Your Child's First Visit
The guidelines have changed! In order to prevent early childhood cavities it is recommended that your child see a pediatric dentist when their first tooth erupts or no later than their first birthday.
Restorative Dentistry
If your child should have a small cavity or even an infected tooth, our Pediatric Dental Team will provide the upmost in quality restorative treatment to maintain the form and function of the mouth.
Dental Sealants
A sealant is a protective coating that is applied to the chewing surfaces of the back teeth, filling in the natural pits and grooves. This creates a barrier between food, plaque, acid, and these cavity prone areas.
Dental Emergencies
If your child should sustain an injury to the mouth or experience dental pain, please give our office a call. Our team is available for emergencies during and after office hours.

It can serve as a source of comfort and help them to explore their world. However, prolonged thumb sucking beyond the age of two to three can lead to oral health issues, including problems with proper growth of the mouth, alignment of the teeth, and changes in the roof of the mouth. What To Know As A Parent/Guardian Sucking is an innate and natural reflex in infants. Young children may use thumbs, fingers, pacifiers and other objects on which to suck to continue to feel secure and happy after their infancy stage. Finger digit or object sucking can be relaxing, induce sleep, or help a child feel secure during a difficult time. Sucking habits that persist beyond the age of 2-3 can cause problems with the proper jaw growth of the mouth and tooth alignment. Frequency, duration, and intensity of sucking will determine whether or not dental problems may result. Children who rest their fingers or pacifiers passively in their mouths are less likely to have side effects than those who have vigorous sucking habits. Usually, children naturally stop between the ages of two and four. Sibling or peer pressure causes many school-aged children to stop. While others do not have a desire to stop no matter the influence of their parents, siblings, peers, or interventions. Pacifiers are not a substitute for thumb sucking as they can affect the teeth the same way as finger sucking. However, use of the pacifier can be controlled and modified more easily than the thumb or finger habit. Dr. Espinosa and Dr. DeSimini generally will recommend pacifier cessation before the age of 2. Most parents report it is easier to take the pacifier away before that age and the body has a better chance of correcting any misalignments and poor jaw shapes. If you have concerns about thumb sucking or use of a pacifier, consult with our team. The Impact of Thumb Sucking on Oral Health While thumb sucking is a normal activity for babies and toddlers, it's important for parents to monitor and manage this habit before it affects their child's oral development. Persistent thumb sucking can result in: Misaligned teeth, leading to bite problems. Changes in the shape of the jaw. Speech impediments. Alterations to the roof of the mouth. A Few Suggestions To Help Your Child Get Through Thumb Sucking: Understand Feelings of Insecurity: Children often suck their thumbs when feeling insecure. Focus on correcting the cause of anxiety, instead of the thumb sucking. Positive Reinforcement: Focus on positive reinforcement rather than punishment. Praise your child for not sucking their thumb and provide small rewards for progress. Reward charts provide visual tracking which helps the child see their progress and be more involved with the process. Identify Triggers: Understand the situations or feelings that prompt your child to suck their thumb. Addressing these triggers can help reduce their need for thumb sucking. Offer Alternatives: Provide comfort through other means, such as a favorite stuffed animal or blanket, especially during times they are likely to suck their thumb. When children want to stop, but subconsciously find themselves sucking, gentle verbal reminders, taping the preferred finger or putting a sock on the hand at night can help them achieve their goal. Set Goals: Create a goal chart to track your child's progress. This visual aid can motivate them to stop the habit. Contact Your Pediatric Dentist: If home strategies are not effective, our doctors can encourage children to stop sucking and explain what could happen if they continue at each of their dental appointments. If a child does not wish to cease their sucking habit, Dr. Espinosa or Dr. DeSimini will prepare the parents that early orthodontic therapy may be necessary. Born and raised in Petaluma, Heather has been part of Novato Children’s Dentistry since her very first steps into the dental field. While studying at the College of Marin Dental Assisting Program in 2016, she began an internship at Dr. Espinosa’s office and quickly discovered her passion for pediatric dentistry. Her natural ability to connect with children — from teaching healthy habits to guiding them through appointments with kindness and calm — made her an instant fit for our team. After graduating, Heather joined us full-time as Dr. Espinosa’s lead assistant and soon became an indispensable part of the practice. Her dedication, skill, and organizational talent earned her the role of managing all patient accounts and insurance from her home office after welcoming her second child — yet she still steps in to assist clinically when needed. Whether behind the scenes or in the operatory, Heather is the backbone of our office operations and truly Dr. Espinosa’s right-hand partner in creating seamless, compassionate patient care. Heather and her husband, Brian — a local firefighter — are raising their two children, Wyatt and Aubrey, with the same warmth and dedication she brings to our team. In her free time, Heather loves watching her kids grow, cooking for family and friends, and taking on creative DIY projects both at home and in the office. We are endlessly grateful for Heather’s loyalty, versatility, and heart. She doesn’t just wear many hats — she makes our entire practice run with a steady hand and a smile. Meet Icelya Dental Assistant (D.A) Icelya was born and raised in Santa Rosa, California, where she still lives close to her family. She began her dental career in a busy multi-doctor adult practice, but quickly discovered her true passion when she joined Dr. Espinosa and the team at Novato Children’s Dentistry. Working with children has given Icelya the opportunity to combine her clinical skills with her natural warmth and ability to make young patients feel comfortable, cared for, and confident about their smiles. She especially enjoys teaching children the “why” and “how” of great oral health habits, helping them take ownership of their dental care in a fun, encouraging way. Outside the office, Icelya is a devoted mom to two energetic and loving daughters, Aubriella and Khaleasi, both born in 2018. When she’s not at the front desk, Victoria loves nothing more than spending time with her loving Husband, their two beautiful energetic boys, and their extended family. Meet Nancy DA Nancy joins our team as a supporting dental assistant. Fortunately, I loved visiting our family dentist, and dental appointments never felt intimidating. In fact, as early as Kindergarten, I boldly told my dentist, “I’m going to be a dentist one day!” True to my Type-A nature, I pursued that dream with determination, earning every degree I needed to arrive where I am today. In dental school, I quickly recognized the need to shift our profession’s focus — from simply diagnosing, drilling, and filling — toward guidance, education, and prevention. So many dental problems can be prevented early, leading not only to healthier smiles in adulthood but also to fewer traumatic dental experiences in childhood. At my practice, my mission is to create a peaceful, loving environment where education and prevention are central. I partner with parents to develop strong at-home habits and approach every child holistically — using gold-standard methods, high-quality materials, and care tailored to everyone’s needs and sensitivities. Because I have personally undergone many dental procedures, I deeply empathize with children in the dental chair. I use gentle techniques to help them move past fear and anxiety, transforming their visits into moments of pride. When children need extra time or additional appointments, we provide the patience and space they deserve. Beyond pediatric dentistry, I have a profound passion for international health. After completing a fellowship in Port-au-Prince, Haiti — providing emergency dental services in an outpatient HIV/TB clinic — I felt called to extend care to underserved communities worldwide. I have since volunteered in Guatemala, Nepal, and Peru, and I am building an ongoing partnership with a small dental clinic serving children of the Peruvian Amazon. My goal is to donate time, recruit other dentists, and help provide essential materials and equipment to this minimally funded clinic. Outside of my practice, I’m a playful and loving partner to my supportive Brazilian Husband, and a proud mom to our amazing daughter, Madalena, born in October 2024. I thrive outdoors — hiking, back-country trekking, skiing, scuba diving, running, playing soccer, beach volleyball, and surfing. I have a love for live music, dancing, and international backpacking adventures — whether solo, with friends, or now with my little family. Our dedicated team is here to provide gentle, compassionate care, ensuring the process is as comfortable and stress-free as possible. We specialize in pediatric population tooth extractions, addressing issues that cannot be resolved through other dental treatments. Understanding Pediatric Tooth Extractions Tooth extractions in children may be necessary for several reasons, including severe decay, infection, orthodontic correction, or trauma. Our primary goal is always to save the natural tooth, but when an extraction is the best option for your child's oral health, Dr. Espinosa and Dr. DeSimini are here to provide expert care. Our Approach to Tooth Extractions Gentle Techniques: Dr. Espinosa and Dr. DeSimini are trained in many different techniques, verbally and physically, to make the process as comfortable and easy as possible. We find that most of the fear and anxiety come at the start of the appointment and by the end we hear statements such as, “You’re done?!” “The tooth is out already?!” or “I didn’t feel a thing!” Sedation Options: Understanding that comfort and calmness are crucial, we may recommend Nitrous Oxide in conjunction with the local anesthetic. This very benign, mild anxiolytic gas is breathed throughout the appointment. The onset is fast, the patient is relaxed while awake, and the reversal is also quick, which allows a patient to even return to school. Upon assessment or in the beginning of the appointment, if we don’t feel the nitrous oxide is sufficient enough to help provide a positive experience for the dental extraction then we have wonderful referrals for local oral surgeons that work with children. They can safely deepen the sedation to help protect their developing psyche from medical procedure trauma. Aftercare Guidance: Post-extraction, we provide comprehensive aftercare instructions to ensure a smooth and swift healing process, including pain management and oral hygiene practices. After Your Child's Tooth Extraction We found that as long as patients follow these recommendations, then our patients do not experience post-extraction complications: Bite on gauze or tissue providing the extraction site with sufficient pressure for 5-10 minutes after the extraction. Continue for 5-10 minute intervals until bleeding or oozing has stopped. After the extraction and the bleeding has stopped, the child can drink liquids from a cup and eat mushy foods such as soup, smoothie, fruit puree with a spoon. Once the local anesthetic has worn off, patients can eat soft foods that require chewing such as rice, beans, noodles, boiled vegetables and may want to chew on the opposite side of the extraction site. Softer diet is recommended for comfort as long as the patient needs it. No sports or exercise for the rest of the day to prevent spontaneous bleeding or pain. No chips, seeds, nuts, or popcorn for 1 week to prevent sharp pieces from embedding into the healing socket. If this occurs a dental procedure visit with local anesthetic is required to clean out the socket and will prolong the healing process. No straws for 1 week to prevent dry socket which results in pain and prolonged bleeding. Ibuprofen and Tylenol combination per the child’s weight every 6 hours and right before bed the first night. Check in with the child in the morning to see if they still have any discomfort. If they do, continue with Ibuprofen/Tylenol combination every 6 hours through the day and right before bed. Continue daily until the child proclaims that they don’t feel pain in the area. Wisdom Teeth and Complex Extractions Please note : Our office does not perform Wisdom Tooth extractions. We may also recommend a referral to an Oral Surgeon to perform a complex extraction or extractions for extremely nervous or anxious patients. Our Sedation Dentistry Options Nitrous Oxide (Laughing Gas): Patients, big and small, can be slightly nervous for dental treatment, despite previous positive experiences at routine dental check-ups or even a positive treatment appointment. Nitrous oxide, also known as “laughing gas” or what Dr. Espinosa and Dr. DeSimini like to call “happy air”, is a great option to help them relax and feel comfortable. It is the most mild form of sedation in which the child is conscious and able to interact during the treatment. The patient will breathe the nitrous oxide/oxygen combination through a nose mask throughout the dental treatment appointment. The nitrous oxide effects of relaxing mental anxiety, relaxing muscle tension, and increasing threshold for discomfort comes on quickly with the full cooperation of the patient. A stuffy nose, mouth breathers, and active crying will interfere with the effectiveness of the nitrous oxide and will not be recommended at that time. Just as quickly as the onset of nitrous oxide takes effect, it is quickly eliminated from the body through normal breathing of 100% oxygen. This allows the patient to return to school or other daily activities after their visit. Nitrous oxide is non addictive, will not put your child to sleep, and has analgesic properties making injections less painful. Not only is it recognized by the American Academy of Pediatric Dentistry as safe and effective, it is widely used in the pediatric dental community to aid in positive experiences during dental procedures for children. In addition, our team may recommend Nitrous oxide for routine dental check-ups and x-rays for patients no matter what age that experience severe gag reflex. Oral Sedation: Oral conscious sedation is a process in which Dr. Espinosa or Dr. DeSimini will administer one or a mixture of several drugs orally and in the office to sedate a child for dental treatment. This type of sedation is usually used on apprehensive patients, younger children, or special needs patients who are not able to cooperate with nitrous oxide alone. This form of sedation can make the child drowsy and they may even fall asleep during treatment, but they are not unconscious. Unlike nitrous oxide, the patient will need a relaxing day at home with an adult after their sedation for full recovery. IV Sedation & General Anesthesia: I.V. Sedation is recommended for pre-cooperative children with SECC, Severe Early Childhood Caries, highly anxious or uncooperative patients, and patients with mental or physical disabilities. This process involves a Dental Anesthesiologist who will sedate the child into a deep sleep and monitor the patient while Dr. Espinosa or Dr. DeSimini will complete all necessary treatment in one visit. I am delighted to share some positive and exciting updates about Novato Children's Dentistry with all of you. Firstly, I wanted to take a moment to express my heartfelt gratitude for your continued support and trust in our practice. Your smiles and the well-being of your children are at the core of our mission, and we are committed to providing the highest level of care possible. On a personal note, the year 2023 has brought significant changes to my life. While I cherish the memories and miss my beloved companion and dog son, Ryker, who recently transitioned to a better place after spending 15 1/2 wonderful adventurous years with me, I am also embracing new beginnings. As I celebrate my 40th birthday this year, I am happily building a home with an incredible partner, and we are eagerly planning for a family of our own. The opportunity to serve children and make a positive impact in their lives remains my lifelong purpose, and I draw inspiration from the incredible families in our practice. In line with our commitment to delivering exceptional care, and while preparing for the growth of my own family, I am thrilled to announce a valuable addition to our team. Dr. Catey DeSimini , an outstanding pediatric dentist, has joined Novato Children's Dentistry ! With 14 years of experience in pediatric dentistry, Dr. DeSimini shares our thoughtful and comprehensive approach to patient care. As the year continues, I will personally introduce her to each one of you, ensuring a smooth transition as we approach our patients’ care as a united team. Allow me to provide a brief introduction to Dr. DeSimini. Born and raised in Lake County, California, she pursued a B.S. in Biological Sciences at the University of California, Santa Barbara (Go Gauchos!). She then went on to earn her Doctor of Dental Medicine (DMD) degree from Tufts University School of Dental Medicine. During her time at dental school, Dr. DeSimini actively participated in community outreach programs, including a dental service trip to Nicaragua with the non-profit organization, Bridges to Community. Her passion for children's dentistry grew during her rotation through pediatrics, leading her to obtain a certificate in Pediatric Dentistry from Tufts University. Dr. DeSimini's dedication and excellence have been recognized with the Richard C. Pugh Achievement Award, awarded to her for scoring in the top 3% nationwide on her Pediatric Dental Board Qualifying Exam. She is a Board-Certified Diplomate of the American Board of Pediatric Dentistry. Prior to joining our practice, she spent 12 years in private practice in the Boston area before relocating to San Francisco with her family in January 2021. Dr. DeSimini is thrilled to be closer to her loved ones and is excited to introduce her family to the places she cherished during her childhood. Dr. DeSimini is a proud member of the American Academy of Pediatric Dentistry, California Dental Association, American Dental Association, and California Society of Pediatric Dentistry. She resides in San Francisco with her husband, Sabino, and their two children, Luca and Anna. Our dental assistant, Heather , recently welcomed her beautiful baby girl, Aubrey, into the world. While Heather has transitioned to a new administrative role, she occasionally visits the office with baby Aubrey, adding a delightful touch to our workdays. We absolutely adore "take your daughter to work days" with Aubrey! During Heather's time working from home, we diligently searched for the perfect in office Dental Assistant, and I'm delighted to announce that we have found the ideal candidate. Having recently relocated from the East Bay back to the North Bay, where she grew up, Shay is excited to serve our community. The Importance of Perinatal Oral Health For Expectant Mothers: Good oral health during pregnancy is vital for the well-being of both mothers and their babies. The American Academy of Pediatric Dentistry (AAPD) recommends that all pregnant women receive oral healthcare and counseling from their general dentist during pregnancy. Research has shown that periodontal disease can increase the risk of preterm birth and low birth weight. Additionally, mothers with poor oral health may be at a greater risk of passing the bacteria which causes cavities to their young children. Mother’s can follow these simple steps to decrease the risk of spreading cavity-causing bacteria: Visit your dentist regularly. Brush at least 2 times a day and floss nightly to reduce bacterial plaque and load. Eat a proper diet, with the reduction of beverages and foods high in sugar & starch. Use a fluoridated toothpaste and rinse every night with an alcohol-free, over-the-counter mouth rinse with .05 % sodium fluoride in order to reduce plaque levels. Don’t share utensils, cups, food, or cleanse pacifiers with their mouths which can cause the transmission of cavity-causing bacteria to your children. Use of xylitol chewing gum (at least 4 pieces per day by the mother) can decrease a child’s caries rate. Routine oral examinations and cleanings. We’ve designed our dental exams to be as comfortable and enjoyable as possible for kids. Each visit begins with a friendly greeting from our front office coordinator, Tina, who might invite your child to draw pictures or help feed the fish in our saltwater tanks. Once ready, Dr. Espinosa, Dr. DeSimini, or one of our dental assistants will escort your family to the designated exam rooms. Before the exam, you will be asked if there are any changes to your child’s medical health, medications, diet, home habits, or cooperation with home hygiene and following instructions. All of these can affect how we prepare for their exam and their risk for infection. During the exam, we’ll take a comprehensive look at your child’s head, neck, and mouth. First, we look for any pathology and asymmetries of the neck, face, and head. Next, we examine the oral cavity which includes the size of the tongue and the tonsils to determine if they are affecting proper airway flow. After, we evaluate the teeth for abnormalities and infections (cavities), gums for health, the occlusion (bite), and any other soft tissue pathology of the mouth. Throughout this exam, we describe what we are doing so our patients feel engaged with the process and free to ask any questions. If recommended, we’ll use our low-radiation digital X-rays with the smallest sensors available to ensure your child’s comfort. Generally, our dental exam appointments include a dental cleaning and polishing. Our Registered Dental Assistants and our Doctors will use disclosing liquid to highlight the plaque on the teeth and gums as part of our oral hygiene instruction. We also include flossing and flossing technique as part of the hygiene instruction too. After, a fluoride treatment to strengthen enamel and protect against cavities is applied. I completed my undergraduate studies at the University of California Santa Cruz (UCSC) in human biology and earned my Doctor of Dental Surgery from University of California San Francisco (UCSF). I continued to complete my Pediatric Dentistry residency at University of Southern California (USC) and Children’s Hospital Los Angeles, where I specialized in the care and treatment of infants, children, adolescents, with emphasis on patients with special health care needs. I enjoy staying involved with local, state, and national organizations that promote and advocate for pediatric oral health. I served as the President of the Alameda County Dental Society. Silver Diamine Fluoride is an FDA-approved liquid medicament used to prevent tooth cavities (or dental caries) from advancing and to prevent new ones from forming. Comprising of silver, water, fluoride, and ammonia, SDF serves multiple roles: Silver acts as an antimicrobial agent, killing bacteria. Fluoride helps in the remineralization of the tooth enamel, making the tooth surface more resistant to decay. Ammonia stabilizes the solution, allowing the effective components to remain potent. SDF may also be used in combination with traditional restorative treatment for added prevention. Application Process: The cavitated tooth is cleaned and dried. SDF is then carefully applied to the decayed lesions of the teeth with a small brush or a micro-sponge for at least 1 minute. December 2024 Dr. DeSimini joined the practice last year, allowing Dr. Espinosa time to focus on starting her family. Now, as she prepares to welcome her daughter, Madalena Marcia Oliveira, she is officially on leave until the end of January. What a year 2024 has been personally and professionally! With Dr. DeSimini joining the practice last year to help share in providing quality dental care so I may have the time and space to work on starting my family, I became pregnant at the beginning of this year! I would also like to shout out our team members, Tina Rodgers who coordinates our schedule and the front office, Icelya “Cel” Pacheco our awesome dental assistant, and Heather Natsios who manages all the office’s back-end tasks and auditing from her home office. Without our team dynamic I would not have been able to enjoy my pregnancy as much as I have over the past 9 months. Now is the time that I can declare that I am officially taking full time leave to prepare and welcome our little girl, Madalena Marcia Oliveira , into our lives! Dr. DeSimini and our team will hold down the fort until the end of January. During this time, we will also be welcoming to the practice Dr. Ella Saeed, a Novato native, fellow USC/Children’s Hospital Los Angeles trained board-certified pediatric dentist. (Bruxism) It is not uncommon and in fact many children will at some point in their development experience grinding their teeth while they sleep. Most parents will report an extreme noise created by the child grinding their teeth at night or notice wear/shortening of their child’ enamel and be concerned of damage to their teeth. Bruxism can be due to psychological and/or physical components. Psychologically, stress due to new environments, schedules, divorce, changes at school, friendships, workload, etc. can influence a child to grind their teeth. Physically, bruxism may be a result of sleep apnea. Upon evaluation, Dr. Espinosa or Dr. DeSimini may refer your child to an ENT specialist (ear, nose, and throat doctor) for imaging and evaluation to assess for sleep apnea. The majority of cases of pediatric bruxism do not require any treatment. If excessive wear of the teeth (attrition) is present, then a mouth guard (night guard) may be indicated, but is also limited based on the age and teeth/jaw development of the child. The good news is most children outgrow bruxism. The grinding decreases between the ages 6-9 and children tend to stop grinding between ages 9-12. Heather welcomed her new baby girl on Sunday, August 14th 2022, weighing in at 6lbs 3oz, and measuring a full 22 inches long. Baby girl, Aubrey, has been a sound sleeper and easy-going baby since the day she was brought into this world. She adores her big brother, Wyatt, who is now 4 years old and been really stepping into his role as the big sibling in the family. Whenever Heather is not directly with the baby, you can bet that Aubrey has her eyes locked on her Mama just watching everything she does. Wyatt is very excited to have gone back to school this year and is currently working really hard to learn how to ride his bike with his Mom and Dad on their nightly walks. As many of our firefighter families know, the kids are always so excited when Daddy (Brian) gets to return home from his work at the Mill Valley station to spend time with the family, especially around the Holiday Season! We are looking forward to having Heather return back to the office in the New Year to assist with the some of the more "administrative" tasks of the front desk, but in the meantime, we are wishing her and her family all of the best as they get to wind down the year and enjoy the Holiday Season together.
