As the son of Dr. Steve Yabuno , Dr. Daniel has been around dentistry his entire life. One of his fondest memories was when his father gave a presentation to his 1st grade class. Since then, Dr. Daniel has been studying hard and made a commitment to becoming the best dentist he can be. He attended the University of California, Irvine where he received his Bachelors of Science degree in Biology and graduated with honors. Soon after, Dr. Daniel moved to the Bay Area to complete his Doctor of Dental Surgery degree at the University of California, San Francisco. Prior to attending dental school, Dr. Daniel worked as a Dental Assistant at multiple offices. While attending school, he continued to work as a Dental Assistant during his breaks to gain as much knowledge as possible. Dr. Daniel is now a recent graduate and well educated in modern and cutting-edge standards of practice. He is a member of his local Western Dental Society, the California Dental Association, and the American Dental Association. In addition, Dr. Daniel enjoys learning about new dental technology and coming up with ways to modernize the practice. On his days off, you’ll find Dr. Daniel working out at the gym or playing basketball and pickleball. He is a lifelong Dodger and Lakers fan and loves watching all sports. HIPAA is a Federal Law that gives you significant new rights to understand and control how your health information is used. Federal HIPAA Omnibus Rule and state law provide penalties for covered entities, business associates, and their subcontractors and records owners, respectively that misuse or improperly disclose PHI. Starting April 14, 2003, HIPAA requires us to provide you with the Notice of our legal duties and the privacy practices we are required to follow when you first come into our office for healthcare services. If you have any questions about this Notice, please ask to speak to our HIPAA Privacy Officer. Our doctors, clinical staff, employees, Business Associates (outside contractors we hire), their subcontractors and other involved parties follow the policies and procedures set forth in this Notice. If at this facility, your primary caretaker / doctor is unavailable to assist you (i.e. illness, on-call coverage, vacation, etc.), we may provide you with the name of another healthcare provider outside our practice for you to consult with. If we do so, that provider will follow the policies and procedures set forth in this Notice or those established for his or her practice, so long as they substantially conform to those for our practice. OUR RULES ON HOW WE MAY USE AND DISCLOSE YOUR PROTECTED HEALTH INFORMATION Under the law, we must have your signature on a written, dated Consent Form and/or an Authorization Form of Acknowledgement of this Notice, before we will use or disclose your PHI for certain purposes as detailed in the rules below. Documentation – You will be asked to sign an Authorization / Acknowledgement form when you receive this Notice of Privacy Practices. If you did not sign such a form or need a copy of the one you signed, please contact our Privacy Officer. You may take back or revoke your consent or authorization at any time (unless we already have acted based on it) by submitting our Revocation Form in writing to us at our address listed above. Your revocation will take effect when we actually receive it. We cannot give it retroactive effect, so it will not affect any use or disclosure that occurred in our reliance on your Consent or Authorization prior to revocation (i.e. if after we provide services to you, you revoke your authorization / acknowledgement in order to prevent us billing or collecting for those services, your revocation will have no effect because we relied on your authorization/ acknowledgement to provide services before you revoked it). General Rule – If you do not sign our authorization/ acknowledgement form or if you revoke it, as a general rule (subject to exceptions described below under “Healthcare Treatment, Payment and Operations Rule” and “Special Rules”), we cannot in any manner use or disclose to anyone (excluding you, but including payers and Business Associates) your PHI or any other information in your medical record. By law, we are unable to submit claims to payers under assignment of benefits without your signature on our authorization/ acknowledgement form. You will however be able to restrict disclosures to your insurance carrier for services for which you wish to pay “out of pocket” under the new Omnibus Rule. We will not condition treatment on you signing an authorization / acknowledgement, but we may be forced to decline you as a new patient or discontinue you as an active patient if you choose not to sign the authorization/acknowledgement or revoke it. Healthcare Treatment, Payment and Operations Rule With your signed consent, we may use or disclose your PHI in order: To provide you with or coordinate healthcare treatment and services. For example, we may review your health history form to form a diagnosis and treatment plan, consult with other doctors about your care, delegate tasks to ancillary staff, call in prescriptions to your pharmacy, disclose needed information to your family or others so they may assist you with home care, arrange appointments with other healthcare providers, schedule lab work for you, etc. To bill or collect payment from you, an insurance company, a managed-care organization, a health benefits plan or another third party. For example, we may need to verify your insurance coverage, submit your PHI on claim forms in order to get reimbursed for our services, obtain pre-treatment estimates or prior authorizations from your health plan or provide your x-rays because your health plan requires them for payment; Remember, you will be able to restrict disclosures to your insurance carrier for services for which you wish to pay “out of pocket” under this new Omnibus Rule. Marinel has been in the field of dentistry for 25 years, beginning with her graduation from University of the East in Manila, Philippines in 1995 with a Doctoral Degree of Dental Medicine. In 1996, she attended a preceptorship program in Prosthodontics at UCLA, and later earned her Dental Hygiene license in Florida in 2010. The UCLA program Marinel attended sparked her interest in Prosthodontics, which strongly ties in with one of her outside office hobbies of arts & crafts. However, Marinel’s greatest enjoyment in dentistry comes from the ability to assist in providing patients with superb dental care, and ensuring patients comfort by guiding and educating them throughout each step of their treatment performed. Outside of the office, Marinel also enjoys cooking and traveling with her family. Anna Registered Dental Assistant Anna is a Registered Dental Assistant, and a Certified Dental Instructor and holds a degree in Psychology. She has been working in the dental field since the late 80’s and joined Dr. Yabuno’s team in the early 90’s. Anna has had a passion for dentistry since the beginning. She finds excitement and self-fulfillment in helping others, and is ever-eager to learn new dentistry, while sharing her knowledge and expertise of traditional methods. Anna is married with two wonderful sons, and a fur baby girl Corgi. In her free time, Anna enjoys walking by the beach, riding bikes, reading, traveling, and volunteering at animal shelters to rescue and support abandoned, stray or abused dogs. Susan Registered Dental Hygienist Susan grew up in the South Bay area and attended undergraduate school at UCLA. Wendy Registered Dental Hygienist Wendy, one of our experienced dental hygienists, has been a part of Dr Yabuno’s team for two years. Her goal is to help patients improve, not just their dental health, but their overall health. Wendy has been practicing since graduating from USC in 1991, and she is continually looking to expand her knowledge. Most recently, Wendy trained as a Myofunctional therapist, learning to recognize signs and symptoms related to obstructive airway issues, which can lead to apnea, cardiac arrest and strokes, among other serious health related issues. Wendy loves to spend her free time with her family, standup paddle boarding with her dog and friends, and drawing and painting in watercolors. She is also an advocate for animals and volunteers at a local shelter. Rebecca Front Office Coordinator Rebecca moved from her hometown of Detroit, Michigan to Los Angeles in 2009, and joined Dr. Yabuno’s dental team in 2012. She attended UCLA before pursuing her Doctor of Dental Surgery degree at Columbia University College of Dental Medicine. She then pursued her Advanced Education in Periodontics at the NYU College of Dentistry. She has been working in dentistry for 9 years and counting joining Dr Yabuno’s dental office in July of 2015. Her favorite part of her position is getting to work with such an amazing team as she addresses the periodontal needs of the community! Dr. Kenigsberg actually met her husband while she was attending dental school in New York. Like her, he was an immigrant who moved to Brooklyn, NY from Ukraine with his family when he was seven years old. Together, they lived in New York for eight years and had a beautiful daughter named Renee there. In October of 2015, they decided to move to Los Angeles, where their second daughter Nicole was born. Dr. Kenigsberg holds great pride in her career and her family. Outside of work, her daughters are her biggest interest, and she loves walking with them, playing dress up and pretend, and traveling. She also enjoys going to the theatre and running. Coffee is something she is very particular about, and she would love to open her own boutique coffee shop one day.
