, your dentist may recommend that you replace the extracted tooth. ↑ Back to top Fillings Traditional dental restoratives, or fillings, may include gold, porcelain, or composite. Newer dental fillings include ceramic and plastic compounds that mimic the appearance of natural teeth. These compounds, often called composite resins, are typically used on the front teeth where a natural appearance is important. There are two different kinds of fillings: direct and indirect. Direct fillings are fillings placed into a prepared cavity in a single visit. Indirect fillings generally require two or more visits. These fillings include inlays, and veneers fabricated with ceramics or composites. ↑ Back to top Fluoride Fluoride is effective in preventing cavities and tooth decay and in preventing plaque from building up and hardening on the tooth’s surface. A fluoride treatment in your dentist’s office takes just a few minutes. After the treatment, patients may be asked not to rinse, eat, or drink for at least 30 minutes in order to allow the teeth to absorb the fluoride. We have helped them be able to go to job interviews with the confidence of a full set of teeth. Delancey Street Foundation is considered the country's leading residential self-help organization for substance abusers, ex-convicts, homeless, and others who have hit bottom. Our average resident has been a hard-core drug addict for sixteen years, abusing alcohol and multiple drugs, has dropped out of school at the 7th grade, and has been institutionalized several times. Many have been gang members; most have been trapped in poverty for several generations. Rather than hire experts to help the people with problems, we decided to run Delancey Street with no staff and no funding. Like a large family, our residents must learn to develop their strengths and help each other. It’s an approach to changing lives that is “against all odds.” We said we were going to take ex-convicts and ex-addicts and teach them to be teachers, general contractors, and truck drivers. They said it couldn’t be done. We said we were going to take 250 people who had never worked and had no skills and teach them to build a 400,000 square foot complex as our new home on the waterfront. They said it couldn’t be done. We said we were going to partner with colleges and get people who started out functionally illiterate to achieve Bachelor of Arts degrees. They said it couldn’t be done. We said we were going to run successful restaurants, moving companies, furniture making, and cafés and bookstores without any professional help. They said it couldn’t be done. We said we were going to do all this with no staff, no government funding, and no professionals. They laughed and said it couldn’t be done. We struggle a lot but we’ve been doing it. For more than 40 years we’ve been developing a model of social entrepreneurship, of education, of rehabilitation, and change that is exciting and full of hope.
