Dr. Hazany describes acne scars as potholes in the street. When fixing a pothole, do you flatten and grind down the surrounding road to the level of the pothole? No! Flattening the rest of the street requires unnecessary work in areas that are not defective, and the street becomes unnecessarily thin, leading to other complications. The proper way to fix a pothole is to fill the pothole and lift it to the surrounding street level. This same logic applies to treating acne scars. Subcision, unlike laser treatment or micro-needling, lifts the scar to the level of the rest of your skin. A hypodermic needle is inserted into the skin to manually break fibrotic strands that tether scar tissue to the subdermal layer. Needles of various sizes are used to mechanically break scar tissue residing in the superficial, middle, or deep layers. Subcision vs. Laser Treatment for Acne Scars Laser treatments, micro-needling, and other treatments that involve vertical movements to flatten and thin the skin to the acne scars’ level are like bringing the rest of the street down to the level of the pothole. Laser treatment is a standard treatment method for acne scar removal performed by many dermatologists. Dr. Hazany agrees that scar reconstruction laser treatment can be useful as a touch-up or for minor unevenness. However, for large areas of scarring (and even smaller areas), Dr. Hazany champions subcision, which avoids unnecessary thinning of the skin and produces a healthier, more robust appearance after treatment. How Does Subcision Work? Subcision is an outpatient procedure that is usually accomplished within a few hours. The patient is administered a local anesthetic, tumescent anesthesia, as well as anti-anxiety medication to ensure comfort during the treatment. Then, a hypodermic needle is inserted into the skin and moved side-to-side to untether the scar tissue from the underlying basement layer. This allows scarred areas to lift to the level of normal skin. This form of “controlled trauma” also stimulates collagen production, which provides support underneath the scarred areas. Subcision can be performed in several layers: superficial-layer, mid-layer, and deep-layer.  Superficial-layer subcision : the skin is infiltrated with a 25-gauge needle right below the epidermis.  Middle-layer subcision : a blunt cannula (usually a 22-gauge cannula slightly larger in diameter than a 25-gauge needle) is used to penetrate deeper into the dermis.  Deep-layer subcision : a special metal tool, called the Hazany Liberator, is inserted into the deepest parts of the dermis to separate the toughest scars from the underlying tissue.  Since middle-layer and deep-layer subcision are more invasive than superficial-layer subcision, an “introducer”-needle is inserted into the skin to create a small opening for the cannula or Hazany Liberator to be inserted. Taylor Liberator Subcision Deep-layer subcision typically uses a special surgical tool called the Taylor Liberator. The Taylor Liberator is a long, blunt instrument with notches at the end which facilitate the breaking of fibrous bands that tether acne scars down. To perform deep-layer subcision, a small incision is first made near the hairline to insert the Taylor Liberator into the deepest layers of the dermis (the superficial subcutaneous plane). As the tool is moved back and forth, the deepest and robust scars are transected. Deep-layer subcision using the Taylor Liberator is a highly effective technique that allows for complete untethering of fibrotic bands and long term results when performed correctly. Because acne scars can vary in depth, deep-layer subcision using the Taylor Liberator is generally most appropriate when patients have scars that are tethered to the deeper layers below the skin. Patients with boxcar scars or rolling scars typically benefit the most from deep-layer subcision. Hazany Liberator Subcision Since the development of the Taylor Liberator, many advances have been made in the field of subcision. Notably, the Hazany Liberator is a new and improved tool that is only available at the Scar Healing Institute . In our hands, this proprietary, patented device has shown better outcomes for atrophic, rolling, and boxcar scars accompanied by a much shorter recovery time than other instruments. Post-Treatment Care It is normal for the first 24-48 hours after the procedure to feel slightly sore in the treatment area(s).  Skin discoloration on the treated site(s) is common but does not always occur.  Mild swelling in the treatment site(s) typically lasts less than 48 hours; in some cases, swelling may persist for up to a week. Blood-tinged oozing is expected after the first hour of treatment. Do not touch the treatment area(s), as this may lead to infection.  For facial subcision: you may resume regular washing or shower activities after 24 hours unless instructed otherwise.  For subcision on the body: no prolonged bathing or soaking treatment area (s) in the first 24 hours. For swelling and soreness, apply a cold, wet cloth to the treatment area in intervals of 10-15 minutes. You may resume normal activities within 48 hours. Do NOT pick scabs that develop in the treatment area. Do NOT wear makeup or other cosmetic products in the treatment area(s) for 24 hours.  It is normal for the treatment area to be sore up to a week.  Apply minimum SPF 50 sunscreen on treatment area(s). Apply a thin layer of Vaseline/petroleum jelly after showering. Acne Scar Subcision Cost The cost of acne scar subcision varies depending on how many layers of subcision you require (e.g., deep layer, middle layer, or superficial layer) and the number of areas you plan on subcising. The long-lasting results of subcision mean you do not have to keep paying money for maintenance treatments. Dr. Hazany emphasizes that side effects can be avoided if used properly and dosed by an expert. Antibiotics Doxycycline and minocycline are the most common antibiotics used to treat acne and rosacea. They are anti-inflammatory and serve to reduce the red bumps that are common with pustular acne types. Dr. Hazany does not believe in long-term use of antibiotics as they have many side effects, including gastrointestinal upset, sun sensitivity, and pigmentary disorders, just to name a few. Dr. Hazany prefers to limit the use of antibiotics and phase them out as the treatment plan progresses. Hormonal Modulation  Hormonal modulation drugs can be an effective way to reduce hormone induced acne. Laser resurfacing is an effective, non-invasive acne scarring treatment that produces dramatic results with minimal downtime. Our office uses state of the art Fotona SP Dynamis Pro Erbium YAG lasers to deliver safe, long-lasting results for patients of all skin types. What are the different kinds and uses for lasers? At our clinic, we use the Fotona4D laser, which has both non-ablative (Nd:YAG) and ablative (Er:YAG) settings. Nd:YAG lasers emit shorter wavelengths of concentrated light, which allows them to penetrate deeper into the skin to target veins, hair follicles, and melanin, where they reside. On the other hand, Er: YAG lasers emit longer wavelengths of light, specifically infrared light, to ablate and thus remodel the top layers of skin. Er:YAG lasers are thus more applicable for scar treatment. Are lasers effective for my acne scars? Although visible improvement can be seen with ER:YAG laser treatment, lasers essentially “sands down” healthy skin around acne scars to match the level of the pitted scars. This allows a more even appearance. As a result, our firm belief is to avoid laser treatments as a first line treatment for acne scars. Instead, we believe that laser treatments are best used as a final “touch-up” treatment that smooths out your skin after your scar revision and healing during the phase system. This way, laser treatments can be used as an effective refining and enhancing treatment to fine tune your skin’s appearance to your desired goals. Moreover, laser treatments can be effective for patients with non-acne scars, notably hypertrophic scars from burns, surgery, or other forms of trauma to the skin. In these cases, we would use a higher ablative Er:YAG setting to remodel the raised skin lesions to flatten them to the level of the surrounding healthy skin. However, in cases where the scar is severely raised and continually growing, as seen with keloid scars, lasers may not be the most viable treatment option.  What are the risks of laser treatment? Although we use a mild setting for our laser, some risks include temporary redness, swelling, and irritation. In rare cases, infection may occur following treatment. All these risks are mitigated or entirely prevented with careful use and proper aftercare. For people of color, lasers can cause post-inflammatory hyperpigmentation. As a countermeasure, patients should avoid UV exposure and apply generous amounts of sunscreen following treatment, especially during the spring and summer months. How long do the results last for laser treatment? Patients will start to notice results 3 to 6 months after treatment. Afterwards, the results will persist anywhere between 3 to 5 years. Some patients may need a touch-up during that time period for results to last longer. What is the cost for laser treatment? The cost of laser treatment will vary depending on the severity and type of scar involved. Some scars will require more highly ablative settings than others, which would influence the cost of treatment. At your consultation and throughout the treatment process, we will discuss laser options depending on how much treatment is required to minimize the appearance of your scars as much as possible. It is a minimally invasive laser procedure that is becoming increasingly more popular among not only celebrities but also other people who are simply looking to get a subtle, natural-looking boost to their appearance. How did it get its name? The Red-Carpet Laser is perfect for getting you ready for your (you guessed it!) “red-carpet” event. In fact, most individuals take only 5 to 7 days to fully recover from the procedure and look their best for special occasions.  When it comes to cosmetic treatments, a lot of people immediately think of facelifts, botox, and fillers. While those are great treatments, not many people are comfortable going under the knife or having substances injected into their face. Some people just want a subtle, natural-looking boost to their appearance. How does the Red-Carpet Laser work? The Red-Carpet Laser works by delivering controlled thermal damage to the outermost layer of the skin, called the epidermis. This removes the damaged skin cells, which stimulates the growth of new, healthy skin. This procedure also induces collagen production in the dermis to thicken the skin, helping to reduce signs of aging, such as fine lines and wrinkles. What type of laser is used for the Red-Carpet Laser? There are a variety of different lasers that can be used for the Red-Carpet treatment. These lasers can either be fully ablative (wipes off the entire top layer of the skin) or fractional (resurfaces only a “fraction” of the skin in a pixelated pattern). Additionally, the red carpet can be performed with either the CO2 or Er:YAG laser.  There are several considerations to make when deciding which laser to use for your Red-Carpet Laser. If you want to completely renew the top layer of your skin, then the fully-ablative option may be more desirable. However, the downtime may be more extensive. Fractional lasers, on the other hand, remove only a portion of the damaged top layer, sparing some healthy skin in between to expedite the healing process. Finally, the decision to use CO2 and Er:YAG is also dependent on your skin type. CO2 induces more thermal damage to the skin to produce more noticeable results but has a higher risk of hyperpigmentation for people with darker skin types. What should I expect during the procedure? The procedure typically takes around 30 to 60 minutes, depending on the extent of the area being treated. Typically, individuals choose to get their face and neck treated, occasionally adding on scalp and/or chest. The treatment areas will be numbed with topical anesthetic prior to the procedure to minimize discomfort. After numbing, the laser is meticulously run across all of the areas. After the procedure is done, you may notice some redness, swelling, and tenderness, which typically subsides within a couple of days. Benefits of the Red-Carpet Laser The Red-Carpet Laser is an effective treatment for promoting a more youthful and radiant appearance. However, the redness slowly tapers off over the course of several weeks. Pain and discomfort : After the anesthetic wears off, patients may feel a burning or stinging feeling on their skin, which is also normal. Patients usually manage discomfort well with over-the-counter pain medications and icing. Dyspigmentation : In rare cases, patients may experience darkening or lightening of their skin after the procedure. It is important to stay out of the sun and follow strict aftercare measures to minimize the risk of permanent pigmentary changes to the skin. How often do I need to get the Red-Carpet Laser? Many people choose to undergo the Red-Carpet Laser once or twice a year. People with mild signs of aging can see substantial results with consistent treatments. Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition. HS signs include small and painful bumps under the skin, especially in areas with many hair follicles and sweat glands. HS is also common in areas where there is frequent skin-to-skin contact such as the buttocks, groin area, inner legs, underarm area, and under the breasts. Women are three times more likely to develop HS compared to men. Experts estimate that nearly 200,000 Americans suffer from moderate to severe HS. What Causes Hidradenitis Suppurativa? Typically, HS results from an immune system overreaction that creates an imbalance of inflammatory proteins in the body. These proteins coalesce in the deeper layers of the skin and increase pressure under the skin. As pressure builds, abscesses containing pus develop. These abscesses can be painful and may rupture, increasing the chances of infection. HS's common misconceptions include it being contagious, sexually transmitted, and the result of improper hygiene; HS is not contagious, not an STD, and not the result of poor hygiene.  How is Hidradenitis Suppurativa Treated?
