We believe patients should be able to access health care in ways that best suit their needs. For some this may mean office visits, for others telephone or video visits, even house calls. Your choice! We strive to provide the care you want and need, when and how you want it. Appointments Services Appointments and Services Appointments Appointments Office Visits Standard Office Visit $210 Standard office visits allow face to face time for focused issues to complex concerns. Being together in the same space is always an advantage for communication and for visits requiring examination or procedures. Good Health Review $420 The Good Health Review is our opportunity to evaluate, review and dialogue about the health factors that impact your vitality. These factors include behavior and lifestyle choices, family medical history, preventive care needs and the status of ongoing chronic conditions. The Good Health Review is also an opportunity for us to better understand you in the context of your REAL life and therefore provide personalized, relevant advice and counsel for your health. Virtual Visits We realize that there are medical concerns that may appropriately and effectively be addressed without having to come into the office. We offer both scheduled Telehealth and Telephone visits, which ever is most convenient and appropriate for you. Telehealth visit $210 Home Visits and After Hours Care Home Visit Fee $300** After Hours Fee (6pm to 8am, M-F) $200** **Fees are in addition to the base visit rate of $210. Care outside of scheduled office hours is possible both offsite and at the TCM office. Arrangements are made on a case by case basis and are subject to provider availability. Other Services Professional Services available at an hourly rate. For example these may include accompaniment to specialist appointments, review of extensive medical records, extensive research. After Hours Urgent Care - Doctors are available by cell phone for urgent matters that cannot wait for regularly scheduled office hours. Telephone visit and/or after hours fees may apply. $420/hr Services Services Good Health Review Your visits may include extended consultations and personalized coaching to create positive lifestyle changes such as weight loss, smoking cessation and stress management. The Good Health Review is our opportunity to evaluate, review and dialogue about the health factors that impact your vitality. These factors include behavior and lifestyle choices, family medical history, preventive care needs and the status of ongoing chronic conditions. The Good Health Review is also an opportunity for us to better understand you in the context of your REAL life and therefore provide personalized, relevant advice and counsel for your health. Urgent Care Temescal Creek Medicine provides same or next-day evaluation for urgent medical problems including minor injuries and diagnosis, and treatment of problems appropriate to primary care (e.g. colds, illness, minor accidents; conditions that don’t require emergency, hospital or specialist care). Preventive Care Temescal Creek Medicine provides thorough, periodic physical exams and age-specific health screenings for men and women—school age children through seniors. Your provider will evaluate your current state of health and recommend improvements for your future health and well-being. Medications & Vaccines Temescal Creek Medicine providers may administer selected medications and vaccines in our office. While some of these items are provided at no additional charge, some are charged to you at our cost. Chronic Disease Management Temescal Creek Medicine incorporates national guidelines to provide our patients with state-of-the-art primary care for hypertension, diabetes, hyperlipidemia, heart disease, asthma, arthritis, osteoporosis and many other chronic conditions. Your physician may provide education and training to manage your chronic condition. If your chronic condition changes and you need more specialized care than your physician can provide, we will refer you to an appropriate specialist health care provider and assist you in making decisions relating to their care. Procedures Temescal Creek Medicine provides a broad range of medical procedures. Procedure fees are in addition to visit charges. A list of fees for procedures are available for patient review prior to any procedure. Vaccinations Ear Canal Flushing Rapid Strep Throat Testing Pregnancy Testing Cryotherapy (freezing warts, skin tags, precancerous lesions) Biopsy and Excision Wound Debridement (tissue cleaning) Incision and Drainage (abscesses) Laceration Repair (suture and dermabond) Splinting Diagnostic and Therapeutic Injection of elbow, knee, soft tissue Pre-employment, work, volunteer, sports participation, or adoption physicals We are happy to provide these types of examinations and complete the necessary forms. Please note these visits are considered administrative exams and are charged at the regular visit rate for all patients. We do not do worker compensation. The model is designed to minimize the dysfunctional relationship between patients, providers and insurance companies. Over 40% of every $1 spent on primary care traditionally goes toward costs associated with insurance billing and reimbursement. Instead of spending that 40% on back office staff and accounting or giving it to insurance companies, Temescal Creek Medicine uses it to deliver the excellent care that you want and deserve. Please see below for answers regarding different insurance products: Insurance and Payment PPO Insurance Answers Patients with PPO (Preferred Provider Organization) insurance are responsible for payment at the time of service. We will provide the appropriate claim form for you to submit to your insurance company for reimbursement. For PPO plans we are considered an “out of network” provider. Reimbursement for out of network care varies from plan to plan. Please check with your insurance carrier to verify your reimbursement amount. I thought PPO was “the good insurance”? Why are you “out of network”? Currently there are hundreds of different PPO insurance products and they are highly variable. Each insurance plan comes with its own restrictions, copayments, reimbursements and payment rules that change frequently and often without notice. Keeping up with these constant changes takes time and resources. We have made the choice to commit our energy to caring for patients not keeping up with insurance companies. Why would I want to go to a provider that I may have to pay more for? Now more than ever PPO insurance carries higher and higher patient copayments and deductibles. This means that patients are often responsible for the first $500 to $1000 no matter where they choose to receive care. Why not assume control over where and how you spend this money? You may use Health Savings Accounts or Flex Spending Accounts to optimize your tax benefits. Different people want and expect different things from their primary care office. If your wants and expectations are currently being met then you may choose to stay where you are. If you find value in our philosophy and approach including longer appointments, less wait time, a deeper relationship with your provider, options for care such as email, video and telephone, and prompt and personalized service then you may find Temescal Creek Medicine makes great sense. If I choose to use Temescal Creek Medicine, does that mean I can't use my insurance for my other health care expenses? Absolutely not! While we are out of network, we can help you make in network choices for your services outside of our office including lab, imaging, specialty care and hospitalization. We take our job of helping you make prudent health care spending decisions very seriously and we will not be shy about steering you away from wasteful health care spending. For many patients, the money spent on good primary care will ultimately result in less total out of pocket health care spending. HMO Insurance Answers Temescal Creek Medicine contracts with HMO plans through Affinity Medical Group for commercial and medicare advantage plans. NOTE: Please call us at 510 230-2372 or email
[email protected] to confirm whether we are currently accepting new HMO patients Who do I sign up with? You may sign up with either Michael Zimmerman, MD or Lauren Pallis, MD. As long as one of our providers is on your insurance card you may be seen by either. Does my HMO plan cover everything? HMO insurance covers office visits and some procedures. You are responsible for your copayments and any charges not covered under your policy. Aren't HMOs bad? What is an HMO anyway? Managed Care or HMO (Health Maintenance Organization) model insurance has received a bad reputation over the years. Some of this is deserved, some is not. The idea behind managed care is to have organized networks of healthcare providers take on real responsibility and accountability for the quality, cost and experience of the care they deliver. The HMO concept is great but unfortunately the first versions of this model were all about managing cost not managing care. The newest iterations of this model are Accountable Care Organizations that do continue to attempt to control costs but do so by focusing on quality and patient satisfaction. The managed care model is distinct from PPO or “fee for service” model in which providers only get paid for doing MORE, with limited if any control on cost or quality. While it is the American Way to want more, we have piles and piles of data demonstrating that more tests and more specialists equals higher costs, more injury, more death and lower quality. Medicare Answers Temescal Creek Medicine accepts Medicare insurance for established patients. Payment for services not covered by Medicare is due at the time of service. Medicare will usually submit uncovered services to your Medicare Secondary if you have it. Uncovered charges will be billed to your credit card on file. NOTE : Unfortunately, we are currently not accepting new Medicare patients. No Insurance - No Problem! Temescal Creek Medicine offers transparent, reasonable pricing for visits and services. A little investment in good primary care can avoid much more costly emergency care or hospitalization. We can also help direct patients to the lowest cost medication options. Zimmerman, M.D. Michael Zimmerman is a family physician who refuses to perpetuate a healthcare system that undermines the hard work of good people. He serves as expert faculty and consultant for the California Academy of Family Physicians and the California Medical Association Foundation chronic care and quality improvement collaborative. He is an alumnus of the UCSF/California Health Care Foundation Health Care Leadership Program and speaks nationally on performance improvement and practice redesign in the small office setting. Dr. Zimmerman is the former Chief Medical Officer at Affinity Medical Group, a Northern California IPA that includes over 600 physicians and cares for nearly 30,000 patients. In his position, Dr. Zimmerman is involved in defining, articulating and leading the quality care imperative for medical group leadership and for community physicians. Dr. Zimmerman was born and raised in the Bay Area and completed his undergraduate work at the University of California, Berkeley. He received his M.D. from Case Western Reserve University in Cleveland, where he was part of the inaugural class of the Robert Wood Johnson Generalist Initiative funded Primary Care Track. He completed training at Ventura County Medical Center Family Practice Residency. He is board certified in Family Medicine. He lives in Oakland with his wife, two children. Beyond medicine he enjoys cycling, baseball and traveling with his family. Lauren Pallis, MD Lauren Pallis joined Temescal Creek in 2023 after working as a hospitalist and then as a primary care physician. Through her prior professional experiences in various healthcare systems, Lauren learned that she wanted a smaller and more personalized setting to practice medicine, in order to be able to provide thoughtful and thorough care. After meeting Drs. Franchett and Zimmerman and learning about the ethos of their practice, she made the easy decision to join Temescal Creek Medicine. Lauren Pallis was born and raised in the Bay Area. She attended the University of California, Berkeley for college and left the Bay Area to attend medical school at Jefferson Medical College in Philadelphia, PA. She returned home to complete her family medicine residency at Contra Costa Family Medicine Residency. After residency, she spent a year in Seattle, WA at Swedish First Hill completing a fellowship in hospital medicine. As a provider, Lauren aims to meet people where they are and help them set realistic and achievable health care goals. She believes that one’s physical health is strongly tied to their mental and emotional health, and strives to understand all three aspects of patients’ lives when developing a treatment plan. Within family medicine, Lauren is particularly interested in women’s health, mental health, exercise and nutrition, as well as managing chronic conditions. Outside of medicine, Lauren enjoys cooking, being in nature, seeing live music, and spending time with friends and family. Shannon McLelland Care Coordinator Shannon was born and raised in Santa Barbara, California. She moved to the Bay Area for college where she received a bachelor's degree in Astrophysics from the University of California, Berkeley. She has worked in the medical field for over a decade. The majority of time has been spent in medical offices helping with everything from referrals to medical records. She also spent some time at Affinity Medical Group working on clinical quality with Dr. Zimmerman. In her free time Shannon enjoys crafts, video games and spending time with her husband and daughter. Kellan Hickman Care Coordinator The excitement of city life drew Kellan from Humboldt County to the Bay Area for college. She attended the University of San Francisco and majored in Spanish. Her love of travel took her to Sevilla, Spain for a year abroad. She continued her studies at San Francisco State University earning a Masters degree in Counseling, with an emphasis in Gerontology. Her previous work includes activities coordinator for older adults and case manager in low-income supportive family housing. Kellan enjoys cooking, running, and spending time with her husband and two children. Marguerite Buttrick Care Coordinator Marguerite has been working in medical office administration since 2006 in a variety of offices, including gynecology and gastroenterology. Originally from New York City and raised in a family of physicians, she is drawn to the healthcare environment and supporting patient needs. Marguerite particularly enjoys the strong sense of community and collaborative spirit shared among the staff at Temescal Creek Medicine. In her spare time she enjoys cooking, getting outside and being with her husband and 2 grown children. Gretchen Dunn Care Coordinator Gretchen Dunn is a Care Coordinator at Temescal Creek Medicine. With over a decade of experience in the healthcare-wellness space, operations and clinic administration. Prior to joining Temescal Creek Medicine, she worked across community health, education, and global mobility roles, building strong skills in service coordination, family support, and cross-cultural communication. Raised in San Francisco, Gretchen studied dance and the visual arts before earning her B.A. from Sarah Lawrence College and her M.F.A. in Photography from the Academy of Art University. Fluent in Italian and inspired by her travels and creative background, she brings a thoughtful, people-centered approach to her work. In her free time, Gretchen enjoys teaching kids Capoeira, and spending time with her daughter and their dogs. Please take a moment to review the items below to ensure we have a common understanding. Access to Care At Temescal Creek Medicine we limit the total number of patients we have to optimize our availability to deliver care. We attempt to balance daily schedules to allow for same day access the majority of the time. We also provide several different ways for you to interact with your doctor. We believe patients should be able to access health care in ways that best suit their needs. For some, this may mean office visits, for others, telephone or video appointments, even house calls. Our goal is to provide the care you want and need, when and how you want it. Timely Communication Temescal Creek Medicine has adopted policies to ensure timely communication between patients and our office. During business hours we strive to answer your calls directly 90% of the time. If you do get a voice message we will return calls within 30 minutes. We strive to reply to patient portal messages within 24 hours, often much sooner. After Hours non urgent voice messages or portal messages will be returned by the following morning. Weekend calls and messages will be returned Monday morning at the latest. The doctors provide 24/7 on call coverage for urgent matters that require a conversation with the doctor. For emergencies please dial 911 or go to the nearest emergency room. Team Care At Temescal Creek Medicine we believe healthcare is a team effort. Dr Zimmerman and Dr Pallis rely on each other and their Care Coordinator staff to be able to provide the best possible care. This team works in an integrated fashion using a shared electronic medical record. While you may often receive care from a specific provider you will get the most out of Temescal Creek Medicine by understanding us as a team. Coordination of Care Your primary care team can play a valuable role in helping to coordinate your health care across a presently fragmented health care delivery system. We have committed significant resources in a comprehensive Electronic Medical Record which allows us to collect and hold data from multiple sources. This includes reviewing and discussing information from tests and studies, specialty visits, and hospital based care. We will often require your assistance to ensure we have information and data from these sources to optimize our ability to help coordinate your health care. Performance Reporting and Improvement Temescal Creek Medicine seeks to improve effectiveness, efficiency, timeliness and other aspects of quality by measuring and reporting performance, comparing itself to national benchmarks, and providing regular feedback to doctors and staff. From time to time we may request your feedback in narrative or survey form and would appreciate your participation. Specialty Referrals If your health care requires the contribution of a specialist we can provide the necessary clinical information for you to bring with you. We have excellent relationships with specialists across the Bay Area and we are happy to make recommendations based on our personal experience and the experiences of our other patients. Patients are ultimately responsible for ensuring specialty and imaging care are covered benefits. We have the expectation that our specialists provide timely, pleasant and appropriate care. After your specialty visit we will often request feedback to confirm your experience was consistent with our expectations. Patient Engagement We believe that active patient participation and engagement is essential to achieve optimal health. Your health care team at Temescal Creek Medicine will work with you to provide support, information and guidance. We are interested in open, forthright conversations and shared decision making with regards to your health and care. Electronic Communication We encourage the majority of communication through messaging on our patient portal. This allows us to receive, route and respond to your message in the safest and most efficient manner. You are always welcome to call us if you have an urgent matter or feel uncomfortable with the patient portal. A note about email. While we do still use standard email for some limited communications, it is definitely the least efficient way to communicate with us. Common sense must always be exercised and Temescal Creek Medicine cannot guarantee the security and confidentiality of standard email communications if you opt to use them with us. Clinically relevant emails may be made part of the medical record and may be forwarded internally to staff in the course of care. Portal messages and email are NOT appropriate for urgent matters or medical emergencies. Do not use e-mail for sensitive matters. If a reply is needed before a message or e-mail is returned please contact the office by phone. Credit Card Authorization Cancellation and No-Show Charge In order to limit the amount of resources we need to divert from patient care to collect payments we require that all patients maintain an active credit card on file with our office. We accept Visa and American Express. Your credit card will be charged only when appropriate for charges not covered by insurance which may include email visits, telephone visit, copays, visit and procedure charges and cancellation charges at the time of service. You will receive an invoice with all charges. You may cancel you credit card relationship with us at anytime by written notification to Temescal Creek Medicine. In order to run an efficient business we rely on patients keeping scheduled appointments. We set aside time and resources based on our schedule. If you cancel an appointment with less than 24 hours notice and we cannot fill the appointment time you will be charged the full cost of the visit. If you "No Show" for the visit there is also a charge for the full cost of the visit.
