Use tab to navigate through the menu items. Root Canal Therapy What is Root Canal Therapy? Root canals are tiny passageways that branch off from beneath the top of the tooth, coursing their way vertically downward, until they reach the tip of the root. All teeth have between one and four root canals. Many tooth problems involve infections that spread to the pulp, which is the inner chamber of the tooth containing blood vessels, nerves and other tissues. When the infection becomes worse, it can begin affecting the roots. A traumatic injury to a tooth can also compromise the pulp, leading to similar problems. A diseased inner tooth brings a host of problems; pain and sensitivity are some of the first indications of a problem; but inside, a spreading infection can cause small pockets of pus to develop, leading to an abscess. Root canal therapy is a remarkable treatment with a very high rate of success, and involves removing the diseased tissue, halting the spread of infection and restoring the healthy portion of the tooth. In fact, root canal therapy is designed to save a problem tooth; before the procedure was developed and gained acceptance, the only alternative for treating a diseased tooth was extraction. Symptoms If you are experiencing any of the following symptoms, call our office right away at (650) 938-9000 to determine if there’s a problem and the proper treatment: Tooth pain Prolonged sensitivity to hot and cold Tooth is sensitive or tender to the touch Swelling Drainage Discoloration of tooth You may need root canal treatment even if you aren't currently experiencing any pain. You may have had pain in the past, but without any intervention, the decay continued until it destroyed the tooth’s nerve. With its nerve dead, the tooth will give you no pain, but will still be in need of immediate care. Procedure Root canal therapy usually entails one to three visits. During the first visit, a small hole is drilled through the top of the tooth and into the inner chamber. Diseased tissue is removed, the inner chamber cleansed and disinfected, and the tiny canals reshaped. The cleansed chamber and canals are filled with an elastic material and medication designed to prevent infection. If necessary, the drilled hole is temporarily filled until a permanent seal is made with a crown. Most patients who undergo root canal therapy experience little to no discomfort or pain and enjoy a restored tooth that can last a life time. With today’s advanced techniques, materials and anesthetics, many of our patients say that having a root canal was a comfortable experience and not traumatic at all. Tooth Extractions Causes for Tooth Extraction Good oral hygiene should always be practiced since the loss of a single tooth can have major impact upon your oral health and appearance. Although dentists will use every measure to prevent tooth loss, there still sometimes necessary occasions when a tooth may need to be extracted. A tooth may need to be extracted if the following occurs: Severe decay Advanced periodontal disease Infection or abscess Orthodontic correction Malpositioned teeth Fractured teeth or roots Impacted teeth After careful examination and treatment, Dr Lou may advise to have a tooth extracted. Before a tooth is removed, the dental assistant will take an x-ray in order to understand the shape and position of the tooth and surrounding bone. Based on the degree of difficulty, we may refer you to a specialist called an oral surgeon. Post-Treatment Expectations A certain amount of pain and discomfort is to be expected following an extraction, which can be minimized with a pain killer and ice packs applied to the face for 15 minutes at a time. After a routine extraction, discomfort should lessen within three days to two weeks. If you have prolonged or severe pain, swelling, bleeding or fever, contact us immediately. Use tab to navigate through the menu items. Periodontal Treatment Progression of Periodontitis Periodontal treatment methods depend upon the type and severity of the disease. Dr Lou or a Registered Dental Hygienist will use pocket depths, amount of bleeding, inflammation, tooth mobility, etc., to make a diagnosis that will fall into a category below: Gingivitis Gingivitis is the first stage of periodontal disease. Plaque and its toxin by-products irritate the gums, making them tender, inflamed, and likely to bleed. Periodontitis Plaque hardens into calculus (tartar). As calculus and plaque continue to build up, the gums begin to recede from the teeth. Deeper pockets form between the gums and teeth and become filled with bacteria and pus. The gums become very irritated, inflamed, and bleed easily. Slight to moderate bone loss may be present. Advanced Periodontitis The teeth lose more support as the gums, bone, and periodontal ligament continue to be destroyed. Unless treated, the affected teeth will become very loose and may be lost. Generalized moderate to severe bone loss may be present. During your visit, Dr Lou will evaluate for periodontal disease and recommend the appropriate treatment, if needed. Treatment Periodontal disease progresses as the sulcus (pocket or space) between the tooth and gums gets filled with bacteria, plaque, and tartar, causing irritation to the surrounding tissues. When these irritants remain in the pocket space, they can cause damage to the gums and eventually, the bone that supports the teeth. If the disease is caught in the early stages of gingivitis, and no damage has been done, one to two regular cleanings will be recommended. You will also be given instructions on improving your daily oral hygiene habits and having regular dental cleanings. If the disease has progressed to more advanced stages, a special periodontal cleaning called scaling and root planing (deep cleaning) will be recommended. It is usually done one quadrant of the mouth at a time while the area is numb. In this procedure, tartar, plaque, and toxins are removed from above and below the gum line (scaling) and rough spots on root surfaces are made smooth (planing). This procedure helps gum tissue to heal and pockets to shrink. Medications, special medicated mouth rinses, and an electric tooth brush may be recommended to help control infection and healing. If the pockets do not heal after scaling and root planing, periodontal surgery may be needed to reduce pocket depths, making teeth easier to clean. Dr Lou or our hygienist may also recommend that you see a Periodontist (specialist of the gums and supporting bone). Once your periodontal treatment has been completed, our dental hygienist will recommend that you have regular maintenance cleanings (periodontal cleanings), usually four times a year. At these cleaning appointments, the pocket depths will be carefully checked to ensure that they are healthy. Plaque and calculus that is difficult for you to remove on a daily basis will be removed from above and below the gum line. Good oral hygiene practices and periodontal cleanings are essential in maintaining dental health and keeping periodontal disease under control! Because they are custom-made, bridges are barely noticeable and can restore the natural contour of teeth as well as the proper bite relationship between upper and lower teeth. Appliances called implant bridges are attached to an area below the gum tissue, or the bone. Crowns Crowns are synthetic caps, usually made of a material like porcelain, placed on the top of a tooth. Crowns are typically used to restore a tooth's function and appearance following a restorative procedure such as a root canal . When decay in a tooth has become so advanced that large portions of the tooth must be removed, crowns are often used to restore the tooth. Crowns are also used to attach bridges, cover implants, prevent a cracked tooth from becoming worse, or an existing filling is in jeopardy of becoming loose or dislocated. Crowns also serve an aesthetic use, and are applied when a discolored or stained tooth needs to be restored to its natural appearance. Types of Dental Crowns There are three common types of crowns that we use in our offices: All-Porcelain Crowns– This type has the most natural appearance and is the most popular. Made from porcelain with a porcelain lining, it is almost indistinguishable from natural teeth. All-porcelain crowns are metal-free and durable, but capable of chipping if proper care is not taken. Gold Crowns – Gold crowns are appropriate when appearance is not a top priority and are mostly used on back teeth. They are durable and give a good chewing surface, but look starkly unlike natural teeth. Porcelain-Fused-to-Metal Crowns – Porcelain-fused-to-metal crowns are both durable and have a very natural appearance when first placed. The metal lining adds strength. However, over time, a dark line may be visible at the gum line as gums recede, revealing the lining’s edge. Procedure A tooth must usually be reduced in size to accommodate a crown. An impression is made of the existing tooth and an impression is made. The impression is sent to a special lab, which manufactures a custom-designed crown. In some cases, a temporary crown is applied until the permanent crown is ready. Permanent crowns are cemented in place. Caring for your Crowns With proper care, a good quality crown could last up to eight years or longer. It is very important to floss in the area of the crown to avoid excess plaque or collection of debris around the restoration. Certain behaviors such as jaw clenching or bruxism (teeth grinding) significantly shorten the life of a crown. Moreover, eating brittle foods, ice or hard candy can compromise the adhesion of the crown, or even damage the crown. Each of the filling materials is briefly explained below: Types of Composite Fillings Gold Fillings - Gold fillings are custom made in a laboratory and then cemented into place. While gold fillings are often the most expensive choice, many consider it the best filling material. Gold inlays are well-tolerated by gum tissues and may last more than 20 years. Silver Fillings - Amalgam (silver) fillings are a more inexpensive choice and are tolerant to wear. Plastic Resin - Composite (plastic) resins are custom made to the exact color of your natural teeth, creating a more natural appearance. Unlike dentures or dental bridges , implants involve surgically placing one or more artificial tooth roots into the jawbone. Implants are so well designed that they mimic the look and feel of natural teeth. Implants are synthetic structures placed in the area of the tooth normally occupied by the root. They are usually made of biocompatible material like titanium or ceramic. The implants are anchored to the jawbone and act as a foundation for an artificial tooth or permanent bridge. In some cases, implants can be used to attach dentures. Surgery is necessary to prepare the area for an implant and place the implant in the mouth. Following the procedure, a period of time is required for the implant to take hold and for bone tissue to build up and anchor the device. In some cases, metal posts are inserted into the implant during a follow-up procedure to connect the tooth. Because implants require surgery, patients are administered anesthesia and, if necessary, antibiotics to stave off infection following the procedure. Dr. Lou can determine if you are a candidate for a dental implant. For a successful implant to take hold, a candidate must have proper bone density and a strong immune system. Like any restoration, implants require diligent oral hygiene and proper care to ensure they last a long time. Advantages of Dental Implants There are other alternatives to replace missing teeth, such as dental bridges or dentures. However, implants have several advantages over other techniques: Dental implants are secure and durable They replace the tooth root as well as the tooth, preventing any bone resorption (shrinking) Implant crowns look like natural teeth – no one but you will ever know the difference The crowns can be cleaned and cared for much like natural teeth Other teeth are not disturbed in the process of placing implants With proper care, they will last the rest of your life Procedure Dental implants are metal anchors that act as tooth root substitutes. They are surgically placed into the jawbone. Small posts are then attached to the implant which protrudes through the gums. These posts provide stable anchors for artificial replacement teeth. For most patients, the placement of dental implants involves two surgical procedures. First, implants are placed within your jawbone. For the first three to six months following surgery, the implants are beneath the surface of the gums gradually bonding with the jawbone. You should be able to wear temporary dentures and eat a soft diet during this time. At the same time, our office designs the final bridgework or denture, which will ultimately improve both function and esthetics. After the implant has bonded to the jawbone, the second phase begins. Dr. Lou will uncover the implants and attach a small healing collar. Then she will begin making your new teeth. An impression must be taken then posts or attachments can be connected to the implants. The teeth replacements are then made over the posts or attachments. The entire procedure usually takes six to eight months. Most patients do not experience any disruption in their daily life. Implant and Crown Placement Dental implants are placed into the bone in a relatively pain-free procedure. The bone and gums will be given time to heal before an abutment and crown are attached. They are made to closely resemble your natural teeth and may even enhance your smile. There are two types of dentures - complete and partial dentures. Complete dentures are used when all of the teeth are missing, while partial dentures are used when some natural teeth remain. A Partial denture not only fills in the spaces created by missing teeth, it prevents other teeth from shifting. A complete denture may be either “conventional” or “immediate.” A conventional type is made after the teeth have been removed and the gum tissue has healed, usually taking 4 to 6 weeks. During this time the patient will go without teeth. Immediate dentures are made in advance and immediately placed after the teeth are removed, thus preventing the patient from having to be without teeth during the healing process. Once the tissues shrink and heal, adjustments will have to be made.Dentures are very durable appliances and will last many years, but may have to be remade, repaired, or readjusted due to normal wear. Procedure The process of getting dentures requires several appointments, usually over several weeks. Highly accurate impressions (molds) and measurements are taken and used to create your custom denture. Several “try-in” appointments may be necessary to ensure proper shape, color, and fit. At the final appointment, your dentist will precisely adjust and place the completed denture, ensuring a natural and comfortable fit. It is normal to experience increased saliva flow, some soreness, and possible speech and chewing difficulty, however this will subside as your muscles and tissues get used to the new dentures. Inlays are smaller, and contained within the cusps of the tooth. Onlays are done for larger areas of decay and extend over to the outside surface of the tooth We are a mercury-free practice. However, many people still have silver/mercury fillings in their mouths from years past. While they are functional, there have been virtually no advances in metal fillings since their inception and there is some evidence that the mercury contained in them is dangerous to your health. Porcelain inlay and tooth-colored restorations (onlay) create fillings that are not only beautiful (or unnoticeable), but also add strength to weakened teeth. These restorations are aesthetically pleasing and very strong, thanks to new bonding technologies. Dr. Lou prefers to place porcelain fillings, which are shaded to match a patient's existing dentition (color of your existing tooth). She understands how to create healthy, functional, beautiful smiles – and tooth-colored fillings are one simple way to maintain a smile's beauty. Traditional amalgam (silver) fillings are dark, and thus conspicuous, and they contain trace amounts of mercury, a harmful heavy metal. They also expand and shrink in response to temperature changes, resulting in tiny fractures within your teeth. Over time this stress cause by the silver filling can cause the tooth to fracture. Porcelain fillings are a better alternative because they are non-toxic and help create an attractive smile. The final result is virtually invisible! Procedure Porcelain fillings typically take two visits. The damaged or decayed area is removed and an impression, or mold, is taken of the tooth. The mold is sent off to the dental labs , which will custom-make the onlay or inlay from porcelain materials. Dr. Lou will bond the inlay or onlay onto the damaged tooth. While this procedure sounds simple, it requires great skill to do it correctly, so it’s important to have a cosmetic dentist who is experienced and skilled with porcelain fillings. During your first visit, Dr. Lou will prepare your teeth which involve removing a very small portion of the enamel from your teeth so that the porcelain veneers fit seamlessly onto the front sides of the teeth. After your teeth have been prepared, an impression of your teeth will be made. This impression will then be sent to a dental laboratory where your porcelain veneers will be custom made for your smile. The creation of your porcelain veneers usually takes a couple weeks. During this time temporary porcelain veneers will be fitted. Many times, people are so pleased with these temporary veneers that they can’t believe the difference in their smile when the permanent porcelain veneers arrive.
