Ovarian cysts are pouches filled with fluid or tissue that are located inside of the ovary. Common ovarian cysts are rarely malignant (cancerous) and can happen in women of any age. Functional cysts Functional cysts are the most common type. They usually have no symptoms and go away on their own.As the name suggests, these can be a normal finding in most women Teratomas These cysts contain different tissue types from elsewhere in the body, usually skin or hair. In rare cases, teratomas can be cancerous. Cystadenomas These cysts are large, often benign tumors that grow on the outside of the ovary causing pain and affecting fertility. Complex Cysts These cysts require closer observation and may require surgical removal. What causes ovarian cysts? A variety of conditions cause ovarian cysts. Women with polycystic ovarian syndrome (PCOS) tend to develop cysts when ovulation is disrupted in the body. Endometriomas are cysts that happen in women with endometriosis, a condition where tissue that normally lines the uterus grows outside of the uterus. The goal of the evaluation of any patient with an adnexal mass or ovarian cyst is to determine the cause or source of that mass or cyst. How are ovarian cysts diagnosed? Most women who experience ovarian cysts will never experience symptoms, and the cysts will be resolved on their own within six to eight weeks after their development. Thus in many situations, follow-up appointments with your doctor and ultrasounds are necessary. In every case, the key to treatment of ovarian cysts relies on the successful diagnosis of the cyst type. Some cysts will be discovered during your routine pelvic exam. Others will be examined if you experience pelvic pain during sex or menstruation. Large cysts, such as cystadenomas, cause a condition called ovarian torsion where the ovary is twisted causing pain on one side of the abdomen that comes and goes or starts suddenly. An ovarian torsion is considered an urgent condition that usually requires rapid diagnosis and surgical treatment. Any ovarian cyst can rupture. This is called a ruptured ovarian cyst or a hemorrhagic cyst.  A very typical presentation of a ruptured ovarian cyst is sudden onset of abdominal pain that is usually mid-menstrual cycle and can immediately follow sexual intercourse. Sometimes immediate surgical intervention is required if there is a concern for heavy internal bleeding from a ruptured ovarian cyst. In the majority of cases, a ruptured ovarian cyst can just be managed with observation and follow-up with your doctor. Evaluation normally includes a pelvic ultrasound. How are ovarian cysts treated? The management of an ovarian cyst depends on our evaluation of that cyst and your symptomatology. Ultrasound examinations and blood tests can give your OB/GYN an idea of the type of ovarian cyst, and a treatment plan can then be developed. Your OB/GYN may also use hormonal therapy or other treatments to regulate hormone levels, treating both the cysts and other symptoms of hormonal imbalance. Many benign appearing cysts are managed with close surveillance alone which includes serial pelvic ultrasounds. Some ovarian cysts require surgery to remove the cyst to alleviate symptoms and determine whether it is cancerous or not. Some women have risk factors that qualify their pregnancies as high-risk from the start, while others develop an unexpected complication that causes their normal pregnancy to become high risk. Regardless of the underlying reason, having a high-risk pregnancy means that you or your baby are more likely to experience unique medical challenges before, during, or after delivery. These challenges can range in severity from minor to life-threatening, making extra care and monitoring from your OB/GYN all the more essential. Whether you’re currently trying to conceive or you’re already expecting, here’s what you should know about the circumstances that can put your pregnancy in the high-risk category. Maternal age When you hear the term high-risk pregnancy, you may automatically associate it with some type of maternal or fetal health problem. But simply being over the age of 35 (especially if it’s your first pregnancy) puts you in the high-risk category, because women past this age are more likely to experience prolonged labor, delivery complications, or require a cesarean delivery. Women of “advanced maternal age” are also more likely to have a baby with a genetic or chromosomal disorder.   Young teenagers fall into the high-risk category, too, as they’re more likely to develop pregnancy-related high blood pressure. They also tend to go into labor earlier and deliver low-birthweight babies more often than more mature women. Medical conditions Preexisting health conditions automatically place additional risks on any pregnancy. Women who’ve been diagnosed with a chronic illness like kidney disease, lupus, thyroid disease, epilepsy, or sickle cell anemia have high-risk pregnancies, as do women who are affected by clinical depression or an anxiety disorder. Common preexisting medical conditions that automatically make any pregnancy high-risk include: Diabetes If you don’t manage your diabetes properly throughout your pregnancy, you have an increased risk of developing gestational hypertension or giving birth early. You’re also more likely to give birth to a very large baby or a baby that has low blood sugar levels, breathing problems, or birth defects.    High blood pressure Poorly managed high blood pressure can cause your baby to develop more slowly than normal or arrive earlier than expected. Placental abruption, a potentially life-threatening complication of high blood pressure, occurs when the placenta separates from your uterus before your baby is born. Obesity If you have a body mass index (BMI) of 30 or higher, you’re more likely to develop gestational diabetes, Type 2 diabetes, and high blood pressure during your pregnancy. Obesity also makes it more likely that your labor will need to be induced or you’ll require a cesarean delivery. Pregnancy complications A pregnancy that starts out normal can become high-risk when certain complications emerge. Women who develop high blood pressure, gestational diabetes, or any other significant health problem during pregnancy become high-risk patients, as do women who develop some type of problem with their placentas. Preeclampsia, which can emerge during the second half of pregnancy, occurs when you develop high blood pressure and protein leaks into your urine. This high-risk condition can adversely affect your health and significantly slow your baby’s growth and development. Because giving birth is the way to resolve preeclampsia, you may need to deliver early if you develop this condition. Fetal complications and multiples Pregnancy risks are higher if you’re having twins, triplets, or other multiples, because carrying more than one baby places additional stress on your body. Your babies are also more likely to develop complications or arrive early. You may be considered high-risk if you have a history of miscarriage, preterm labor, or cesarean delivery. Likewise, if you’ve already had one child with a birth defect, any subsequent pregnancies may be treated as high-risk. This helps ensure you receive extra diagnostic testing to detect birth defects in utero; in some cases, defects can be treated before birth.   Expectant mothers with herpes, chickenpox, and other dangerous infections require careful fetal monitoring to reduce the risk of birth defects or growth problems. You may also require extra care and monitoring if your baby doesn’t appear to be growing normally. With top-level obstetrics and gynecology training from one of the most revered medical centers in the world, she provides expertise on pelvic pain, infertility evaluations, and minimally invasive surgeries. Dr. Valenton received her Bachelor’s and Master’s Degrees in Psychology from the University of California, Berkeley, moving on to work as a senior therapist for children with autism. After that, she earned her Medical Degree from the University of Vermont College of Medicine. Following her time in Vermont, she completed her internship and residency at the world-renowned Cedars-Sinai Medical Center in Los Angeles, California. There she received exceptional training in the fields of obstetrics and gynecology, where she is still an attending physician to this day.
