Essentials at a Glance
Program Type: Acute
To Make Referrals: Clients are referred through emergency services providers. We accept referrals 24 hours a day, 7 days a week through our referral line at 510-915-4977. We cannot accept walk-ins.
# of Beds: 26 beds
Who We Serve: Adults ages 18 and older who are experiencing an acute exacerbation of symptoms of mental illness who require 24-hour supervision and/or assistance with psychiatric recovery.
Services: All services are person-centered with the goal of enhancing an individual’s safety, health and quality of life. Services include, but are not limited to:
Collaborative treatment planning with a recovery focus
Comprehensive evaluation and risk assessment
Symptom management skills training
Crisis planning and prevention
Supportive counseling (group and individual)
Medication administration, education and training
Independent living skills training
Dietary consultation
Discharge planning and linkage to community support
Heritage PHF usual and customary per diem rate: $2,000 semi-private, $2,250 private
Hours of Operation: Open 24 hours a day, 7 days a week
About the Program: Treatment is based on the belief that even during the acute phase of an illness, all people have the potential to rehabilitate and make choices that support their hopes and dreams. Heritage’s program is staffed by a multidisciplinary team that provides assessment, symptom reduction, medication services, discharge planning, and community resources and supports development. We provide an environment where residents have increased control, responsibility and choice to support their ability to build relationships and reintegrate into the community.

Over these many decades, we have expanded care from inpatient to community-based services, from siloed systems to integrated care, and from a purely medical model to recovery-focused, evidence-based programs across 35 counties and five states. This evolution has never been easy—behavioral health has long been stigmatized and underserved in the broader healthcare system—but we have always moved forward with our partners in a spirit of determination and innovation. In 2025, we faced new and unanticipated external challenges to our country’s core safety net systems. While the changes have not been fully implemented, and greater challenges lie ahead, we are already witnessing destabilizing effects on people’s lives as food, healthcare, and basic needs are increasingly at risk. The pressure on behavioral health services has intensified at precisely the moment when millions across the country need them most. Telecare has weathered difficult times before. We have stepped up as providers, problem solvers, and advocates, and we are prepared to do so again. This year’s annual report is a testament to the innovative work our enduring partnerships yielded in 2025. It also reaffirms our commitment to the core belief that drives everything we do: recovery is possible. We are here to partner for good, no matter what the future holds. As a large inpatient and outpatient provider with nearly six decades of experience serving individuals with complex needs, we realize that this is a pivotal time to come together to end the suffering. We feel the urgency among those we serve and recognize the immediate ways we must break down barriers so we can enhance access and deliver recovery results. This year’s annual report highlights our most essential work, which would not be possible without vibrant collaboration at a state and local level. We worked shoulder to shoulder with many counties, states, and health plans to dramatically expand access and enhance services. This ranged from mobile crisis and acute care in urban settings to an innovative model of telehealth in rural areas. We came together in 2024 in ways we are uniquely prepared to do. We look forward to working together in 2025 as a safety net, with professionalism, passion, and vision to ensure those who are most vulnerable do not fall through the cracks. Looking Back on 2023 In calendar year 2023, we focused on what we can do as a provider to create critically needed access to care, innovative services, and systemic solutions. As our communities continued to suffer escalating rates of serious psychological distress, substance use disorders, and suicide; we met with our county, state, and health plan partners to learn more about their priorities, and how we can work together for a better future. We are glad to share the positive progress we’ve made working in tandem with our customers and community partners. Our hope is that this “good progress, good news” approach will allow us to focus on our strengths, and areas that are working, so we can build on these in the year ahead with optimism. This will be especially important given the scope of mental health policy and emerging budget uncertainties now on the horizon. Looking Back on 2022 In calendar year 2022, we saw the continued importance of resilience across the communities we serve. Not only did we need resilience to continue navigating the COVID-19 pandemic, we also needed resilience and determination to respond to the increased need for mental health services that is surging across our nation. This year’s report highlights some of the most significant emerging trends Telecare is seeing and our progress in advancing creative solutions together. Looking Back on 2021 In calendar year 2021, COVID-19 continued to reshape our world, changing the ways we ensure safety for those who are most vulnerable, and transforming behavioral health service delivery. The silver lining has been the deepening partnerships we have established with our customers, our communities, our staff, and our clients. Art, Lida, and Morton believed that clients could achieve better results when they received services that were individualized, focused, and provided in a more home-like setting. They placed the client at the center of the organization and created a culture where employees and clients were supported in reaching their full potential. Their vision still guides Telecare today and is carried on by Anne Bakar , Telecare's President and CEO, and daughter of co-founder Morton Bakar, who heads up the family- and employee-owned company. While Telecare has evolved over the years to address the changing needs of the people we serve, our overarching goal remains the same. We Help People to Feel Better Personally We believe that respectful, recovery-centered services can make a difference in people's lives. After getting services at Telecare, clients company-wide responded to the Mental Health Statistical Improvement Project (MHSIP) Survey, and said: We Integrate Physical Health, Mental Health, and Substance Use Care In 2017, Telecare partnered with Santa Clara County to open Partners in Wellness, the nation’s first mental health Pay For Success program. In 2019, we received 30-month performance results on that program. The results exceeded expected target savings. We Provide Effective Community-Based Programing Our research has shown that enrolling in one of our community-based programs for a year can mean: We Help Transition Individuals to Lower Levels of Care Telecare Community Alternatives Team (T-CAT) was developed by Telecare Corporation to meet the specific needs of those experiencing crisis or transitioning between levels of care in Washington State. The T-CAT program provides a range of voluntary, community-based, short-term wraparound services with the goal of minimizing inpatient stays. The program has two components: Transitional Support & Hospital Diversion. Originally one program, the model was so successful, we now have T-CAT programs in Pierce , Thurston, and Mason counties. Pierce County T-CAT Outcomes We Help Individuals Transition Out of Jail and Back Into Their Community When people with mental illness transition out of jail and back into the community, the hurdles can be significant. We help people re-imagine their lives, make progress in recovery, and stop the cycle of recidivism. Using Telecare's Recovery-Centered Clinical System (RCCS) to address Justice-Involved Mental Health (JIMH) issues has been key to creating a recovery-centered environment. Recovery is possible, here’s the evidence Telecare was founded on the belief that rehabilitation and recovery from serious mental illness are possible. We call our unique recovery approach the Recovery-Centered Clinical System, or RCCS. The RCCS is based on a belief that recovery can happen, but it does not cause recovery to happen. All programs are designed with Telecare's Recovery-Centered Clinical System (RCCS) as their foundation. The RCCS encompasses evidence-based practices and unique approaches developed over our company's history. Community-Based Telecare provides a range of services in the community for people with, or at risk for, mental illness. Our community-based services are designed for people who can successfully live independently in the community with intensive services and supports. Services can be provided on a voluntary or involuntary basis. Our acute programs link individuals with ongoing care in the community to prevent readmission. The typical length of stay is 3-10 days. Learn More » Residential Telecare's residential programs provide alternatives for individuals with mental illness who would otherwise be placed in higher levels of care, such as state hospitals and IMDs (Institution for Mental Disease). In home-like settings, residents address the issues and behaviors that put them at risk for institutional placement. Our community-based services are designed for people who can successfully live independently in the community with intensive services and supports. Assertive Community Treatment (ACT) The ACT model is a well-established approach for providing community-based services to individuals with complex needs. Services include assessment, services planning, housing assistance, pre-employment and vocational services, psychiatry and medication management, co-occurring disorder services, peer services, and assistance building natural supports. Full Service Partnerships (FSP) FSP programs are California-specific community-based programs. FSPs are consumer-driven and highly collaborative programs that provide an array of wraparound services to individuals with complex needs, supporting people in addressing mental health symptoms and conditions, while building natural recovery supports and resources. Case Management / Care Coordination These programs provide strength-based wraparound supports for individuals with mental illness, but with less intensity than the ACT or FSP levels. With the goal of preventing readmission, discharge teams provide time-limited case management for individuals leaving inpatient environments. Care Coordination helps clients, primarily those with mental illness and chronic health conditions, to navigate the system of care, thereby the reducing use of emergency and inpatient services. Prevention / Early Intervention These programs provide psycho-education, early intervention treatment, and coping skills development for people at-risk for mental illness. Support and education are also provided to family members. Free-Standing Acute Psychiatric Programs These programs are staffed by a multi-disciplinary team that provides assessment, symptom reduction, medication services, discharge planning, and resources and supports development. Along with being licensed by the state, a number of Telecare's inpatient acute facilities are Joint Commission accredited and Medicare certified. Crisis Telecare's crisis programs help people experiencing mental health crises to regain a sense of stability and avoid more restrictive settings. Crisis programs are typically unlocked settings for people to enter on a voluntary basis. Walk-In / Urgent Care These programs provide services to individuals who self-refer. Services include assessment, medication support, brief treatment, and links to community services. The typical length of stay is 6-8 hours. Crisis Stabilization / 23-Hour These programs provide screening/triage, medication support and brief treatment in a safe, protective setting. These programs are sometimes co-located with acute inpatient programs, allowing for a 'warm hand-off' transfer when needed. The typical length of stay is up to 24 hours. Telecare was founded on the belief that rehabilitation and recovery from serious mental illness are possible. We have consolidated our learning and experience into the Recovery-Centered Clinical System (RCCS), a richly personal, holistic, and comprehensive approach to recovery. The system was developed internally over 20 years with extensive involvement from Telecare's clinical and operational staff at all levels. The RCCS was also deeply shaped by direct input and collaboration with our clients and peer employees. Telecare has provided training and consultation on our RCCS with selected customers, including the State of Oregon. The RCCS is based on a belief that recovery can happen, but it does not cause recovery to happen. Instead, the RCCS strives to create an environment that supports recovery. It does this by concentrating on two distinct but complementary areas: culture and conversations. Culture is focused on the recovery environment. The recovery environment is created collaboratively by staff and the individuals served, and by the interpersonal relationships that are developed to foster a supportive program setting. At Telecare, culture work focuses on five areas of awareness: uniqueness, power, judgment, motivation, and respect. We believe that when we are mindful of these areas of awareness and thoughtful about our words and actions, we can cultivate an environment where recovery can grow and thrive. Conversations are focused on the individual. Conversations encompass five components: identity, hope, choice, harm, and connection. We believe that when people are asked about themselves and these important topics, with curiosity and non-judgment, that it creates space and opportunity for people to awaken and enliven their recovery process. Learn More Telecare's Recovery-Centered Measures (RCM) Tool Telecare created a research-validated tool to support our RCCS. This tool, Recovery-Centered Measures (RCM), enables Telecare programs to measure their recovery-centered culture. Staff as well as individuals served give feedback on how the culture impacts them and the services being delivered. Results are used to identify and grow strengths within a program as well as to provide an opportunity for cultural improvement initiatives. She is the only commissioner who brought a dedicated behavioral healthcare perspective to the Commission. In that capacity, she successfully advocated for specific behavioral health workforce recommendations that were included among the Commission’s final top ten recommendations. These include the following final recommendations: Expand the number of psychiatric residency positions in the state. This would enable California to add 2,202 psychiatrists to its workforce by 2030. Research Under certain circumstances, we may use and disclose health information about you for research purposes. Research is information about a group of people collected for the purpose of improving the treatment of conditions. For example, a research project may involve comparing the health and recovery of all clients who received one medication to those who received another, for the same condition. All research projects, however, are subject to a special approval process.
