Well, it’s interesting because so many people claim to have it, but not all of them do. So let me elaborate on this. The most common description that patients give me when discussing sciatica is simply “I have leg pain”. Although sciatica typically does cause leg pain, the disorder itself is a bit more complicated than that. So let us first understand the anatomy of the lower back to get a better understanding of sciatica. The Anatomy Every human body is comprised of many bones, 206 to be exact. There are a series of bones that run the length of your body that make up your spine. The spine holds the body erect. It acts as a pathway for the spinal cord to run the distance between your brain and your tailbone via the spinal canal. The bones that make up the spine are called vertebra. The lower back (lumbar) is made up of 5 vertebrae and connects the rest of the spine to the tailbone (sacrum). Between each vertebra there is a piece of cartilage called a disc. The cartilage is elastic enough to allow the spine to bend and twist but tough enough to support the body’s weight. Each combination of vertebra, disc, vertebra creates a tunnel for nerves to pass through on either side of the spine called Foramen . These nerves are the culprit for sciatica pain. What is Sciatica? When you're looking at the anatomy of the lower back, focus on the segments between the L4 and S1 . Now let’s take a look at the nerves between each segment — the ones between the L4 and L5, and the L5 and S1. These particular nerves are most affected. When these nerves exit the back, for the most part, they bundle up to form a larger nerve called the sciatic nerve . So, when one of these nerves are compressed or ‘pinched’, then pain, numbness, tingling and/or weakness can travel down the leg. This is sciatica. Sciatica can affect either leg or both at the same time. But we need to determine the reason why the nerves are compressed in the first place. Let’s talk about that next. What is the Cause of Nerve Compression? There are a few common spine conditions that can compress a nerve. The most common are bulging and herniated discs . Healthy Sleep Habits: Limit screen time before bed. Blue light emitted from our electronic device screens affects melatonin production, which adversely affects our sleep. Eat your last meal 3–4 hours before bedtime. This allows for proper digestion before bed. Set and follow a normal sleeping routine. Wake up and go to sleep at consistent times, including on weekends. Sleep in a cool room. Ideally 60–68°F, or utilize a temperature-controlled mattress or mattress pad. Brushing your teeth hurts. Pulling up your pants hurt. Reaching for the remote hurts. Everything you do makes it hurt! Then come to find out you have tennis elbow, but wait, you don’t even play tennis! Recently an unusual number of patients coming into the office have complained of a pain to the outside of their elbows. The area is often slightly inflamed. It is warm, red and tender to the touch and it is generally painful with movement. So much so, that sometimes they have described it as a sharp stabbing pain. Looks like classic symptoms of tennis elbow, but how did they get it? The culprit has been their mobile phone. My mom always said spending too much time on the phone is bad for you and in a way, she was right! A lot of us have been in quarantine, working from home and limited in our ability to do outdoor activities or meet up with friends and family so we are keeping up with the outside world by extensively using our iPads, mobile phones and laptops. When you use your mobile phone, an iPad, or even read a book for excessive amounts of time, the muscles to the outside of the forearm can become very tight, inflamed, and irritated just like an avid tennis player swinging a racket. More specifically, the two muscles on the outside of the forearm, extensor carpi radialis longus and extensor carpi radialis brevis. So how did it get its name Tennis Elbow? Pain to the outside of the elbow would often occur when avid tennis players would develop excessively tight and irritated muscles to the outside and back of their forearms. As these muscles became overworked from swinging their rackets, the muscles would start to pull away at their insertion site. This area is called the lateral epicondyle. When this area becomes inflamed, a person can develop a disorder called lateral epicondylitis or more commonly referred to as Tennis Elbow. In addition to the lateral epicondyle, the highly sensitive periosteum surrounding the epicondyle and the muscular tendons can also become irritated and very painful. What are the signs and symptoms of Tennis Elbow? The area can be warm, red and tender to the touch. The outside of the elbow can be dull and achy at rest, to sharp and stabbing with any kind of movement. External rotation of the wrist can increase the pain. In fact, no other surgery has a syndrome named for its failures—Failed Back Surgery Syndrome. Some are better trained at non-surgical alternatives, are more conservative and will recommend those options; while others believe their surgical outcomes are better than those published in the medical literature and will more quickly schedule surgery. Why Do Back Surgeries Have Such Inconsistent and Poor Outcomes? Most doctors try not to admit this, but they are rarely sure of the exact cause of your pain. They may believe it’s a pinched nerve, a herniated disc or spinal stenosis since they see it on an MRI; but, they know the real cause of your pain may not even be visible on your MRI. Doctors are greatly influenced by your MRI findings, even though most studies have shown them to be only about 50% accurate in determining the cause of your pain. In other words, MRIs show abnormal tissue, but not pain. So we try to correlate the exam findings and your symptoms with your MRI. But there are many pain sensitive areas in your spine and once the area is cut, you may develop a new one. These are the most common reasons for poor surgical outcomes: Healthy tissue is cut or removed while leaving the pain producing tissues behind. The surgery removed the pressure from an injured nerve, but later scar tissue formed from the surgery causing greater nerve compression than before. The cut muscles or scar tissue that formed, from the surgery, changed the normal spinal mechanics leading to increased spinal degeneration and pain.
