Our In Network Insurance Providers Payment Expectations Payment is due at the time of service, including estimated out-of-pocket costs. We accept Credit/Debit Cards, Apple Pay & Google Pay, and HSA/FSA Cards. Patients using FSA/HSA CARDS must call their provider and pre-approve the amount Marin Modern Dentistry will process. We accept Checks, but please note we do not accept cash. Financing is available through CareCredit and Cherry Finance. A card on file is required for all patients and may be used for balances due, missed appointment fees, and pre-approved payments. You will be notified prior to any charge. Dental Insurance HUMANA, UNITED CONCORDIA, GUARDIAN, METLIFE, or AETNA have consistently misprocessed our claims as IN NETWORK while our office is OUT OF NETWORK. Marin Modern Dentistry DOES NOT HONOR ANY INSURANCE ADJUSTMENTS FROM THESE PLANS. If your insurance plan incorrectly adjusts any amounts from the check received, we will collect that error/difference owed from the patient . We submit claims as a courtesy, but you are responsible for any amount not covered by insurance. We only provide estimates, not a guarantee of payment from your dental plan; final patient responsibility is determined after claim processing. You must notify us of any insurance changes prior to your appointment. Claims remain open for 60 days only. After 60 days, you are responsible for any balances not paid by your plan (including appealed claims). We are only in-network with: Blue Shield, Delta Dental, SunLife, United Healthcare, Ameritas, Dental Benefit Providers, Principal, and Beam. Uninsured Patients A 10% discount is available when payment is made in full at the time of service. Missed Appointments & Cancellations Your appointments are very important to all members of our team at Marin Modern Dentistry, and we value your time. We respectfully ask that if you are unable to keep your appointment, you provide us with a 2 business day notice so that we can offer that time to another patient. Failure to notify us of your inability to attend your appointment within 2 business days will result in a $100.00 per hour charge to the patient. Late Payments, Balances & Collections Payment is due upon receipt of your statement. Our office gives patients 30 days to take care of balances from the date insurance payment was received. After 30 days, we will charge the card on file. Accounts with balances over 90 days past due will be sent to collections. Minors Minors must be accompanied by a parent or legal guardian unless we have signed consent for them to visit the practice alone. The accompanying adult is responsible for payment. Financial Arrangements Deposits may be required for major and/or cosmetic procedures. We are happy to discuss solutions for financial hardship, and financing is available through CareCredit and Cherry Finance. FAQs CEREC Single Visit Crowns 01. Can anyone receive CEREC crowns? Usually, Yes. 02. I’ve had a metal crown most of my life. Is it possible for me to replace it with a CEREC crown? Usually, yes. 03. Could a CEREC crown be a better replacement for a broken or chipped veneer? CEREC crowns may be used to restore front teeth but there may be other materials that are more esthetically pleasing. 04. Are CEREC crowns more expensive than traditional crowns? Are there long-term cost advantages? Most insurance companies do assist with covering CEREC crowns, similar to conventional types of crowns. The long term cost advantages of CEREC crowns are: Less risk of recurrent decay due to bonding method. Less tooth structure is removed using the CEREC method. The material is very durable and healthier for your gum tissue. 05. I have very small teeth. Could a CEREC crown still be used in my case? Yes, the CEREC technology allows us to customize the size and shape of each tooth. 06. My teeth are a bit stained and discolored. Would a CEREC crown look strange next to my other teeth? An esthetic crown may be a better option because the lab technician has better control over color variations. However, if the tooth is not visible when you smile or speak, it may not be a concern. 07. Can you whiten a CEREC crown? No, restorative materials typically do not respond to whitening. 08. Are there certain foods to avoid after receiving a CEREC crown? A CEREC crown is similar to your natural tooth structure but could break or fracture when eating hard candies or if you use your teeth to open bottles 09. Does the procedure hurt? You are anesthetized during the procedure to ensure your comfort. 10. How durable are CEREC crowns? CEREC crowns are very durable, and their scientific makeup is very similar to that of natural tooth enamel. 11. Will I have any sensitivity in my teeth after getting a CEREC crown? Most patients do not have sensitivity or pain after a CEREC crown but there is always a chance of sensitivity after any dental procedure. If this occurs, please make sure to contact us so we can assist you in sensitivity management. 12. What happens if I damage my CEREC crown? Depending on the extent of damage, CEREC crowns can either be repaired or replaced. If the integrity of the crown is not compromised and is not rough or bothersome to the patient it can be left as is. 13. How much of my tooth is removed? Only the damaged, defective or decayed portion of the tooth is removed during the CEREC procedure. Therefore, more of your natural tooth structure is retained. When traditional crowns are made healthy tooth structure needs to be removed in order for the crown to cover the tooth and to allow retention. 14. What is the difference between a crown and veneer? A crown typically covers both the front and back of a front tooth and covers the biting surface of a back tooth. A veneer is a thin porcelain layer that covers the front surface of a tooth. 15. Will I still see that black line at the gum line if I replace my old crown? The black line seen at the gumline with traditional crowns is the metal portion under the porcelain layer. When using a CEREC crown to replace old crowns, there is no metal material, so any black lines are eliminated. Periodontal Treatment 01. Will my insurance cover periodontal treatment? A proud Bay Area native, she earned her Doctor of Dental Surgery from the University of California, San Francisco, one of the nation’s top dental programs. Following dental school, she completed an Advanced Education in General Dentistry (AEGD) residency, further refining her skills in comprehensive dental care. With more than 10 years of experience serving patients throughout the Bay Area, Dr. Johnston is highly proficient in all aspects of general dentistry including preventive care, restorative procedures, and extractions. She has a special passion for cosmetic dentistry and finds great fulfillment in helping patients transform their smiles through veneers, whitening, and complete smile makeovers. Her artistic eye and meticulous attention to detail ensure that each smile is as natural as it is beautiful. Dr. Johnston also has advanced training in the treatment of temporomandibular joint (TMJ) disorders. She offers therapeutic Botox as a non-invasive, effective option to relieve jaw tension, facial pain, and symptoms related to clenching or grinding. Believing strongly in the power of prevention, Dr. Johnston takes an educational approach to dentistry—empowering her patients with the knowledge and tools they need for long-term oral health. He is a certified specialist in restorative and cosmetic dentistry and, since 1978, has owned The Marin Center for Restorative & Cosmetic Dentistry in Novato & Mill Valley. He is a member of the American Academy of Cosmetic Dentistry and the American Dental Association, and he is a fellow of the International Congress of Oral Implantologists. Locally, he is a member of the California Dental Society and the Marin County Dental Society. He has completed additional education and training in a number of specialties, including sedation dentistry, nonsurgical periodontal laser therapy, and CEREC restorations. Dr. McConnell integrates advanced technology and waste reduction, minimally invasive techniques, and non-toxic materials into his patients’ treatment plans while maintaining a paperless office. His commitment to environmental responsibility is a natural outgrowth of his dedication to the highest standard of patient care. Performing digital X-rays in lieu of film and using CEREC natural porcelain crowns rather than traditional laboratory-made restorations reflect the green initiative he instituted in 2001. Considered a compassionate practitioner as well as a master clinician and dental artist, Dr. McConnell was recently nominated by his peers and selected by the Consumers’ Research Council of America as one of “America’s Top Dentists.” He is also a dental health expert for Smart Now, a website devoted to providing health, fitness, and wellness information to the public. He played collegiate football at Saint John’s University before transferring to the University of Nevada, Reno, where he completed his Bachelor of Science in Biology with honors. He went on to earn his Doctor of Dental Surgery (DDS) degree at the University of the Pacific Arthur A. Dugoni School of Dentistry, where he was honored as the runner-up for the prestigious Henry Sutro Award for Excellence. He is committed to lifelong learning and continues to pursue over 100 hours of continuing education annually in the fields of cosmetic dentistry, general dentistry, and clear aligner treatment.
