If you need testing and/or treatment for sexually transmitted diseases we're here for you. We provide some free services for low income people who qualify for the Family PACT program. This program also provides contraception.
Immigration Exams
We provide exams for the i693 immigration form. Please call our office for information about what to bring to the exam. You will need vaccines and a blood test.
DMV Commercial Driving Examinations
We provide the physical examination for truck drivers and bus drivers who need a Class A drivers license.
Free Services
We provide free vaccines for children through the VFC program as well as physical exams for school entry through the CHDP program.

Services Diabetes Care We provide comprehensive care for those patients with diabetes. We prescribe oral diabetic medication and the various types of insulin. We strive to maximize dietary control by education the patient on the avoidance of "bad carbohydrates" and a generally healthy diet. We aim for the patient to have a hemoglobin A1C of 7.0 or below. Just as important we focus on preventive care for diabetes including, diabetic eye exam annually, foot exams, LDL cholesterol control, and screening annually for kidney complications. Medical Care We treat urgent medical conditions such as lacerations, upper respiratory infections, gout attacks, urinary tract infections, skin infections, dizziness and many others. Call or drop in if you are in need of care. We do reserve the urgent visits for these type of illnesses. If you need routine services such as blood pressure checks, medication adjustment, routine physical exams please make an appointment. Family Planning We provide a number of free services to low income people who qualify. Please call us to find out if you qualify for the following free services. Immigration Exams We provide physical exams for form i693 for immigration purposes. Exam Requirements Examination by the doctor Lab tests: QuantiFERON Syphilis Test Gonorrhea Test To start call and schedule an appointment for lab work. We can draw your blood here at the office. Lab results take a week. If all of your lab results are normal we will schedule you an appointment with the doctor. If your lab results are positive, we will require additional testing and/or treatment. On the day of the visit please bring the following: Photo identification or passport Written record of vaccines Required vaccines (for adults): Tdap, MMR, Flu (during flu season), Hep B, Polio and varicella if not immune. USCIS number (A-number) or work permit Immigration Vaccine Age Range 18-59 years Tdap (valid for 10 years) Varicella (not needed if you have history of chickenpox) Flu shot (seasonal flu shot) IPV-Polio MMR Hep B 60-64 years Tdap Varicella Flu shot IPV-Polio MMR 65 years & above Tdap Varicella Flu Shot IPV- Polio Pneumococcal Vaccines for children We provide vaccines necessary for children to go back to school at little to no cost. Children eligible for the Vaccines for Children program include: Uninsured MediCal-eligible American Indian or Alaska Native Click the link for more information Internal Medicine We treat the full spectrum of adult illnesses including chronic illnesses such as diabetes, rheumatoid arthritis, hypertension, asthma, psoriasis, and many others. Internal medicine is the branch of medicine that treats adult illnesses that are NOT surgical problems but rather are treated with medication. Doctors who practice internal medicine are called "internists" and receive three years of hospital based training caring for sick patients. We will refer patients to highly skilled subspecialists in the area as the situation requires. Other services Dermatology: we removed skin tags and other benign lesions, treat acne, itchy rashes such as poison oak and urticaria, toenail problems, rosacea, and other skin conditions Heart: we perform EKG’s in the office, counsel on reducing risk for heart disease, and we work closely with local community cardiologists for evaluation of heart symptoms. Cancer prevention: Male and female cancer prevention. Immunizaitons Digestive problems: gastritis, hemorrhoids, colon screening, liver, gall bladder evaluations. Neurologic: migraines, tension headaches, facial nerve paralysis (Bell’s Palsy). Lab: we order blood testing and draw blood in our office for your convenience, and send out to Quest and Labcorp. He attended UC Berkeley for undergraduate studies. Then he attended University of Illinois College of Medicine in Chicago where he earned a degree in medicine. He trained in the specialty of Internal Medicine at the Alameda County Medical Center. He has been serving the East Oakland and South Stockton since 1997. Immigration Exam Form I-693 I693 Exam 3 Easy Steps Step 1 1st Visit Call to schedule your first appointment to get your process started. Appointments available with a few days. You can reach us in Oakland - 510-261-1677 Stockton - 209-460-1900 The first appointment consists of doing required lab tests. Also bring your vaccination records. Step 2 Phone Call A week after your lab appointment, call the office to get your Lab results. If any test is abnormal then further treatment or testing may be required. If labs are normal a staff member will schedule your physical appointment immediately. Step 3 2nd Visit For your exam with the doctor please bring the following: Identification Card Vaccine Record Appointments are usally available quickly Vaccines Required T-DAP VARICELLA POLIO INFLUENZA ( October 1 to March 31) HEPATITIS B (only if age 59 or younger) MMR (only if born in 1957 or later) Pneumococcal (only if age 65 or older) More About Vaccines Frequently asked questions Immigration Exam How long does the entire process take? It takes a minimum of 4 to 6 days. The lab testing is the limiting factor. They usually take 4 to 6 days to come back. After we get the lab results we can do the exam as quickly as you need it. Our office hours are Monday to Friday, 9 am – 5 pm We are open Saturdays in Oakland but not usually for i693s with rare exception. What is the office address? The examination includes a review of your medical and vaccination history, along with mandatory lab tests for tuberculosis, gonorrhea, and syphilis based on age. The U.S. government mandates certain vaccinations, which are based on your age and other factors. COVID-19 vaccination is NO LONGER REQUIRED. What are the costs associated with the Medical Examination? $250 for the doctor’s examination. Vaccines and labs are a separate charge. What vaccines do I need? If you are 18 to 59 years of age you need 6 vaccines: the 1. Tdap 2. Polio 3. MMR (needed if born 1957 or later) 4. Hep B (up to age 59) 5. Varicella 6. Flu shot (if Flu Season) If you are 60 years old or older, you DO NOT need Hepatitis B. If you are 65 or over you DO need the pneumonia vaccine. If you are born 1956 or earlier, regardless of age, you DO NOT need the MMR If you are 17 years old or younger you need the full pediatric schedule. Get Vaccine Information from the CDC More information and FAQs on the USCIS website Can I do lab test to show immunity instead of getting vaccines? Yes. This is optional. You can bring if immunity lab tests from your doctor or we can do the tests here. The USCIS accepts lab tests that show immunity for the following vaccines: 1. MMR 2. Hepatitis B 3. Varicella. Polio lab immunity presents a challenge. If you do a lab test for polio immunity please be aware that the test must include polio types 1,2 and 3 , but that most labs only do 2 out of 3 types. The USCIS will not accept a polio immunity test that is done for only 2 polio types. So people usually have to get the polio vaccine. Where can I get my vaccines? You can get the vaccines at a pharmacy or at your own primary care office. Just bring if proof of the vaccines. You can also get the vaccines at our office. Our vaccines prices are in line with those of local pharmacies. We do not bill insurances for vaccines. Is fasting required for the blood test? No. What do I need to bring to the medical examination? Please bring a government issued ID such as a passport or driver’s license, and bring vaccination records. Also bring your insurance card if you want to use your insurance for lab testing. When do I get my completed I-693 form? We give you the completed form on the day of your examination with the doctor. What if I am pregnant? You can still do the exam. A delay may occur if you need a chest x-ray due to a positive TB blood test. If you TB blood test is negative (normal) then you will not have this delay. What part of the year is the influenza vaccine required? The annual flu shot is required if your i693 examination will be signed between September 1st to March 31. You do NOT need the flu shot if signed April 1 to August 31st. What if I need a copy of an i693 exam your office completed in the past? There is a fee of $50 for the reissuance of the I-693 form. Where can I get a copy of the i693 form and instructions Get the latest i693 and instructions from the USCIS website. Scroll down to Forms and Documents and you will see two links, one for the i693 and one for instructions. Click the link below to go the USCIS website. Get i693 What lab tests are needed? Quantiferon (Tuberculosis) for all ages. Syphilis between ages between 18 and 45. Gonorrhea for all applicants between ages 18-24 years. If an applicant has an adverse sexual medical history, ages can be 15 and up. NO fasting is required. Can I bring the test result from my own doctor's office? No. These tests must be ordered here at our office. We can draw the blood test here at our office. We use Labcorp and Quest Diagnostics. Those labs can bill your insurance for the cost of the labs. Bring your insurance card when you come for your lab test. Is the I-693 exam like an annual physical? No. The I-693 is a specific evaluation from the USCIS that screens for certain infections and vaccines. This exam dose NOT check for general health or medical conditions other than those specified. How long is the form good for? The I-693 does not expire. This is a change by the USCIS took effect April 4, 2024. However, the USCIS updated the policy to say that the USCIS is good for the current application and you must submit the exam at the time you file the I-485.
