When treatment is complete, your child will have no recollection of the time it took, the pain, the noise, or the discomfort. We are pleased to offer your child the option of comfortable, safe, and effective sedation dentistry. Our pediatric dental patients who benefit from sedation dentistry have: A fear or anxiety of being at the dentist A hard time sitting still for long periods of time A hard time getting (and staying) numb from anesthetics What is sedation dentistry? Sedation dentistry allows your pediatric dentist to provide a variety of dental treatments safely and comfortably for children who experience anxiety when visiting the dentist. There are several benefits to sedation dentistry, including: No memory of undergoing the procedure No sense of time while under sedation No sense of smell or sound No fear or anxiety during treatment Levels of Sedation Sedation dentistry is closely regulated by law, and there are three sedative states at which your child's dentist can administer treatment: mild sedation, moderate sedation, and deep sedation. Mild Sedation — Nitrous Oxide (laughing gas) inhalation is the lightest form of sedation dentistry and is often used for children with mild anxiety. Your child will remain awake throughout the entire procedure and will be able to breathe normally, but he or she will feel a great sense of relaxation.  Moderate Sedation — Used for patients with moderate dental anxiety and for patients who need longer or more complex procedures, conscious sedation often refers to the use of light sedation typically taken orally. With conscious sedation, your child will remain awake throughout the procedure, but will be in a deep state of relaxation. Patients may feel a little drowsy afterward but typically fully recover within a couple of hours.   Deep Sedation — Patients receiving deep sedation go between consciousness and unconsciousness during their dental procedure. Patients often have no recollection of the treatment and are unable to respond to commands even if they are awake at times during the procedure. This is usually administered via an IV with the help of a board certified dental anasthesiologist.  Recovery is usually a couple of hours after treatment is completed.   How are sedatives administered? Inhalation — Nitrous oxide, or laughing gas, is the most frequently used method for easing mild to moderate anxiety. Recovery is quick so your child can resume normal activities immediately. Oral — An extremely common technique for dental sedation is oral sedation. It is easy and does not require the use of needles. Your child will be given a prescription pill which, taken about an hour before the appointment, will have your child fully relaxed by the time you arrive at our office. She was encouraged to go into dentistry by her siblings who are also in the field. She absolutely loves working with both the little ones and their families to help promote a positive life long relationship with the dentist.  Dr. Meekay attended the University of California, Davis, where she received her Bachelor of Science in Neurobiology, Physiology & Behavior. She then attended the University of the Pacific, Arthur A. Dugoni, School of Dentistry where she earned her Doctorate of Dental Surgery(DDS). Upon completion of her DDS, she completed a one year General Practice Residency in Brooklyn, New York. After one cold winter on the east coast she came running back to the Bay area where she was awarded her Masters of Science in Craniofacial Sciences and her Advanced Education certificate in Pediatric Dentistry at the University of California, San Francisco (UCSF).  Dr. Meekay enjoys providing patients with the knowledge to improve and maintain a healthy mouth! Dr. Meekay is constantly learning and implementing new up-to-date procedures and techniques. She is also involved in organized dentistry, and finds it important to stay active in dental societies to best advocate for her patients and community. During dental bonding, a white filling is placed onto your child's tooth to improve its appearance. The filling “bonds” with the tooth, and because it comes in a variety of tooth-colored shades, it closely matches the appearance of your child's natural teeth. Tooth bonding can also be used for fillings instead of amalgam. Many patients prefer bonded fillings because the white color is much less noticeable than silver. Bonding fillings can be used on front or back teeth, depending on the location and extent of tooth decay. Bonding is less expensive than other cosmetic treatments and can usually be completed in one visit to our office. However, bonding can stain and is easier to break than other cosmetic treatments, such as porcelain veneers. If it does break or chip, tell your doctor. The bonding can generally be easily patched or repaired in one visit. ↑ Back to top Dental Crowns Crowns are a restorative procedure used to improve a tooth’s shape or to strengthen a tooth. Crowns are most often used for teeth that are broken, worn, or have portions destroyed by tooth decay. A crown is a “cap” cemented onto an existing tooth that usually covers the portion of the tooth above the gum line. In effect, the crown becomes the tooth’s new outer surface. Crowns can be made of porcelain, metal, or both. Porcelain crowns are most often preferred because they mimic the translucency of natural teeth and are very strong. Crowns or onlays (partial crowns) are needed when there is insufficient tooth strength remaining to hold a filling. Unlike fillings, which apply the restorative material directly into the mouth, a crown is fabricated away from the mouth. A crown is created in a lab from your child's unique tooth impression, which allows a dental laboratory technician to examine all aspects of your child's bite and jaw movements. The crown is then sculpted just for your child so that his or her bite and jaw movements function normally once the crown is placed. ↑ Back to top Teeth Extractions There are times when it is necessary to remove a tooth. Sometimes a baby tooth has misshapen or long roots that prevent it from falling out as it should, and the tooth must be removed to make way for the permanent tooth to erupt. At other times, a tooth may have so much decay that it puts the surrounding teeth at risk of decay, so the doctor may recommend its removal. Infection, orthodontic correction, or problems with a wisdom tooth can also require removal of a tooth. When it is determined that a tooth needs to be removed, your child’s dentist may extract the tooth during a regular checkup or may request another visit for this procedure. The root of each tooth is encased within the jawbone in a “tooth socket”, and the tooth is held in that socket by a ligament. In order to extract a tooth, the dentist must expand the socket and separate the tooth from the ligament holding it in place. While this procedure is typically very quick, it is important to share with the doctor any concerns or preferences for sedation. ↑ Back to top Dental Fillings Traditional dental restoratives, or fillings, may include gold, porcelain, or composite. Newer dental fillings include ceramic and plastic compounds that mimic the appearance of natural teeth. These compounds, often called composite resins, are typically used on the front teeth where a natural appearance is important. There are two different kinds of fillings: direct and indirect. Direct fillings are fillings placed directly into a prepared cavity in a single visit. Indirect fillings generally require two or more visits. These fillings include inlays, and veneers fabricated with ceramics or composites. ↑ Back to top Fluoride Fluoride is effective in preventing cavities and tooth decay and in preventing plaque from building up and hardening on the tooth’s surface. A fluoride treatment in a dentist’s office takes just a few minutes. After the treatment, your child may be asked not to rinse, eat, or drink for at least 30 minutes in order to allow the teeth to absorb the fluoride. Depending on your child’s oral health or the doctor’s recommendation, a fluoride treatment may be required every three, six, or 12 months. ↑ Back to top Custom Mouthguards Whether your child wears braces or not, protecting his or her smile while playing sports is essential. Mouthguards help protect the teeth and gums from injury. If your child participates in any kind of full-contact sport, the American Dental Association recommends that he or she wear a mouthguard. Choosing the right mouthguard is essential. There are three basic types of mouthguards: the pre-made mouthguard, the “boil-and-bite” fitted mouthguard, and a custom-made mouthguard from the dentist. When you choose a mouthguard, be sure to pick one that is tear-resistant, comfortable and well-fitted for your mouth, easy to keep clean, and does not prevent your child from breathing properly. Are you prepared for the arrival of your baby’s first tooth? Follow these guidelines and your son or daughter will be on the way to a lifetime of healthy smiles! Caring for Gums Even before your baby’s first tooth appears, the gums can benefit from your careful attention. After breast- or bottle-feeding, wrap one finger with a clean, damp washcloth or piece of gauze and gently rub it across your baby’s gum tissue. This practice both clears your little one’s mouth of any fragments of food and begins the process for building good daily oral care habits. Baby’s First Tooth When that first tooth makes an entrance, it’s time to upgrade to a baby toothbrush. There are usually two options: a long-handled toothbrush that you and your baby can hold at the same time, and a finger-puppet-like brush that fits over the tip of your pointer finger. In each case, the bristles are soft and few. At this stage, toothpaste isn’t necessary; just dip the brush in water before brushing. If your little one doesn’t react well to the introduction of a toothbrush, don’t give up. Switch back to a damp washcloth for a few months and try the toothbrush again. During the teething process, your child will want to chew on just about anything, and a baby toothbrush with a teether can become a favorite toy during this period. Brushing with Toothpaste When a few more teeth appear, you can start using toothpaste with your child’s brush. At this stage, use only a tiny amount of fluoridated toothpaste (the size of a grain of rice). From the beginning, have your little one practice spitting the toothpaste out after brushing, which should not be swallowed at any age. Avoiding Cavities Don’t give your baby any sort of sweetened liquids such as flavored drinks or soda. Even the sugars present in fruit juice, formula, and milk (this goes for breast milk as well) can cause decay, so regular teeth and gum cleaning is vital. Also, make sure your baby never goes to bed with a bottle; sugary liquids in prolonged contact with teeth are a guarantee for early-childhood decay, also called baby-bottle caries. First Visit to the Dentist It’s recommended that you bring your baby in for a visit within six months of the first tooth’s eruption – usually around his or her first birthday. Since decay can occur in even the smallest of teeth, the earlier your baby visits us, the more likely he or she is to avoid problems. We’ll look for any signs of early problems with your baby’s oral heath, and check in with you about the best way to care for your little one's teeth. Remember that preparing for each dental visit with a positive attitude goes a long way toward making your child comfortable with regular checkups. Setting a Good Example As part of the natural learning process, little ones are expert mimics, and you can take advantage of this talent. Brush and floss daily while your child is watching, and he or she will intuit at an early age the importance of your good habits. As soon as your child shows interest, offer a toothbrush of his or her own and encourage your toddler to “brush” with you. (You’ll find toothbrushes with chunky, short handles that are easy to grip.) Most children don’t have the dexterity necessary to thoroughly clean their own teeth until they’re about six or seven, so you’ll have to do that part of the job. Try different tactics to make brushing fun: flavored toothpaste, a toothbrush with a favorite character on it, or singing songs about brushing. It is made with two acting ingredients: the silver component is an anti-microbial agent that kills bacteria and prevents the formation of new biofilm, and the fluoride prevents further demineralization (softening) of the tooth structure. Treatment with SDF does not eliminate the need for restorative dentistry (fillings, crowns, etc.) to repair function or aesthetics, but is effective at preventing further decay. SDF is a simple and noninvasive way to treat carious lesions (cavities), and it can be done from the comfort of our dental office. The teeth are brushed without paste and rinsed. The carious teeth are isolated, kept dry, and all excess debris is removed. A microbrush is dipped in a drop of SDF and placed on the lesion(s) for two minutes. Any decayed areas will stain black. Healthy tooth structure will not stain. Excess SDF is removed and patients are instructed to not eat or drink for one hour. Why use SDF? Bitten Lip or Tongue If your child has bitten their lip or tongue severely enough to cause bleeding, clean the bite gently with water and use a cold compress (a cold, wet towel or washcloth pressed firmly against the area) to reduce or avoid swelling. Give us a call to help determine how serious the bite is. Object Caught In Teeth If your child has something caught between their teeth, use dental floss to gently remove it. Never use a metal, plastic, or sharp tool to remove a stuck object. If you are unable to remove the item with dental floss, give us a call. Broken, Chipped, or Fractured Tooth If your child has chipped or broken a piece off of their tooth, have them rinse their mouth with warm water, then use a cold compress to reduce swelling. Try to locate and save the tooth fragment that broke off. Call us immediately. Knocked Out Tooth If your child’s tooth has been knocked out of the mouth, find the tooth and rinse it with water (no soap), taking care to only touch the crown of the tooth (the part you can see when it’s in place). Place the tooth in a clean container with milk. Call us immediately and/or head to the hospital. If you act quickly it's possible to save the tooth. Loose Tooth If your child has a very loose tooth, it should be removed to avoid being swallowed or inhaled. Toothache If your child complains of a toothache, rinse their mouth with warm water and inspect the teeth to be sure there is nothing caught between them. If pain continues, use a cold compress to ease the pain. Do not apply heat or any kind of aspirin or topical pain reliever directly to the affected area, as this can cause damage to the gums. Children’s pain relievers may be taken orally. Schedule an appointment immediately. Broken Jaw If you know or suspect your child has sustained a broken jaw, use a cold compress to reduce swelling. Call our emergency number and/or head to the hospital immediately. In many cases a broken jaw is the result of a blow to the head. Severe blows to the head can be dangerous and even life-threatening. Avoiding Injury You can help your child avoid dental emergencies. Child-proof your house to avoid falls. Don't let your child chew on ice, popcorn kernels, or other hard foods. Always use car seats for young children and require seat belts for older children. And if your child plays contact sports, have them wear a mouthguard. When new teeth arrive Your child’s first primary, or “baby,” teeth will begin to erupt between the ages of six to 12 months, and will continue to erupt until about age three. During this time, your child’s gums may feel tender and sore. To help alleviate this discomfort, we recommend that you soothe the gums by rubbing a clean finger or a cool, wet cloth across them. You may also choose to make use of a teething ring. Your child’s primary teeth are shed at various times throughout childhood. Permanent teeth begin erupting at age six, and continue until age 21. Adults have 28 permanent teeth (32, teeth including wisdom teeth). Adopting healthy oral hygiene habits As your child’s teeth erupt, be sure to examine them every two weeks, looking for lines and discoloration that may be caused by decay. Remember that sugary foods and liquids can attack a new tooth, so take care that your child brushes his or her teeth after feeding or eating. We recommend brushing two times a day for optimal oral hygiene. Brushing can be fun, and your child should brush as soon as the first tooth arrives. When a baby’s tooth erupts, parents should brush the tooth with a soft-bristled toothbrush and a pea-sized amount of toothpaste.
