Dr. Lowenstein came to us with an established brand identity already, but it didn’t quite scream sophistication like he had hoped. For him, though the individual elements of his brand- logo, colors, imagery, fonts, and words- had the makings of modern, when they coalesced on his website, the impression was anything but. To achieve the conservative yet modern, simple yet sophisticated look that Dr. Lowenstein desired, the design didn’t need more; it needed less. Less effort. Subtler imagery. Fewer features. And simpler choices. For the sophisticated, minimalism reigns supreme. Reimagining​ ​the​ ​patient’s​ ​experience From bedside manners to the state-of-the-art surgery center, we wanted prospective patients to vicariously experience their journey with Dr. Lowenstein directly from his website. If we could offer patients the information and media necessary to simulate their own experience, not only could we reduce friction and apprehension throughout each phase of their journey, but we could better set expectations, too. Using Dr. Lowenstein’s preexisting library of multimedia content as a starting point, we went to work architecting the site so that visitors could experience each phase of the patient journey from any page. The marriage of search, social, and content Before launching our content marketing program, our first objective was to define Dermacare’s business goals and target patient. Since Dermacare wanted to increase consultations for a full suite of non-invasive face and body rejuvenation treatments, we made the decision to take a needs-based approach to our positioning. In other words, we would meet the broad needs of a narrowly defined audience – in this case, women over 40 years old. Not only did this demographic have more disposable income, but they also showed a higher interest in aesthetic treatments than their younger counterparts. Knowing that 97% of patients aren’t ready to purchase on their first visit, the next step was to transform Dermacare’s outdated website into a marketing hub – a place where we could reliably funnel the new search and social traffic we were about to acquire and expect visitors to convert to patients over time. After establishing our website infrastructure, we created a weekly blog series to function as the anchor of our editorial strategy, driving traffic from search and promoting Dermacare’s thoughts and expertise on social. Then we supported our blog with a monthly email newsletter to build a list of subscribers that would receive our marketing message every month by choice. 1. Building a hub for discoverability and conversions We had myriad goals for Dermacare’s new website, all that were paramount to our content marketing success: First, since 75% of people judge the credibility of a business based on their website appearance alone, we needed a more contemporary design that better reflected Dermacare’s contemporary business model. Second, the website would serve as the foundation of our SEO, so we needed to establish DermacareSanDiego.com as a reputable authority on all things skin care if we expected to rank first in Carlsbad and Rancho Bernardo. Last, because the website would be the destination for every prospective patient at some point in their journey, not only did it need to work for Google, but it also needed to nurture prospects along their journey from visitor to patient. 2. 21st century storytelling To build trust with our target patients and earn their attention, we used weekly educational blog posts that sold Dermacare’s thoughts, ideas, knowledge, and expertise. More importantly, our blog posts would help deepen Dermacare’s topical expertise and authority on skin rejuvenation, resulting in an increase in search rankings and organic traffic. Blog posts also served as the foundation of our monthly newsletters. With our newsletters, we could align blog post topics with relevant specials and offers and give target patients a valuable resource to subscribe to in exchange for their attention. Since we started blogging for Dermacare in 2015, we’ve accrued 10,635+ pageviews to blog posts alone and over 1,000 new subscribers to our newsletter. 3. The spokes to our hub Because we were targeting the 40+ demographic, we chose Facebook as our primary distribution model for publicising our content, building a following, and driving traffic back to the website. By taking advantage of Facebook’s custom tabs, we transformed Dermacare’s business page into a micro-site, complete with before and after photos, team page, consultation form, and newsletter opt-in. This allowed patients to get the full experience of Dermacare’s message and offerings without ever having to leave Facebook. And with social posts comprised of blog articles, live streaming, educational post, curated news from around the industry, and personal posts from Dr. Birchall and his staff, we were able to triple traffic to dermacaresandiego.com in one year. Results Total two year traffic growth from our content marketing efforts, including referral, social, direct, and organic, rose to above 52,000 visitors in 2016 alone – a 24% increase from when we started . As a result, I’ve sat with my anxiety longer then it deserves, I’ve avoided decisions altogether, and I’ve beaten myself with guilt over poor, impulsive choices made when I didn’t have the willpower to defeat them. Decision fatigue would at times cripple my self-control (see impulsive, irresponsible corner cutting), blind my vision, and whisk away my concentration (somewhere Andy from Headspace was calmly saying, “Your mind can be pretty restless at times. Like a monkey in a cage.” Thanks, Andy. But I couldn’t hear you.). I felt like Simba during the stampede scene: scratching, clawing, and gripping for survival, only instead of wildebeests it was choices trampling me to the ground. I was everywhere but present, paralyzed by choice, suffocated by indecision, and intoxicated with despair. We all know how this ends :/ Though I didn’t know it at the time, I was drifting through the perfect storm of cognitive overload. And it was perfectly predictable. After all, life is a Russian nesting doll of choice, and I was making too many of them. But this story has a happy ending. And so will yours. It has too. Because every day we wake up (choice one) and show up for life where people depend on us: family, friends, colleagues, customers, and even patients. And every day decision fatigue shows up, too ( it’s science, not personal ). Decision fatigue is as human of a condition as happiness or sadness, even though at some point someone tried to convince you that “analysis paralysis” was your problem, not theirs. It’s our problem. And it’s only getting worse. Unfortunately, cognitive depletion, or, as author Barry Schwartz would argue, a “ tyranny of choice ,” affects us all. And there’s only one way to stave off total depletion: to plan for it, because it is coming. Only then can we all be better leaders, loved ones, colleagues, doctors, business owners and providers of value. This article is about the lessons I’ve learned on my journey from a seed of indecision to a bloom of revelation; it’s about finding ways to win the battle of willpower so we can make better decisions, create stronger teams, and lead better businesses; and it’s for anyone who has ever answered the question, “What do you want to eat for dinner?” with “I don’t care.” Let’s go! Mindfulness is the first step toward liberation Decision fatigue, or ego depletion , refers to the escalating mental tax your willpower pays every time you make a decision: the more choices you make each day, the harder the subsequent choices become. So much so that by the end of the day, you’re either descending a 90 ft. wave of impulsive irrationality or dodging decisions like they’re flying wrenches because your brain no longer has the energy to make compromises, analyze trade-offs, or regulate self-control. Delightful, I know. As it turns out, our willpower is finite and exhaustible. And to add calamity to an already crippling phenomenon, we can’t even tell when we’ve reached the end of our rope: No measurable gas tank No quantifiable decision limit No physical fatigue No indication whatsoever that our cognitive reasoning has declined and that we can no longer trust ourselves So we push forward, not knowing what we don’t know, into the abyss of ego depletion, where we frolic with impulsion and drown in indecision. My journey through the abyss started like everyone’s: with a seed of indecision. Then another. And another. Until all at once my life at work and home became a constant crossing of the Rubicon , as it does : new IM marketing strategy, new website build, new house (furnishing a new home is insufferable btw), new *expensive* hobby (I must’ve spent 25K hours researching every new lens, light, and LED screen for my cameras), and a new baby on board (baby registry… just. baby registry.) I’d heard about decision fatigue before and even suspected its presence. But I didn’t approach my days, my months, my projects, my team, or my anything mindful that willpower had a shelf life, that certain choices depleted more cognitive resources than others, or that decision fatigue was lurking around the corner, even if I couldn’t hear it. Since I wasn’t mindful of my fatigue, I couldn’t plan for it. And since I didn’t plan for it, I couldn’t minimize its impact on anyone . As with anything, mindfulness was the first step toward liberation. Longer projects experience higher risks of decision-fatigue Decision fatigue usually happens at the end of the day, when you may still have the will but you no longer have the power . However, it can also occur toward the end of a project, at all times of the day. For example, as we neared the ninth month of our website build , decision fatigue would creep its way into everyone’s mind, body and spirit before the bean water came to a brew. We weren’t paralyzed by indecision, however; we were rejuvenated by decisions it seemed- awful decisions. I like to call this the “Sure” or “Go for it” stage of a project: somewhere between total conniption and DGAF , where the mental fog is so dense that you think it’s a good idea to launch a website after 24 straight hours of work (yep, impulsive irrationality ): “Everything’s good guys. Let’s launch!” “What about mobile though?” “Mobile? … My mobile is in my hand. Where’s yours? “Where’s my hand?” “No- you’re mobile. Nevermind. Ready to launch?” “Sure.” But why does this happen? The longer a project wages on, the more finite our cognitive attention spans become. Ironically, the more time we spend weighing the pros and cons of our options (trade-offs), the more impaired our decision-making becomes, too: “As it turns out, your willpower is like a muscle. And similar to the muscles in your body, willpower can get fatigued when you use it over and over again. Every time you make a decision, it’s like doing another rep in the gym. And similar to how your muscles get tired at the end of a workout, the strength of your willpower fades as you make more decisions.” – James Clear, behavioral psychologist And at the end of every project (including ours) there comes a deluge of decisions that you should have already made but didn’t ; instead, you spent the entire time up until now putting them off or pondering them. If we can approach our projects knowing that every end marks the inevitable culmination of indecision, then we can fill our projects with micro-deadlines that force us to act early , not ponder. Don’t set yourself up to make critical decisions in the eleventh hour. They don’t typically end well. Anything longer than 90 days is at risk of high-velocity decision fatigue in my opinion, especially if it’s a project that doesn’t produce anything tangible (sometimes we just need to see it!). Don’t plan big life events at the same time as big career events, or vice versa Decision fatigue doesn’t discriminate between choices at work or choices at home. They’re just choices, and your willpower is counting backward from an unknown number until it implodes. During the stride of our website build, we had two linchpins (our designer and developer) take paternity leave at different times. During the same time I found out that I would also be a dad, and that’s when my wife and I began our search for a bigger home (and… baby registry ). Defining a winning strategy Over 15,000 Australians travel to Thailand every year for discounted plastic surgery. But a growing number of them are returning with serious infections, asymmetry, and botched results. Of that group, the most vulnerable demographic is the 18-24 year old women seeking breast surgery. Armed with little disposable income, misinformation about ethical plastic surgery, and a youthful zeal for enhancing their breasts, unqualified surgeons have welcomed their naivety with open arms. Motivated by their conviction to offer the highest standards of plastic surgery at affordable prices, and by their desire to offer medical tourists a safer alternative, the team at M.I.P.S. knew they were uniquely qualified to exceed the needs of these vulnerable patients. What they didn’t know was how to reach them in an authentic way. So our first step was to build a unique brand identity and website that would not only resonate with young women’s interests and needs in an authentic way, but that would also function as the marketing infrastructure for our entire social campaign. Next, we would have to choose a social channel for attracting new patients that also hosted the largest and most engaged group of 18-24 year old women in Australia. More importantly, because our social strategy relied heavily on the precision of social media advertising to reach our audience, it was important that we worked with platforms that offered granular targeting options so we could tailor our message to a hyper-specific group of women who were ready to buy. 1. Scratch made brand identity The vision for what would become YourBreast started as an idea, like all successful visions. But that idea didn’t come with a name, a voice, a message, or an identity. So after we gained a thorough understanding of the interests, desires, pain points, and social preferences of our target audience, our first step was to create an identity that supported those characteristics and attributes, starting with a name. We chose YourBreast for two reasons: first, we wanted to promote autonomy from the first impression-your body, your choice; second, the name typified everything M.I.P.S. hoped to empower in young women, namely strength, confidence, and individuality. But a name is just a name. So we supported the name with bold colors, strong typography, a simple logo, and alluring imagery that resonated specifically with young women. To emphasize the marriage of affordability and quality for our price-conscious audience, we anchored our messaging with price transparency, education of standards and accreditation, and proven case studies from real patients. 2. Authentic participation Our audience goal aimed for quality, not quantity. Rather than add 150K followers from around the world (most of whom would never become patients), we chose to stay small and build an audience comprised of local Melbourne women- one person at a time. And we chose Instagram as our primary social channel because that’s where our target patient frequented most. Since we wanted to create a close community that advocated authenticity, transparency, participation, and inspiration, and let the surgeons work do the talking for them, we built our organic audience using inspirational posts and before and after case studies, and we championed high standards and YourBreast’s ethos of individuality and confidence. Then, we caught serendipity on a whim when a few courageous patients took it upon themselves to start a branded campaign on behalf of YourBreast: #BabesofYB. We started with zero Instagram followers. Today, YourBreast has a modest yet influential Instagram following of 2,700 loyal patients and prospects, and has a conversion rate of 4.5% from organic (non-paid) Instagram visitors. 3. Precision targeting Since building a quality Instagram following comprised of actual prospects requires a long-term investment, we relied on social media ads to help YourBreast promote their message immediately. With the precision of Instagram advertising, not only could we confidently predict ad spend and traffic volume, but we could also measure our return on investment down to the dollar. So they contacted us to help them build an SEO strategy that would parlay their offline experience into online authority and win new patients from search. Big decisions. Big Apple. The first step in defining our winning strategy required a prescient understanding of both the New York Group for Plastic Surgery’s business goals and the nature of search in their specific market. From the beginning, NYGPS wanted to increase revenue from their liposuction-related procedures (lipo, smart lipo, laser lipo), as well as increase overall traffic and conversions from search. But the hyper competitive landscape of New York meant hundreds of other surgeons were lobbying for the same goals and rankings, and most of them had a head start. If we were going to help NYGPS achieve their business goals through SEO, we would have to concentrate our resources on a few critical objectives rather than spread ourselves thin competing for everything. Armed with intelligence from a comprehensive technical SEO audit, competitive research, and thorough discovery meetings with both teams, we built a three-tiered strategy that would help us accomplish NYGPS’ business goals. 1. A foundation for growth NYGPS built their previous website using Bitrix, a dated content management system (CMS) heavy with unnecessary code, slow to load, and hard for search engines to crawl and index. And aside from performance, it failed the mobile-friendly test and offered a poor user experience for patients. So our first step toward improving rankings, increasing conversions, and generating revenue through SEO was to ensure we were building on a stable foundation, which meant overhauling their entire website for search engines and patients. In November, several months after starting our SEO program, we launched their new website on WordPress, a much faster and lighter CMS, featuring a new brand identity and user experience. With an improved website architecture, faster speed, and better path to conversion for each patient, search engines could now crawl and index the site easier, more frequently, and with more trust, and patients could better find answers to their questions. As a result, in less than a month post launch, organic traffic increased by 78% on desktop and 111% on mobile as rankings skyrocketed, and conversions increased by 188% from the influx of new visitors. Also, average time on site increased by 121%, bounce rate decreased by 35%, and pages per visit increased by 180%. Local mobile Local Pack (or 3 Pack ) results on Google refer to the three map listings that appear atop search results for category-defining keywords like “plastic surgeon” or “dentist.” Though NYGPS already ranked high page one for “plastic surgeon in New York” in organic results, a huge portion of that traffic clicked on Local Pack results first, especially on mobile. The only problem was that NYGPS, at the time, ranked on page ten (+100) locally. Despite the challenge, we decided that rather than invest our resources in competing head to head with entrenched incumbents for organic results, we could potentially double traffic by winning a #1 position in the Local Pack- where everyone else ignored. The biggest issue keeping NYGPS from ranking in local results wasn’t domain authority; it was inconsistent business information (“citations”) across the internet from the myriad address changes and phone numbers over the years. And since Google treats inconsistent business information like antitrust signals, we spent 12 months cleaning up old citations and building hundreds of new industry-specific and local business listings. In one year , and over 200 business listings later, we took NYGPS’ underperforming local page from page ten to #1 for primary keywords like “plastic surgeon in New York.” The results? Search engine traffic increased from 3,300 average monthly visitors to 6,545 average monthly visitors. And website form submissions increased from 36 to 72 contacts per month (100% increase). Liposuction or bust While our local mobile and website rebuild strategies led to the most favorable increases in overall search engine traffic, it didn’t solve for NYGPS’ most critical business objective: to increase revenue from liposuction-related procedures. We were lucky: NYGPS understood the strategic importance of narrowing our focus on a single category vs. spreading our resources thin by trying to be all things to all people in a competitive market. If we wanted to take NYGPS from page three to page one on Google for liposuction-related keywords, it was going to take everything we had. Over the first 18 months of our SEO program, it was critical that all of our marketing activities aligned with the singular goal of building NYGPS as an authority on liposuction. So blog posts, social activity, blogger outreach, citation building, and link building all focused on deepening NYGPS’ perceived topical expertise on liposuction- nothing else. In January of 2014, we started at #23 (page three) for “Liposuction New York.” By mid-2015, we had successfully positioned NYGPS at #1 overall- a place it has stayed ever since. Shortly after, keywords like “laser lipo New York” and “smart lipo New York” followed suit, and by the end of 2015, all of our liposuction-related keywords were high page one of Google. The results?
