Our patients feel nothing during any procedure due to our anesthetics. Dr. Frey offers sedation, not for pain, but to help patients deal with strong feelings of fear and anxiety that arise when they visit the dentist. After receiving sedation and successfully completing their appointment, patients often change the entire way they view dental appointments going forward. Sedation could be a great option if you: ● Have difficult overcoming anxiety about the dentist ● Have a low pain threshold ● Have very sensitive teeth ● Have a very strong gag reflex ● Need a large amount of work done Once you are sedated and comfortable, we then properly numb, or anesthetize, the area we are working on, ensuring that your procedure is pain-free. How Does Sedation Dentistry Work? It’s simple: you’re given a small pill to take 1 hour before your appointment, and you will feel very drowsy by the time you arrive. At the office, you’re given a thick, warm blanket, and we make sure you’re absolutely comfortable. Is Sedation Dentistry Safe? We closely monitor the dosages for our sedation dentistry patients, plus our IV sedation is handled by an anesthesiologist, so you can be assured you’ll be properly sedated and comfortable, yet completely safe. What Types Of Sedation Is Offered At David S. Frey, DDS? At Dr. Frey’s, we offer two forms of sedation, oral or IV sedation. Both methods deliver a similar sort of relaxation and a kind of twilight sleep, where you’re awake but totally relaxed. Both methods also tend to have an amnesia type of effect afterward, where you don’t remember much about your appointment. Sedation Vs. Sleep Dentistry This is a bit of a misnomer. Patients sometimes think they will be put “under,” as they are with general anesthesia. This is not the case. With both oral and IV sedation, the patient is always awake but sedated. What Is Oral Sedation? With oral sedation, a pill, usually similar to Valium, is given about 30 minutes to an hour before the start of your procedure. This will make you drowsy, but you’re still awake and you can still respond to certain directions from Dr. Frey. If you prefer moderate sedation, we can simply increase the dosage. How Long Does Oral Sedation Last? For most patients, the effects of oral sedation last from two to four hours after their appointment. You need a ride to and from your appointment, plus shouldn’t drive for an additional 12 hours. What Are The Risks With Oral Conscious Sedation? Oral sedation also has an amnestic effect. While technically a side effect, most patients see this as a benefit because they have no recollection of their dental visit. Oral medications can also cause dry mouth, headaches, dizziness, and occasional nausea, but these all pass quickly. What Is IV Sedation? At Dr. Frey’s, we have an anesthesiologist who handles our IV sedation. This type of sedation is delivered intravenously and works instantly. An advantage to IV sedation is that we can adjust the levels continually, for instance when Dr. Frey may need you to respond to more instructions, we can lighten the level for a bit and then increase it again. How Long Does IV Sedation Last? For some patients, IV sedation wears off almost immediately after the administration is stopped. For others, they can feel its effects for a few hours after their appointment is over. Patients shouldn’t drive or operate machinery for 24 hours afterwards. Can I Drive After Sedation Dentistry? With either oral or IV sedation, you should not drive afterwards (12 hours for oral sedation and 24 hours for IV sedation). So, plan on getting a ride to and from our offices. Can Sedation Dentistry Allow Me To Handle More Involved Appointments? Another great thing about sedation dentistry with Dr. Frey is that it allows you to handle longer, more involved appointments. Often more complex procedures, such as smile makeovers or root canals can involve more time for your appointment, or could often require multiple appointments. When a patient is sedated, however, Dr. Frey is able to perform more work in a single session. Sore gums can be rejuvenated, crowns and dentures can be replaced, yellow or stained teeth can be whitened, and chipped teeth can be bonded or covered with veneers — all while you’re basically in twilight sleep. Temporomandibular joint disorder isn’t a sudden event, such as injuring your knee. It can start in childhood, and can be evidenced by mouth breathing. Mouth breathing is often a sign of a misaligned jaw. Because the tongue needs to sit lower when a person breaths through his or her mouth, this can affect the development of the upper jaw in a child, leading the jaw to develop in the shape of a V unlike the U-shape it would normally be. Those would be congenital problems with bite balance and alignment. But often TMJ develops as a result of stress and nightly teeth clenching during sleep. Traumatic injury to the jaw or jaw joints can lead to TMJ. Research also shows a genetic tendency. The obvious signs include an under-bite or an over-bite. An unbalanced bite could result from one cause, or a combination of factors. Allergies during childhood, thumb sucking, old dentistry that needs to be replaced, or the shifting of teeth due to tooth loss can all lead to a bad bite. Injury or trauma An injury or trauma to your jaws from an accident or any intense physical impact, can lead to damage to your muscles or joints. Any time your body absorbs the impact of a serious blow, it transfers throughout the rest of your body, altering the function of the muscles and joints in your head, neck, and face. This results in discomfort and irritation related to TMJ. Skull deformation Pressure and force exerted on a baby’s head during birth affects the shape and development of the skull and the bones in the head and face. If there is a misalignment of the bones, muscles, or ligaments, this can alter the development of the TMJ. It can also result in the misalignment of jaws and skull bones, causing an unbalanced bite. Genetics The fact is that your jaw structure, teeth positioning, and entire facial composition are inherited. This can include the sensitivity to your environment leading to allergies and congestion. If you inherited a small jaw or if your teeth are too large to fit your mouth, the result is a bad bite and misaligned jaw. Airway issues Airway issues can cause TMJ pain, an improperly formed jaw, and bite issues. If you have an unbalanced bite, your tongue can’t sit where it should, which as a result can block air from getting to your lungs. Improper breathing may be due to childhood allergies. When a child is constantly congested, the tongue drops down allowing air to pass over it. The tongue is no longer in balance and the upper jaw’s growth changes over time. This can result in a compromised breathing airway. Unbalanced dentistry Even if you have gone through orthodontic work to straighten your smile, there could still be invisible problems. You can have a perfectly straight set of teeth but still be suffering from TMJ symptoms and have a misaligned bite. Who Is At Risk For Developing TMJ? There are no reliable statistics on the number of Americans who have TMJ, but it is estimated to be over 10 million people. TMJ is more common in women than men. Stress is a known risk factor. If a person is under an inordinate amount of stress he or she may begin clenching their teeth during the day and grinding them at night. This begins a sequential effect of stressed muscles, damaged teeth, and chronic pain. There also appears to be a genetic factor in developing the condition. Can Stress Cause TMJ? Stress can be exasperated by TMJ but doesn’t cause it. Some doctors believe in Psychosocial Biology which states that stress causes TMJ, but Dr. Frey doesn’t believe this. Dr. Frey believes in the Structural-Functional Camp, meaning there is some sort of structural imbalance in the cranium and/or bones causing the TMJ. Because there is a shared ligament from the temporomandibular joint into the ear, TMJ disorder can cause tinnitus. If the shared ligament is super tight it causes ringing in the ears . Can Braces Cause TMJ? Braces could cause TMJ because the original bad bite would cause an imbalance in the joint. Pain may not have been felt before the braces, but shifting the teeth can cause the TMJ pain to increase while the imbalances in the jaw are corrected. Can Someone With Dentures Get TMJ? Yes, especially if the vertical dimension is too low meaning they are over-closed. This could cause the jaw joint to go too far back in the patients’ head. What Causes Some TMJ Night Guards Not To Work? Night guards may not be sufficient treatment for TMJ as they can cause an imbalance in harmony in the jaw joint muscles of the head and neck. A calm bite means calm muscles which is head-ache free. We say that the bite is like the terminus of the spine. The reason the lower jaw can move in any direction allows for stress in the body to release itself through the mouth. Posture has been shown in the literature to be affected by your bite position. Breathing and the size of your airway is affected by how your teeth come together. When the lower jaw comes forward, the tongue follows it and thus open the airway. Nutrition and how you chew your food is affected by your bite. The better you can chew your food, the better digestion healthier you will feel. A healthy bite can allow patients to avoid future dental problems like breaking teeth and allow their teeth to last for years. How is an overbite and underbite corrected? Technology today can allow dentists to find a physiologic correct (harmonized) bite without surgery or braces. For many years, the most common recommendation for correction was orthognathic, or jaw-correcting surgery. This is un-necessary. Can I avoid surgery with non-surgical bite correction? Many times, patients come to us and inform us that they were told they needed Jaw Surgery to correct their deep over-bite, under-bite, cross bite or just plain old bad bite. We have developed many systems that allow us to correct and re-align your bite without surgery. This is how we do it….. Phase One: Finding the sweet spot! Calm muscles, a Decompressed Jaw Joint and a Level Bite! This phase involves the use of TENS and/or EMG to relax the overworked muscles of the jaw. Only when these muscles are relaxed can the doctor can obtain the most accurate data related to bite. We call this Physiologic Rest! This means determining proper bite position, jaw joint position , and calm low active muscles, is the sweet spot we are looking for. We then capture this position using dental Play doh. This position that is captured is used to design a “trial bite” that is worn for a short time. We call this an orthotic. Patients wear this for anywhere from 1 to 3 months. When this trial bite has been thoroughly tested and the patient feels healthy and good, phase two may begin. Phase Two: Porcelain Restoration vs Orthodontics vs A Hybrid of the two–start with Ortho and finish with Porcelain This phase involves keeping the bite position we found indefinitely. How can we do that? By either holding it there with the fabrication of porcelain restorations , such as porcelain veneers or dental crowns or moving the teeth into this position using Orthodontics. Non-surgical correction for overbite or underbite can be achieved using this method How long does it take to correct and Bad Bite? Typically we find harmony between the muscles of the head and neck, the jaw joint and the bite by using an orthotic. The orthotic is placed in the position of the new bite. The orthotic takes two visits to diagnose, and fabricate spaced out within a week between dental visits. We like for our patients to acclimate to it for one to three months and make sure its comfortable.
