Services Diabetes Care We provide comprehensive care for those patients with diabetes.   We prescribe oral diabetic medication and the various types of insulin. We strive to maximize dietary control by education the patient on the avoidance of "bad carbohydrates" and a generally healthy diet.   We aim for the patient to have a hemoglobin A1C of 7.0 or below. It takes a minimum of 4 to 6 days. The lab testing is the limiting factor. They usually take 4 to 6 days to come back. After we get the lab results we can do the exam as quickly as you need it. The examination includes a review of your medical and vaccination history, along with mandatory lab tests for tuberculosis, gonorrhea, and syphilis based on age. The U.S. government mandates certain vaccinations, which are based on your age and other factors. COVID-19 vaccination is NO LONGER REQUIRED. What are the costs associated with the Medical Examination? $250 for the doctor’s examination. Vaccines and labs are a separate charge. What vaccines do I need? If you are 18 to 59 years of age you need 6 vaccines: the 1. Tdap 2. Polio 3. MMR (needed if born 1957 or later) 4. Hep B (up to age 59) 5. Varicella 6. Flu shot (if Flu Season) If you are 60 years old or older, you DO NOT need Hepatitis B. If you are 65 or over you DO need the pneumonia vaccine. If you are born 1956 or earlier, regardless of age, you DO NOT need the MMR If you are 17 years old or younger you need the full pediatric schedule. Get Vaccine Information from the CDC More information and FAQs on the USCIS website Can I do lab test to show immunity instead of getting vaccines? Yes. This is optional. You can bring if immunity lab tests from your doctor or we can do the tests here. The USCIS accepts lab tests that show immunity for the following vaccines: 1. MMR 2. Hepatitis B 3. Varicella. Polio lab immunity presents a challenge. If you do a lab test for polio immunity please be aware that the test must include polio types 1,2 and 3 , but that most labs only do 2 out of 3 types. The USCIS will not accept a polio immunity test that is done for only 2 polio types. So people usually have to get the polio vaccine. Where can I get my vaccines? You can get the vaccines at a pharmacy or at your own primary care office. Just bring if proof of the vaccines. You can also get the vaccines at our office. Our vaccines prices are in line with those of local pharmacies. We do not bill insurances for vaccines. Is fasting required for the blood test? No. What do I need to bring to the medical examination? Please bring a government issued ID such as a passport or driver’s license, and bring vaccination records. Also bring your insurance card if you want to use your insurance for lab testing. When do I get my completed I-693 form? We give you the completed form on the day of your examination with the doctor. What if I am pregnant? You can still do the exam. A delay may occur if you need a chest x-ray due to a positive TB blood test. If you TB blood test is negative (normal) then you will not have this delay. What part of the year is the influenza vaccine required? The annual flu shot is required if your i693 examination will be signed between September 1st to March 31. You do NOT need the flu shot if signed April 1 to August 31st. What if I need a copy of an i693 exam your office completed in the past? There is a fee of $50 for the reissuance of the I-693 form. Where can I get a copy of the i693 form and instructions Get the latest i693 and instructions from the USCIS website. Scroll down to Forms and Documents and you will see two links, one for the i693 and one for instructions. Click the link below to go the USCIS website. Get i693 What lab tests are needed? Quantiferon (Tuberculosis) for all ages. Syphilis between ages between 18 and 45. Gonorrhea for all applicants between ages 18-24 years. If an applicant has an adverse sexual medical history, ages can be 15 and up. NO fasting is required. Can I bring the test result from my own doctor's office? No. These tests must be ordered here at our office. We can draw the blood test here at our office. We use Labcorp and Quest Diagnostics. Those labs can bill your insurance for the cost of the labs. Bring your insurance card when you come for your lab test. Is the I-693 exam like an annual physical? No. The I-693 is a specific evaluation from the USCIS that screens for certain infections and vaccines. This exam dose NOT check for general health or medical conditions other than those specified. How long is the form good for? The I-693 does not expire. This is a change by the USCIS took effect April 4, 2024. However, the USCIS updated the policy to say that the USCIS is good for the current application and you must submit the exam at the time you file the I-485.
