Rosacea is a common inflammatory skin condition affecting more than 14 million people in the United States. There are different forms of rosacea. The most common form presents as facial redness over the nose and cheeks. Patients often describe their tendency to flush and blush easily. Other forms of rosacea consist of acne-like breakouts, thickening or “hypertrophy” of oil glands leading to a bulbous nose, and irritation, redness, and swelling of the eyes. Book Now Rosacea often presents between the ages of 30 and 50, and risk factors for this condition include being fair-skinned, blond-haired or blue-eyed and being from Celtic or Scandinavian ancestry. Although we don’t know exactly what causes rosacea, family genes, dysregulation of the immune system, and a skin mite called  demodex  that lives on everyone’s skin may play a role. Depending on the type of rosacea you have, treatments may be targeted toward improving facial redness, pus bumps, or thickened skin. Treatments might include laser therapy, topical or oral medications, and surgical procedures. Although rosacea is a chronic problem, your symptoms can be improved. Allergic reactions to foods or medications may cause full body skin rashes. The key to treating these rashes is stopping consumption of the food or medication causing the rash. Once this occurs, the rash often dissipates. Sometimes resolution of the rash can be accelerated by use of topical or oral medications. However, some rashes may be so severe that they require immediate medical attention. If you develop an associated fever, chills, throat swelling, difficulty breathing, or headache with your rash, it is important that you seek medical care immediately. Eyelid surgery, also known as blepharoplasty, is a procedure that improves the appearance of the upper and/or lower eyelids. Blepharoplasty can be helpful if you have sagging, drooping eyelids that make you look and feel tired. The procedure can also be used if excess skin makes it challenging to see and impacts your visual field. A facial plastic surgeon at Moy-Fincher-Chipps removes excess tissue from the area around the eyes to improve their appearance. Sometimes fat will also be repositioned to help rejuvenate the face. Blepharoplasty is an excellent way of correcting the appearance of drooping upper eyelids or bags under your eyes that make you look older than you are. By having a blepharoplasty, you can look more alert, rested, and youthful. Upper Eyelid Surgery An upper blepharoplasty is eyelid surgery performed on the upper eyelids. Eczema is a general term used to describe itchy, red, often scaly skin, and is most commonly associated with either  atopic dermatitis  or  contact dermatitis . Book Now Atopic dermatitis  is a skin condition that often presents in childhood, usually in the first year of life. The skin’s barrier is compromised and it becomes extremely itchy. Scratching and rubbing result in a red rash that may flake, blister, or even bleed. In babies, the rash is commonly seen on the scalp, forehead, and cheeks. We don’t know the exact cause of atopic dermatitis, but both genetics and the environment play a role. Although atopic dermatitis can extend into adulthood, most children grow out of the condition by the time they are teenagers. Treatment of the eczematous rash associated with atopic dermatitis can be challenging and requires patience and persistence. Your doctor will devise a careful skin care regimen and may prescribe topical or oral medications depending on the quality and extent of your rash. Keeping your eczema under good control is important to minimize itch and to prevent skin infections, which are more common on eczematous skin. Contact dermatitis  also causes an eczema-type itchy red rash. In these cases, the skin is either irritated by or allergic to a caustic substance. Irritants might include acids, corrosives, or chemicals, and allergens might include plants like poison ivy, metals like nickel, preservatives, fragrances or other chemicals. Whatever the culprit substance, the skin reacts by becoming itchy, red, scaly, and occasionally blistered. The first step of treatment is stopping skin contact with the irritant or allergen. I came to Los Angeles from Colorado in 1992, and I fell in love with the vitality and culture of this city. After training at UCLA and USC, I joined MFC at UCLA. Cysts are common and may occur anywhere on the skin, especially the back. A cyst is an invagination of skin, which forms a sack beneath the skin’s surface. Because the lining of this sack consists of normal skin, it continues to shed skin cells just like our skin does. These dead skin cells become compressed and as more accumulate, the cyst grows into a lump. Occasionally, if the sack ruptures beneath the skin, an inflammatory reaction results that causes pain, redness, and drainage of a smelly fluid containing the dead skin cells. If this happens, you should see your dermatologist for treatment to decrease the risk of secondary infection and scarring. Although cysts are considered benign and can be watched without treatment, they can also be removed by simple surgery. During this outpatient procedure, the cyst is numbed with local anesthesia, and then its sack is carefully dissected out of the skin through a small incision. Once removed, the wound is stitched and a small scar results. Surgical excision is the most definitive way to remove cysts. If they are simply drained, they will refill over time. Fincher, Chipps and Lauren Moy) in Mohs Micrographic Surgery and in Cosmetic Surgery through an American Academy of Cosmetic Surgery Fellowship. Dr. Moy’s impressive credentials included past president and diplomate of the  American Academy of Dermatology, American Society of Dermatologic Surgeons, and American Board of Facial Cosmetic Surgery.  He was also a member of the  American College of Mohs Surgery, American Academy of Facial Plastic and Reconstructive Surgery,  and  American Board of Facial Cosmetic Surgery , and published more than 200 scholarly articles. Dr. Moy received the prestigious  Samuel J. Stegman, MD Award for Distinguished Service  for his remarkable contributions to the field of dermatologic surgery. He specialized in cosmetic and facial plastic surgery, face lifts, liposuction, laser skin resurfacing, eye lifts, and Mohs Micrographic Surgery. He had performed over 30,000 cases of Mohs Micrographic Surgery and facial plastic surgery over the past 25+ years of clinical practice. Dr. Moy’s medical background and accomplishments included completion of his dermatology residency training at UCLA. Dr. Moy then completed his Facial Cosmetic and Mohs Micrographic Surgery Fellowship training at the University of Pittsburgh Center for Health Services. He was a professor at the David Geffen School of Medicine at UCLA, was a former Co-Chief of the UCLA division of Dermatology, and a former Chief of Dermatologic Surgery. He served on the Editorial Board of the Archives of Facial Plastic Surgery,  was a former president of Los Angeles County Medical Association, Bay District and was the past Editor-in-Chief of the Dermatologic Surgery Journal. He served on the Editorial Board of Archives of Facial Plastic Surgery and Aesthetic Surgery.  Due to Dr. Moy’s love for teaching, he trained over 20 fellows in Mohs Micrographic and Cosmetic Surgery and mentored 250+ healthcare workers in dermatology.  Because of his generous spirit, he also volunteered his dermatologic services at the Venice Family Clinic for over 25 years. Pollogen is a radiofrequency device that reverses the signs of aging by tightening the skin. Radiofrequency energy heats the deep layers of the skin without damaging its surface. Dysport is similar to Botox, but it is manufactured by a different company. Like we have many brands of soda, Botox now has competing products as well. Dysport is FDA-approved and is used to relax muscles and reduce wrinkles. As we animate our faces, our muscles contract to form forehead creases; smile, scowl, and squint lines. Dysport works by disrupting signals between nerves and muscles, so that the muscles aren’t able to “flex.” This causes less muscle movement and prevents creases and wrinkles from becoming etched into the skin. Although Dysport is best for wrinkles that are formed when animating the face, it can be used to soften some deeper lines that are present at rest. Multiple Dysport treatments can improve these wrinkles over time, but lines may not completely disappear. This is like trying to iron-out pressed pant pleats. You can reduce the creases, but faint fabric folds may still show. Book Now Most often Dysport is used to target horizontal forehead lines, squint lines (crow’s feet), and scowl lines (“angry 11s”). With specialized technique, it can also be used to increase eyebrow arch, smooth “sniff” or “bunny” lines, raise a down-turned smile, correct a pebbly chin, elongate the face, minimize a masculine jaw, soften vertical neck bands, and control excessive sweating on the palms, soles, & under the arms. Dysport takes 2-7 days to reduce muscle movement, and it typically lasts between 3-4 months. Muscle movement gradually returns as its effects wane. Repeat treatments every 3-4 months help soften lines most effectively. At Moy-Fincher-Chipps, our goal is to maximize your comfort during treatment sessions. Because we use very fine needles, most patients experience minimal discomfort. Topical numbing cream and ice packs are available if desired. Liposuction is a surgery to remove excess fat from under the skin by suction. First, what is fat? Fat is tissue (otherwise known as adipose tissue) made up of cells that store energy and insulate the body. Fat is generally subcutaneous — located underneath the skin. Where fat is deposited around the body depends on a person’s gender, age and genetics. In men, fat tends to congregate in the chest, abdomen and buttocks. Recovering from Liposuction After your liposuction procedure, you will return to the comfort of your home wearing compression garments. Bruising and swelling may occur initially and these begin to fade after approximately one week. Most patients are able to return to work in 5-7 days. If a facelift or neck lift is performed alone, the skin often doesn’t match the newer lifted appearance, making it look “done.” By rejuvenating the skin at the same time as surgery, the final result is a much more natural. The laser also beautifully softens and eliminates vertical lines around the mouth and tightens eyelid skin, which no surgical or injectable filler procedure can accomplish well. How Does it Work? In the 1990s, fully ablative CO2 laser resurfacing was the only technology available, and because this laser removed the entire skin’s surface at once, healing was prolonged and there was a higher chance of scarring or permanent pigmentary changes after treatment. More recently, fully ablative devices have been replaced with fractionated devices that offer both a faster recovery and fewer side effects. The fractionated CO2 laser works by focusing microscopic beams of light into the skin in a grid pattern. Because the laser targets water, a major component of the skin, it immediately vaporizes tissue when fired. Vaporized tissue removes brown spots and sun damage, and the resulting micro-wounds created by the laser stimulate a wound healing process that leads to collagen synthesis. Ultimately, this smoothens lines and tightens skin. Because there are small areas of “normal” skin between the micro wounds, healing is accelerated. Typically, pink new skin is visible 5-10 days after the procedure. Who is a Good Candidate? Healthy patients with realistic expectations who are seeking total skin rejuvenation or treatment of scars from surgery or acne are excellent candidates for fractionated CO2 laser resurfacing. Patients with active inflammatory skin conditions may require treatment of these conditions before lasering, and those with very dark skin are not good candidates because of a risk of changing the skin’s native color. Your Consultation? During your evaluation, your board-certified dermatologist will review your medical history and medications as well as evaluate your skin and discuss your expectations. Certain medical conditions and medications can increase procedure risks and recovery time. If you smoke, it is best to stop or reduce your smoking before treatment as smoking can slow the recovery process. We may also recommend prophylactic anti-viral medication to prevent any cold sore outbreaks after your procedure. Pretreatment Your dermatologist will define the problem areas and the treatment pattern to be used and take baseline pre-procedure photographs. Typically, we recommend wearing comfortable clothing and having a family member or friend with you to drive you home after the procedure.  Before treatment you will receive an entire briefing of what to expect, potential risks and side effects, and how to care for yourself after the procedure. The Procedure A combination of topical and injected local anesthetic will be used to maximize your comfort before the procedure. We use many specialized techniques to minimize discomfort during the numbing process. If you are anxious, you may be offered a medication to help decrease anxiety, but you will be awake during your treatment. General anesthesia is unnecessary and puts you at increased risks. Once numb and comfortable, your dermatologist will use a combination of treatment settings specifically tailored to your skin type and degree of rejuvenation desired. Following treatment, an emollient is applied and post-care instructions are re-reviewed. The whole procedure from photos to post-treatment typically takes 1.5-2 hours. Post-treatment and recovery You will receive detailed instructions for postoperative care. Following these instructions is essential for maximizing your comfort, for achieving best results, and to avoid complications. After your CO2 laser resurfacing treatment, you will look like you have a bad sunburn and your skin will be warm. Initially, swelling is usually most significant around the eyes. It is normal for the skin to be moist and even contain some crusting. Over the course of 3-5 days, the old damaged skin will begin to come off and light flaking will continue for up to 10 days. Scarring can be an unavoidable consequence of healing from an injury, burn, wound, acne or surgery. When the skin is wounded, a complex series of events takes place which aims to prevent infection, preserve function, and restore the skin’s contour to its original form. Depending on how deep a skin injury is, the wound may heal beautifully with no apparent scarring, or it may leave a raised or depressed red, white, or pink line. Although the body attempts to recapitulate normal skin, the healing process is imperfect and scarring is a mark of the body’s best attempt at restoring normality. Many internal and external factors affect the likelihood of scarring. Poor nutrition, inadequate wound care, and smoking can all negatively impact the healing process and worsen scarring. Alternatively, excellent wound care, thoughtful nutrition, and seeking medical care early can optimize your chance of minimal scarring. Book Now Facial scaring is one of the most difficult challenges for a cosmetic surgeon. Our reconstructive and cosmetic surgeons must consider not only your appearance, but also the form and function of the tissue. Scars near the eyes can pull at the eyelids, creating chronic tearing and dry eyes that can eventually cause permanent damage or even blindness. Scars near the nose and mouth can affect breathing and eating. Facial scars are also quite visible and can cause significant personal distress and reduced self-esteem. Surgical scar revision can improve the appearance of scars, and in some cases, relocate a scar to a less obvious location. Surgical scar revision can also reduce tension and improve the function of joints, if scarring has caused limited mobility. We often combine surgical techniques with laser treatments to further refine and reduce the appearance of scars. In fact, our office routinely uses  fractionated erbium  or  CO2 lasers  at the time of surgical scar revision to optimize results. Our goal is to minimize the appearance of your scar to make you more comfortable with your appearance and improve your outlook. A variety of surgical techniques are available for scar revision. Often a combination of techniques will provide the best cosmetic outcome. Considerations in surgical scar revision: The type of scar, its depth and location determine the appropriate treatment. Age, sex and your genetic and ethnic makeup are important considerations. Facial scars can affect esthetic appearance and facial expressions, and it is crucial to restore and preserve facial symmetry when revising scars in this area. Acne scars are common and have many subtypes. Some are best treated with surgical intervention, but others may be better targeted by lasers, dermabrasion , injectable fillers or microneedling . Burn scars that cover large areas of the skin tend to pucker when they heal. This is called contracture because the skin contracts during healing. Contracted scars over joints are particularly problematic because they can reduce normal range of motion. Often, contracted scars require a series of laser treatments in addition to possible surgical release of excess tension for optimal results. Removal of skin cancer with MOHS surgery will produce cosmetically acceptable scars when performed by board-certified, fellowship trained reconstructive surgeons like those at Moy-Fincher-Chipps. But, facial scars from older treatments or older repairs may require revision to achieve cosmetically acceptable solutions. The timing of surgical scar revision is important and depends on the scar’s location and age of the scar, as well as patient’s age and skin type. Scars that are pink or brown in color may require laser treatments to address and correct these color problems. Scars that are thick and ropey or painful may require steroid injections for flattening. Surgical options include Z-plasties and other skin flaps to reorient scar lines, skin grafts, direct excision, dermabrasion, and laser surgery. Your options will be discussed with you at your consultation. But you should be aware that at times, in order to achieve optimal improvement, more than one procedure may be needed. When is scar revision surgery recommended? Scars tend to improve on their own as they mature. It takes about 12-18 months for a scar to mature. But, signs that a scar is worsening or not healing appropriately may require intervention before this time. We will discuss the best approach for your scars and give you realistic expectations on what can be achieved with our state-of-the-art technologies. Although scar revision surgery does not completely erase scars, it makes them much more cosmetically acceptable. When combined with dermabrasion or laser resurfacing, revised scars may become nearly imperceptible. Improving the appearance of your scars can change the way to feel about yourself.
