All-On-Four dental implants are more stable and secure than standard dentures. The All-On-Four dental implant is an option for patients who need numerous teeth replaced, or even a whole upper or lower set of teeth. The procedure is a relatively new technique in dentistry and allows patients to receive an entire arch of prosthetic teeth in a single appointment with a surgeon. By using just 4 dental implants instead of one implant per tooth, the procedure is minimally invasive and can replace an entire set of teeth without the need for repeat surgical visits. About All-On-Four Dental Implants The All-On-Four treatment is used to replace the entire upper or lower set of teeth. The procedure creates a permanent prosthesis by using 4 dental implants which act as an anchor for a bridge or overdenture of 12 to 14 teeth. This technique avoids having to have extensive bone grafting procedure. All-On-Four dental implants can replace both damaged and missing teeth and provide a long-term, permanent alternative to dentures. About the All-On-Four Procedure Four screws are placed into the jawbone and gums and left to heal sufficiently, before the implant bridge can be secured in place. Once implanted, the bridge will never need to be removed and will resemble the appearance of your natural teeth.As with all dental implants, All-On-Four dental implants are placed during an outpatient surgical procedure in Santa Monica Center for Oral Surgery & Dental Implants. If surgical packing was placed, leave it alone. The packing helps to keep the tooth exposed. If it gets dislodged or falls out, do not get alarmed but please contact our office for instructions. Bleeding Some bleeding or redness in the saliva is normal for 24 hours. Excessive bleeding (your mouth fills rapidly with blood) can be controlled by biting on a gauze pad placed directly on the bleeding wound for 30 minutes. If bleeding continues profusely, please call for further instructions. Swelling Swelling is a normal occurrence after surgery. To minimize swelling apply an ice bag, or a plastic bag or towel filled with ice, on the cheek in the area of surgery. Apply the ice continuously, as much as possible, for the first 36 hours. Diet Drink plenty of fluids. Avoid hot liquids or food. Soft food and liquids should be eaten on the day of surgery. Return to a normal diet as soon as possible unless otherwise directed. Pain You should begin taking pain medication as soon as you feel the local anesthetic wearing off. For moderate pain, one or two tablets of Tylenol or Extra Strength Tylenol may be taken every 3-4 hours. Ibuprofen (Advil or Motrin) may be taken instead of Tylenol. Ibuprofen bought over the counter comes in 200 mg tablets: 2-3 tablets may be taken four times daily, not to exceed 3200mg daily for an adult. Consult our practice for individuals under 18. Do not take the two medications at the same time.For severe pain, the prescribed medication should be taken as directed. Do not take any of the above medication if you are allergic to them, or have been instructed by your doctor not to take it. Do not drive an automobile or work around machinery. Avoid alcoholic beverages. Pain or discomfort following surgery should subside more and more every day. If pain persists, it may require attention and you should call the office. Oral Hygiene Oral cleanliness is essential to good healing. Clean your mouth thoroughly after each meal, beginning the day after surgery. Brush your teeth normally if possible. Rinse with warm salt water (1 teaspoon of salt in a cup of warm water) six times a day. Continue this procedure until healing is complete. REMEMBER:   A clean wound heals better and faster. Activity Keep physical activities to a minimum immediately following surgery. If you exercise, throbbing or bleeding may occur. If this occurs, you should discontinue exercising. Sedation dentistry (also known as “Sleep Dentistry”) uses medication to help patients relax or even sleep during dental procedures for painless dental care. Sedation dentistry enables patients who may have a low pain threshold, very sensitive teeth, a gag reflex, or moderate-to-severe anxiety the ability to undergo more advanced procedures for teeth and gum health. At Santa Monica Center for Oral Surgery & Dental Implants, we administer an IV sedation that puts you to sleep safely and comfortably, and provides for the ultimate painless dental care. About Sedation Dentistry What is Sedation Dentistry or “Sleep Dentistry”? Sedation dentistry is used to help patients relax while they are having treatment. Medication will be used but you usually remain awake, unless you opt for a general anesthetic which will make you sleep. Santa Monica Center for Oral Surgery & Dental Implants will discuss the sedation options with you so that you can choose the one you prefer and which he feels is best suited to your circumstances: A low level of sedation – you will feel completely relaxed but will not be asleep. Moderate sedation – you will feel more relaxed, even drowsy and although not fully asleep, you may not remember much afterwards. Deep sedation – you will wake up easily but will be more or less unconscious. General anesthesia – you will be fully asleep/unconscious and will not be aware of anything. Santa Monica Center for Oral Surgery & Dental Implants will choose from a choice of sedation methods: Minimal inhaled sedation – you will be asked to breathe ‘laughing gas’ (nitrous oxide) mixed with oxygen via a mask. You will feel relaxed throughout the procedure and as this is not a heavy sedative, you can drive afterwards. Oral sedation – your dentist can decide on how much of this to use; for a minimal effect, you will just take an Anti-anxiety pill. Like taking Valium, you will be drowsy and relaxed. Larger doses will make you more sedated and you may even fall asleep during treatment. Moderate IV sedation – this is administered through a vein and your surgeon will decide on how much sedation you need. Heavy sedation/general anesthesia– with this method, you are generally unconscious so will not be aware of the procedure at all. As well as being sedated, your dentist will still apply a local anaesthetic into your gum so that you will not feel any pain whilst he works on your teeth. Sinus Lift - Santa Monica Center for Oral Surgery Book Appointment Sinus Lift Sinus Lift Sinus Lift What is a Sinus Lift? The maxillary sinuses are behind your cheeks and on top of the upper teeth. These sinuses are empty, air-filled spaces. Some of the roots of the natural upper teeth extend up into the maxillary sinuses. When these upper teeth are removed there is often just a thin wall of bone separating the maxillary sinus and the mouth. Dental implants need bone to hold them in place. When the sinus wall is very thin, it is impossible to place dental implants in this bone. The key to a successful and long-lasting dental implant is the quality and quantity of jaw bone to which the implant will be attached. If bone loss has occurred due to injury or periodontal disease, a sinus augmentation can raise the sinus floor and allow for new bone formation. A sinus lift is one of the most common bone grafting procedures for patients with bone loss in the upper jaw. The procedure seeks to grow bone in the floor of the maxillary sinus above the bony ridge of the gum line that anchors the teeth in the upper jaw. This enables dental implants to be placed and secured in the new bone growth. Am I A Candidate For A Sinus Lift Procedure? A sinus lift may be necessary if you: are missing more than one tooth in the back of your jaw are missing a significant amount of bone in the back of your jaw are missing teeth due to a birth defect or condition are missing most of the maxillary teeth and require support for dental implants How is this Procedure Accomplished? Most commonly, a small incision is made on the premolar or molar region to expose the jaw bone. A small opening is cut into the bone, and the membrane lining the sinus is pushed upward. The underlying space is filled with bone grafting material, either from your own body or from a other sources. Sometimes, synthetic materials that imitate bone formation are used. After the bone is implanted, the incision is sutured and the healing process begins. After several months of healing, the bone becomes part of the patient’s jaw and dental implants can be inserted and stabilized in the newly formed sinus bone. If enough bone between the upper jaw ridge and the bottom of the sinus is available to sufficiently stabilize the implant, sinus augmentations and implant placement can sometimes be performed as a single procedure. If not enough bone is available, the sinus augmentation will have to be performed first, then the graft will have to mature for up to several months, depending upon the type of graft material used. Once the graft has matured, the implants can be placed. The sinus graft makes it possible for many patients to have dental implants that previously had no other option besides wearing loose dentures. A sinus grafting (also known as sinus lift or sinus augmentation) technique is a delicate procedure requiring high level of surgical expertise. At Santa Monica Center for Oral Surgery and Dental Implants, this procedure is routinely performed under local anesthesia in less than 30 minutes. The science and art of treating these injuries requires special training involving a “hands on” experience and an understanding of how the treatment provided will influence the patient’s long term function and appearance. They are on staff at local hospitals and deliver emergency room coverage for facial injuries, which include the following conditions: Facial lacerations Intra oral lacerations Avulsed (knocked out) teeth Fractured facial bones (cheek, nose or eye socket) Fractured jaws (upper and lower jaw) About Facial Trauma Maxillofacial Trauma Facial trauma involves, bone fractures, soft tissue injury to the gums and skin or, damage caused to facial nerves, salivary glands or the eyes. Injury can be caused by falling, a road traffic incident, assault, or a sports accident.The treatment of facial trauma covers: Teeth that have been knocked out (Avulsed teeth) Fractured upper or lower jaw Fractures to eye socket, nose or cheek bones Facial lacerations Lacerations of the mouth (Intra-oral lacerations) The treatment of damaged teeth Avulsed Teeth – A tooth that has been knocked out can be saved if fast action is taken. Carefully put the tooth in some milk or saliva to keep the tooth moist. Water should only be used if nothing else is available. Don’t try and clean it, leave it exactly how it is and then get to the dentist as quickly as possible.If the tooth or bone aren’t damaged, your dentist will be able to put the tooth back into place and hold it in with a soft wire splint. The root should then re-attach itself to the bone within a month or so and then the splint can be removed. Fractured upper or lower jaw Fractures to the lower jaw (mandible) and the upper jaw (maxilla) may need orthognathic surgery and orthodontic treatment to enable you to open and close your mouth, chew properly and align your jaw correctly so that your normal bite is re-established.The dentist will either wire up your jaw to let the fractures heal or place the bones back in position and then use screws and plates to hold them in place until they are healed. This procedure is called ‘rigid fixation’ and has a faster recovery period than jaw wiring. Fractures to the eye socket, nose or cheek bones Fractures of the face will cause swelling and the sensation that teeth are not fitted together as they are normally. Opening and closing your mouth may also be difficult. If the fracture isn’t serious you may be advised to take a liquid diet for a specified amount of time in order to let the injury heal by itself. If a dental procedure is required, your jaw will either be wired to keep it shut, or as in the case of a fractured jaw, the bones will be placed surgically in position and then held together with screws and plates. Facial lacerations Injuries to the face not only require suturing to repair the damage and prevent infection, but your dentist can also check and treat damage to the salivary glands and ducts, as well as your facial nerves. Lacerations to the mouth (Intra-oral lacerations) Intra-oral lacerations may need suturing and require careful checking to ensure that there is no damage to teeth or any fractures to the face or jaw. Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally for your convenience, a surgery can be performed the same day as the consultation for healthy patients not on blood thinners. This is especially true for healthy patients scheduled to have wisdom teeth removal under general anesthesia. However, for those with a complex medical history, or those who are on blood thinners, a pre-surgical consultation will be required and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation: Your surgical referral slip (given to you by your dentist or physician). Any recent x-rays if applicable – If your dentist or physician has taken x-rays you may request that they forward them to our office or you may pick them up and bring them to our office. If additional films are necessary they can be taken at our facility. Current list of medications If you have medical and/or dental insurance, please submit them with your online patient registration form or bring your insurance card(s) with you for us to scan. This will allow us to help you process any insurance claims in a timely manner. IMPORTANT: All patients under the age of 18 MUST be accompanied by a parent or legal guardian. Surgery Prep If you are anticipating local anesthesia or nitrous oxide: You may eat and drink as you normally would. No dietary restrictions. No smoking (including recreational drugs as well) for at least 24 hours before surgery. Ideally, cut down on or stop smoking as soon as possible prior to the day of your surgery. You may drive yourself to and from your appointment. If you have an illness such as a cold, sore throat, stomach or upset bowels, please notify the office. No smoking for at least 24 hours before surgery. Ideally, cut down or stop smoking as soon as possible prior to the day of surgery. A responsible adult must accompany the patient to the office, remain in the office during the procedure, and drive the patient home. The patient should not drive a vehicle or operate any machinery for 24 hours following anesthesia. If bleeding occurs, place a gauze pad directly over the bleeding socket and apply biting pressure for 30 minutes. If bleeding continues, bite on a moistened black tea bag for thirty minutes. The tannic acid in the black tea helps to form a clot by contracting blood vessels. If bleeding occurs, avoid hot liquids, exercise, and elevate the head. If bleeding persists, call our office immediately. Do not remove the immediate denture unless the bleeding is severe. Expect some oozing around the side of the denture. Use ice packs (externally) on the cheek near the surgical site. Apply ice for the first 36 hours only. Apply ice continuously while you are awake. For mild discomfort use aspirin, Tylenol, or any similar medication; two tablets every 3-4 hours. Two to three tablets of Ibuprofen (Advil or Motrin) can be taken every 3-4 hours. For severe pain, use the prescription given to you. If the pain does not begin to subside after 2 days, or increases after 2 days, please call our office. If an antibiotic has been prescribed, make sure to finish your prescription unless you have an allergic reaction.. Drink plenty of fluids. If many teeth have been extracted, the blood lost at this time needs to be replaced. Drink at least six glasses of liquid the first day. Do not rinse your mouth for the first post-operative day, or while there is bleeding. After the first day, use a warm salt water rinse every 4 hours and following meals to flush out particles of food and debris that may lodge in the operated area. (One teaspoon of salt in one cup of warm water). After you have seen your dentist for denture adjustment, take out the denture and rinse 3 to 4 times a day. Restrict your diet to liquids and soft foods that are comfortable for you to eat. As the wounds heal, you will be able to resume your normal diet. The removal of many teeth at one time is quite different from the extraction of just one or two teeth. Because the bone must be shaped and smoothed prior to the insertion of a denture, the following conditions may occur, all of which are considered normal: The area operated on will swell, reaching a maximum in two days. Swelling and discoloration around the eyes may occur. The application of a moist warm towel will help eliminate the discoloration. The towel should be applied continuously for as long as is tolerable, beginning 36 hours  after  surgery. ( Remember:  ice packs are used for the first 36 hours only). A sore throat may develop. The muscles of the throat are near the extraction sites. Swelling into the throat muscles can cause pain. This is normal and should subside in 2-3 days. If the corners of the mouth are stretched, they may dry out and crack. Your lips should be kept moist with an ointment like Vaseline. There may be a slight elevation of temperature for 24-48 hours. If your temperature continues to rise, notify our office. If immediate dentures have been inserted, sore spots may develop. In most cases, your dentist will see you within 24-48 hours after surgery to make the necessary adjustments and relieve those sore spots.
