Clear aligners are thin, removable plastic mouthpieces that secure closely to your teeth and gently move them into the correct position. Typical treatment time is between 10 and 24 months — but if you’re an adult who had braces as a child and your teeth have only shifted slightly over the years, you may need aligners for as little as a few months. Advantages of Clear Aligners vs. Traditional Braces With traditional braces, patients live with a mouthful of metal 24 hours a day, often for years. They need to see an orthodontist regularly to adjust the braces as teeth move. Brushing and flossing can also be a challenge with metal braces. There are many reasons to love clear aligners. They are extremely inconspicuous, having almost no noticeable impact on your appearance. They can also be removed — so it’s easier to eat or drink, and you don’t have to deprive yourself of any foods that may be restricted by braces. And unlike braces, clear aligners can easily be taken out to maintain good oral hygiene. Patients with clear aligners require fewer visits to a dental office than those with braces. Depending on the length of treatment, patients may have to return simply to have new impressions or scans taken of their teeth to create additional sets of aligners once the initial phase of treatment is completed. While it’s recommended that clear aligners be worn for up to 22 hours per day, most patients find them to be very comfortable, as they don’t have any sharp metal components. And clear aligners are easily maintained with gentle brushing with a toothbrush and water. Are You a Candidate for Clear Aligner Treatment? Clear aligners are typically used for patients with mild to moderate teeth crowding or minor alignment or spacing issues. Patients who have severe crowding or spacing problems, or severe underbites, overbites, or crossbites, may require more complex treatment. Clear aligners are best for adults or teenagers. Straightening a child’s teeth is more complicated — children, and their mouths, are constantly developing, and younger kids may struggle with having attachments on teeth and remaining compliant with maintaining the condition of aligners. However, little can be done to adjust their fit; they’re bulky, they make breathing and talking difficult, and they provide little or no protection. Dentists do not recommend their use. Boil and bite mouth protectors. These can also be bought at many sporting goods stores and may fit your teeth better than stock mouth protectors. A boil and bite mouthguard is made from thermoplastic material. It is placed in hot water to soften, then placed in the mouth and shaped around the teeth using finger, bite, and tongue pressure. This type of mouthguard doesn’t offer as precise a fit as a custom-fitted mouthguard and can cause irritation of soft tissues. It can also fall out during use. Custom-fitted dental mouthguards. A mouthguard individually designed and made in a dental office or a professional laboratory can offer a better fit and protection than any other type. Custom mouthguards are designed specifically for your teeth. Dental guards can be made to vary in thickness for a better fit for teeth and gums and to alleviate jaw strain. Store-bought mouthguards typically only cover upper teeth — your dentist can make a mouthguard that protects bottom teeth as well. Added protection. Depending on the sport you play or the severity of your bruxism, custom dental guards, can be made thicker to provide more protection. Increased comfort. Can I Wear a Mouthguard if I Have Braces? Yes. Your dentist can design a mouthguard that goes over braces, protecting both the braces and your teeth. Caring For Your Mouthguard Caring for your mouthguard is easy to do: Rinse your mouthguard with cold water or with a mouth rinse before and after each use, or clean it with mild soap and a toothbrush. Place the mouthguard in a firm, perforated container to store or transport it. This permits air circulation and helps to prevent damage. If the mouth guard is acrylic, keep it in fresh, clean water. Protect the mouth guard from high temperatures — such as hot water, hot surfaces, or direct sunlight — to minimize distorting its shape. Occasionally check the mouthguard for general wear. Replace your mouthguard if you find holes or tears in it, or if it becomes loose or causes discomfort. Extraction by a Dentist or Oral Surgeon is Necessary for Many Patients Wisdom tooth extraction is a common surgical procedure to remove one or more wisdom teeth — the four permanent adult teeth located in the back corners at the top and bottom of your mouth. Sometimes these teeth can be a valuable asset to the mouth when healthy and properly aligned — but wisdom tooth extraction is required if a tooth doesn’t have room to grow (impacted wisdom tooth), resulting in pain, infection, or other dental issues. To prevent potential future problems, some dentists and oral surgeons recommend that they remove your wisdom teeth even if impacted teeth aren’t currently causing issues. Most patie nts will undergo oral and maxillofacial surgery to have some or all of their wisdom teeth removed. Learn More About Wisdom Teeth Removal What Are Wisdom Teeth? Wisdom teeth, or third molars, are the last permanent teeth to appear (erupt) in the mouth. These teeth usually appear between the ages of 17 and 25. Some people never develop wisdom teeth. For others, wisdom teeth erupt normally — just as their other molars did — and cause no problems. Many develop impacted wisdom teeth that don’t have enough room to erupt into the mouth or develop normally. Impacted wisdom teeth may erupt only partially or not at all. How Do You Know If Your Wisdom Teeth Need to Be Removed? When wisdom teeth are misaligned, they may position themselves horizontally, be angled toward or away from the second molars, or be angled inward or outward in the mouth. Poor alignment of wisdom teeth can crowd or damage adjacent teeth, the jawbone, or nerves. Wisdom teeth may remain enclosed within the soft tissue and/or the jawbone, or they may only partially erupt through the gum tissue. Partial eruption of the wisdom teeth creates an opening for bacteria to enter the tooth and cause an infection, which results in pain, swelling, jaw stiffness, and general illness. Partially erupted teeth are also more prone to tooth decay and gum disease because their hard-to-reach location and awkward positioning make brushing and flossing difficult. You’ll likely need your impacted wisdom tooth pulled if you’re experiencing the following: Pain Trapped food and debris behind the wisdom tooth Infection or gum disease (periodontal disease) Tooth decay in a partially erupted wisdom tooth Damage to a nearby tooth or surrounding bone Development of a fluid-filled sac (cyst) around the wisdom tooth Complications with orthodontic treatments to straighten other teeth Preventing Future Problems with Wisdom Teeth It’s difficult for dentists to predict future problems with impacted wisdom teeth. However, for many, the rationale for preventive extraction includes the following: Symptom-free wisdom teeth could still harbor disease. If there isn’t enough space for the tooth to erupt, it’s often hard to clean properly. Serious complications with wisdom teeth extraction occur less often in younger adults. Older adults may experience difficulty with wisdom tooth surgery and complications after surgery. What To Expect From Wisdom Teeth Extraction During the procedure, your dentist or oral surgeon may use one of three types of anesthesia, depending on the expected complexity of the wisdom tooth extraction and your comfort level. Options include: Local anesthesia. With a local anesthetic, you’ll experience the same numbing near the extraction site as you would during other common dental procedures. During teeth removal, although you’ll feel some pressure and movement, you shouldn’t feel any pain. Sedation anesthesia. Administered through an intravenous (IV) line in your arm, sedation anesthesia suppresses your consciousness during the procedure. You don’t experience pain and will have limited memory of the procedure. You’ll also receive local anesthesia to numb your gums. General anesthesia. In special situations, you may be offered general anesthesia. When administered, you lose consciousness. You’ll experience no pain and will have no memory of the procedure. Local anesthesia is also given to help with postoperative discomfort. During a wisdom tooth extraction, your dentist or oral surgeon: Makes an incision in the gum tissue covering the tooth Removes bone that blocks access to the tooth root Divides the tooth into sections if it’s easier to remove it in pieces Removes the tooth Cleans the extraction site of any tooth or bone debris Stitches the wound closed to promote healing (if necessary) Places gauze over the extraction sites to control bleeding and to help blood clots form Potential Complications of Wisdom Teeth Removal Potential complications after having your wisdom teeth removed include: Dry socket. This occurs when either a blood clot has failed to form in the extracted tooth socket or else the blood clot that did form has been dislodged. Your dentist will treat the dry socket by placing medication in the socket. Paresthesia. This is a rarer complication, where nerves close to the extraction site are bruised or damaged during tooth removal. Sedation can be used for everything from invasive procedures to simple teeth cleaning. It’s sometimes referred to as “sleep dentistry,” although that’s not entirely accurate: patients are usually awake, except those under general anesthesia. There are different levels of sedation offered, depending on a patient’s anxiety level and the complexity and duration of their planned procedure: Minimal sedation — The patient is awake but relaxed. Moderate sedation — Formerly called “conscious sedation.” Patients may slur their words when speaking and may not remember much of the procedure. Deep sedation — Patients are on the edge of consciousness but can still be awakened. General anesthesia — Patients are completely unconscious. For minimal sedation, patients take a pill, typically about an hour before a procedure. The pill will make you drowsy, although you’ll still be awake. A larger dose may be given to produce moderate sedation. Some patients become groggy enough from moderate oral sedation to fall asleep during the procedure, though they can be awakened with a gentle shake. IV sedation. Patients receive the sedative drug through a vein, so it goes to work more quickly. This method allows the dentist to continually adjust the level of sedation. Deep sedation and general anesthesia. Patients receive medications that make them either almost unconscious or totally unconscious during the procedure. With general anesthesia, you cannot be awakened until the effects of the anesthesia wear off or are reversed with medication. Regardless of which type of sedation patients receive, they’ll also typically require a local anesthetic — a numbing medication at the site in the mouth where the dentist is working — to relieve any pain associated with the procedure. Who Can Receive Dental Sedation? Sedation may be a good choice for patients who have the following: Had traumatic dental experiences A strong fear of dental work Difficulty being numbed A low pain threshold A bad gag reflex Very sensitive teeth Complicated dental needs, such as root canals or a dental implant A large amount of dental work to be completed A dislike of the sounds and smells associated with dental care Anxiety about needles or injections A hard time sitting still in the dentist’s chair Children are sometimes given sedation if they are terrified of going to the dentist or refuse to cooperate during the visit. Nitrous oxide tends to be safe in children, and almost every dentist can administer it. A smaller percentage of pediatric dentists are trained to give oral sedation. Oral sedation can be safe when kept within the recommended dosage for a child’s age and weight. Who Can Perform Sedation? Most dentists can administer minimal sedation, and an increasing number are becoming qualified to give moderate sedation. However, only a small percentage of dentists who have completed the Commission on Dental Accreditation (CODA) program in deep sedation and general anesthesia can use those more complex techniques. These dentists often specialize in oral surgery. Some dentists use a dentist anesthesiologist, who is specially trained in giving all levels of sedation and anesthesia and monitoring your vital signs, including your blood pressure, during your dental procedure. How Safe is Sedation Dentistry? While there is always a risk in receiving anesthesia, it’s usually safe when given by experienced dentists. However, people who are obese or who have obstructive sleep apnea should talk to their doctor before having sedation, as they’re more likely to develop complications from the anesthesia. Patients should ensure that their dentist is trained and certified to administer the type of sedation they’ll be receiving. Before the procedure, your dentist should review your medical history, including any relevant health problems, discuss any medications you’re currently taking, outline the risks of sedation, and answer any questions you may have. Tooth Removal Can Be Stress-Free Sometimes a tooth is too damaged to function properly or too decayed to remain in the mouth. Several other factors can also require tooth removal. While the thought of such a procedure can cause many patients distress, your dentist or oral surgeon can perform a painless tooth extraction with the latest dental technology and present you with treatment options to permanently replace the extracted tooth. Learn More About Tooth Extractions What is a Tooth Extraction? In a tooth extraction, your dentist removes a tooth and its root from the mouth. The tooth removal process can be accomplished in a single dental appointment, aided by either numbing anesthetic or sedation. Patients typically recover within a few days, and the space created by the pulled tooth can be filled with a dental implant , a bridge , or a denture . When is an Extraction Necessary? Although permanent teeth were meant to last a lifetime, there are a number of reasons why tooth extraction may be needed. Along with a badly decayed or damaged tooth, other reasons include: A crowded mouth. Sometimes dentists pull teeth to prepare the mouth for orthodontia or clear aligner therapy. The goal of orthodontia is to properly align the teeth, which may not be possible if your teeth are too big for your mouth. An impacted tooth. If a tooth cannot break through the gum (erupt) because there isn’t room in the mouth for it, your dentist may recommend pulling it. This is common with wisdom teeth. Infection. If tooth decay or damage extends to the pulp — the center of the tooth containing nerves and blood vessels — bacteria in the mouth can enter the pulp, leading to infection . Often this can be corrected with root canal therapy (RCT) , but if the infection is so severe that antibiotics or RCT do not cure it, a surgical extraction may be needed to prevent the spread of the infection. Risk of infection. If your immune system is compromised — for example, if you are receiving chemotherapy or have an organ transplant — even the risk of infection in a tooth may be enough to pull it. Periodontal (gum) disease. If p eriodontal disease — an infection of the gum tissue and bones surrounding and supporting the teeth — has caused a loose tooth, it may be necessary to remove it. The Extraction Process Most extractions are uncomplicated. Typically, your general dentist will review your x-rays to ensure the right tooth, root, socket, and bone conditions are present for a simple extraction. If the tooth is impacted, your dentist must cut open gum tissue to access the tooth. Sometimes, a hard-to-pull tooth must be removed in pieces. A local anesthetic will be used to numb the area; for more complex extractions, your dentist may choose to use sedation to make the process easier for you. Your dentist will apply slight pressure with a specially designed instrument to gently and gradually ease the tooth from its socket. Once the tooth has been removed, a blood clot usually forms in the socket. Your dentist will pack a gauze pad into the socket and have you bite down on it to help stop any bleeding. If required, your dentist will place a few stitches — usually self-dissolving — to close the gums over the extraction site. Occasionally, the blood clot in the socket breaks loose, exposing the bone in the socket. This is a painful condition called dry sockets. If this happens, your dentist will likely place a sedative dressing over the socket for a few days to protect it, as a new clot may form. Tips for Recovering from a Tooth Extraction The healing process after a tooth extraction is usually straightforward, but it’s important that you follow your dentist’s instructions about caring for the area to avoid pain, swelling, and infection and to prevent dry sockets. In addition, the following can help minimize discomfort and speed recovery: Take painkillers as prescribed. Bite firmly but gently on the gauze pad placed by your dentist to reduce bleeding and allow a clot to form in the tooth socket. Change gauze pads before they become soaked with blood. Otherwise, leave the pad in place for three to four hours after the extraction. Apply an ice bag to the affected area immediately after the procedure to reduce swelling. Apply ice for 10 minutes at a time. Relax for at least 24 hours after the extraction. Limit activity for the next day or two. Avoid rinsing or spitting forcefully for 24 hours after the extraction to avoid dislodging the clot that forms in the socket. After 24 hours, rinse your mouth with a solution made of 1/2 teaspoon of salt and eight ounces of warm water. Do not drink from a straw for the first 24 hours. Do not smoke, which can inhibit healing. Eat soft foods, such as soup, pudding, yogurt, or applesauce, the day after the extraction. Then, gradually add solid foods to your diet as the extraction site heals. When lying down, prop your head with pillows. Lying flat may prolong bleeding. Continue to brush and floss your teeth, and brush your tongue to help prevent infection. Avoid brushing and flossing the extraction site. Recovery typically takes a few days, but wisdom teeth and more complex extractions may take up to a couple of weeks. When your dentist has determined that the extraction site has sufficiently healed, they can proceed with your preferred treatment option for replacing the missing tooth. Almost all cancers in the oral cavity and oropharynx start in squamous cells, which are flat, thin cells that form the mouth and throat lining. These cancers are called squamous cell carcinomas. Other types of the oral cavity and oropharyngeal cancer can occur in the minor salivary glands in the lining of the mouth and throat, and in the lymphoid tissue in the tonsils and the base of the tongue, where cancers called lymphomas can start. Oral cavity and oropharyngeal cancers occur most often in the tongue, tonsils, oropharynx, gums, and floor of the mouth. They are less often found in the lips, the minor salivary glands, cheeks, sinuses, and hard and soft palate. During your consultation, your dentist will fully explain the procedure to you and answer any questions you may have. Your dentist will also provide guidance on the type of outcome that can be expected once the procedure has been completed. A second appointment will then be scheduled for the actual procedure. During the procedure, remnants of lost teeth will be extracted, and an incision will be made in the gums to expose the jawbone. If ridge reduction is required to ensure a perfect fit for the implants, the ridge will be reduced by removing predetermined measurements of bone. The remaining bone will be smoothed and prepared for the insertion of the dental implants. Pilot holes for the implants will be drilled at angles to obtain the greatest possible strength and stability while avoiding contact with nerves or sinus cavities. Once the implants are inserted, the dental surgeon will close all incisions in the gums. A temporary prosthesis will be attached to the anchor points to protect them while the patient’s gums and jawbone heal. Patients will be prescribed pain relievers as needed and will be provided with information on maintaining proper oral health to accelerate healing. Your dentist may schedule follow-up appointments to monitor progress. It is recommended that patients follow a soft-food diet for the first few months after the procedure to allow the implants to bond with the jawbone. The bonding process is usually complete after 3-4 months, at which time the patient can introduce firmer foods into their diet. The patient can then return to their dentist for another set of impressions, which will be used to craft the final fixed bridge prosthesis. The dentist will remove the temporary prosthesis and take molds of the gums and implants so that the final prosthesis will fit perfectly. After the impressions have been taken, the temporary prosthesis will be replaced until the final version has been crafted. What Types of Dental Cleanings Are There? There are four types of dental cleanings: Prophylaxis Dental Cleaning. The word prophylaxis means “to prevent disease.” Therefore, prophylaxis dental cleanings are used to clean healthy teeth in order to prevent them from becoming affected by tooth decay or gum disease. Prophylaxis cleanings remove plaque and tartar from teeth using a dental scaler or water stream. Most dental patients only require a prophylaxis cleaning, especially if they have kept up with daily dental hygiene and semi-annual dental appointments. Scaling and Root Planing Cleaning. Scaling and root planing cleanings, also known as deep cleanings, are used for both teeth and gum pockets in order to treat gingivitis (inflamed gums) and periodontitis (gum disease). Scaling is the process that removes plaque and tartar from the tooth’s surface and in the gum pockets below the gum line. This removes bacteria and helps to reduce gum inflammation. Root planing is the process of smoothing the tooth roots, which helps the gums reattach to teeth and eliminate gum pockets. Periodontal Maintenance Cleaning. Whereas prophylaxis dental cleaning is done as preventative care, periodontal maintenance takes care of existing problems with your oral health. They, therefore, need to be scheduled more frequently. Periodontal maintenance cleanings utilize scaling and root planing to remove plaque and tartar from teeth and gum pockets and to smooth out the tooth roots in order to treat existing gum disease. Gross Debridement Cleaning. Gross debridement cleanings are the most heavy-duty of teeth cleanings, designed to clean teeth covered with copious amounts of plaque and tartar. Patients who require gross debridement generally struggle to maintain their oral hygiene and often haven’t visited their dentist for a significant amount of time. Since tartar is especially hard, and an electrical tool is used to loosen tartar during gross debridement cleanings. Once the excess plaque and tartar have been removed, a prophylaxis cleaning is performed to further clean the teeth. What Are the Advantages and Disadvantages of Professional Teeth Cleaning? There is virtually no downside to having your teeth professionally cleaned. Cleanings are as minimally invasive as dental treatments can be, requiring no numbing of the mouth and little time in the dentist’s chair. Regular (prophylaxis) teeth cleanings typically take 30-60 minutes to perform. Some patients may dislike the feel or taste of the gritty toothpaste used for the deep brushing phase or the taste of the fluoride treatment, but these phases each last just a few minutes. There are significant advantages to having your teeth professionally cleaned on a regular basis: Plaque and tartar removal. Brushing and flossing at home can’t remove tartar. Preventing tooth decay. Tartar causes cavities and tooth decay, which require more invasive (and expensive) dentistry to repair. Preventing gum disease . Professional cleanings keep gums healthy, preventing infections which would also require more advanced dental treatments. What Happens During a Teeth Cleaning? Professional teeth cleanings typically follow these steps: 1. Physical Exam. Most teeth cleanings are performed by a dental hygienist. Before the cleaning process begins, they start with a dental exam, using a small mirror to check around your teeth and gums for any potential concerns. 2. Removal of plaque and tartar. The dental hygienist uses a scaler to get rid of plaque and tartar around your gum line, as well as between your teeth. The more tartar there is on your teeth, the more time your hygienist will need to scrape a particular spot. 3. Gritty toothpaste cleaning. The hygienist then brushes your teeth with a high-powered electric brush and gritty toothpaste. This is a great way to give your teeth a deep cleaning and remove any tartar left behind from the scaler. 4. Expert flossing. Whether you floss regularly at home or not, nothing beats an expert flossing session. Your dental hygienist can get deep between your teeth and locate any potential trouble spots where you might bleed at the gums. 5. Rinsing. Next, you’ll rinse out your mouth to get rid of any debris. Your dental hygienist may give you a rinse that contains liquid fluoride. 6. Applying fluoride treatment. The last step of the cleaning process is a fluoride treatment. This treatment is used as a protectant for your teeth to help fight against cavities for several months. Your dental hygienist will place a foamy gel or a sticky paste containing fluoride on your teeth for one minute. After removing the gel or paste, your dentist will inform you as to when you can next eat or drink (typically within 30 minutes.) 7. Other potential steps. X-rays are normally done once a year.
