Who follows this Notice This Notice applies to Mission Neighborhood Health Center (MNHC), all of its sites (Shotwell Clinic, Excelsior Clinic, Mission Neighborhood Resource Center, Latino Prevention Center, John O'Connell School-Based Teen Clinic), and all employees, staff, volunteers, students, and other personnel involved in your care. Our duties to you Federal law (HIPAA, 45 CFR Parts 160 & 164) and California law (the Confidentiality of Medical Information Act, Civil Code §§ 56–56.37) require us to: Keep your protected health information private. Give you this Notice of our legal duties and privacy practices. Follow the terms of the Notice currently in effect. Notify you if a breach of your unsecured protected health information occurs. How we may use and share your information We may use and share your protected health information for treatment, payment, and health care operations — for example, with the providers caring for you, your insurance plan, or for quality reviews. Other permitted uses include public health activities, required reporting (such as suspected abuse), and as required by law. Who qualifies Every patient — insured or uninsured, documented or undocumented, employed or not — is invited to apply. Eligibility is based only on family size and household income, not on immigration status, social security number, or proof of address. Open to all MNHC patients across all five clinic sites. Recertification is required once a year, or sooner if your income changes. If you cannot provide income documentation, you may complete a self-attestation form. Patients who refuse to provide income information are charged the standard fee. What to bring to apply Photo ID for the head of household (any government-issued ID is fine — driver's license, passport, consular ID, Municipal ID). Proof of household income for the past 30 days (recent pay stubs, Social Security or unemployment letter, self-employment ledger, or a signed self-attestation form). Proof of household size (tax return OR a signed declaration listing dependents). If you have insurance, your insurance card. If you have no insurance, that is OK — we will help you apply for Medi-Cal, Healthy SF, or Covered California if you qualify. Bilingual English/Spanish enrollment counselors are available at every clinic to walk through the form with you. The application takes about 15 minutes. MNHC runs wellness groups designed to promote healthy living and patient empowerment — from chronic disease management to group prenatal care to peer HIV support. Schedules change each quarter, so call at the start of the quarter for the current calendar. Available groups Group diabetes education A monthly series where patients with pre-diabetes or diabetes learn about nutrition, glucose control, medication management, and meal planning — alongside MNHC health educators and registered dietitians. By referral Promotores — peer diabetes program Community members living with diabetes provide prevention and self-care education to their neighbors, in collaboration with SF DPH, the American Diabetes Association, and CARECEN. Community-based Centering Pregnancy Groups of 8–10 pregnant patients with close due dates meet together with a midwife and bilingual care team throughout pregnancy and after delivery — medical care, nutrition, family planning, and peer support. By cohort · English & Spanish Clínica Esperanza Knitting Group Since 2007, patients of Clínica Esperanza and their families gather to knit, embroider, and build community — a low-barrier, stress-reducing space for people living with HIV. Call us Get care Community Programs Mission Neighborhood Resource Center Bilingual, drop-in care and social services for homeless residents of the 16th Street corridor. Founded in 2002. Harm-reduction model. A community response since 2002 The Mission Neighborhood Resource Center (MNRC) was founded in 2002 as a community response to the neighborhood's growing homeless population. MNRC advocates for housing and resources to enhance the physical, social, emotional, and economic health of homeless residents in the 16th Street corridor and the surrounding Mission neighborhood. MNRC operates on a harm-reduction model — focused on dignity, community-building, and meeting basic human needs while residents move toward greater stability and well-being. All services are bilingual and drop-in.
