UCLA Health is committed to helping eligible patients who are not able to pay for all or part of their care. UCLA Health’s commitment to our mission and vision includes providing full financial assistance (100% discount) or partial discounts to eligible patients who are low income, uninsured or under-insured. Further information on UCLA Health’s Financial Assistance Program is provided below. Eligible services The Financial Assistance Program applies to emergency or other medically necessary healthcare services provided and billed by UCLA Health. Separately-billed physician professional services furnished in inpatient, outpatient, and emergency hospital departments are not eligible for consideration under the Financial Assistance Program. Determination of eligibility Eligibility is determined based on review of a completed Financial Assistance Application and supporting documents, including proof of income and liabilities. Generally, patients with family income at or below 400% of the Federal Poverty Level will be eligible for a discount of 100%. Patients with family income between 401% and 450% of the Federal Poverty Level may be eligible for a partial discount based on income level. If you receive financial assistance under our policy, you will not be charged more for emergency or other medically necessary care than the amount generally billed to patients having Medicare fee-for-service coverage. In some instances, patients may be presumptively determined eligible for financial assistance. At UCLA Health, our expert teams are consistently ranked among the top in the nation by U.S. News & World Report . When you choose us for head and neck surgery, you benefit from: Targeted care: We provide many subspecialized programs so patients receive personalized care for their head and neck related conditions. For example, in 2005, we established the first Voice Center in the western United States. We are also one of only a handful of centers in Southern California where patients can have surgery for cochlear implants. Recognized expertise: UCLA otolaryngologists (head and neck specialists) are nationally and internationally known as leaders in their fields. Each year, more than 1,000 patients come from other countries to receive care at UCLA Health. Many of our head and neck surgeons achieve recognition as Southern California Super Doctors, a list voted on by health care professionals and published by Los Angeles Magazine. Exceptional outcomes: Since we established our program more than 20 years ago, we have performed more than 1,500 microvascular reconstructive surgeries (surgeries that use microscopes). Our patients experience outstanding outcomes — our success rate with these procedures is higher than 99%, exceeding the national average of around 90%. Our areas of care We have a wide range of programs and services to connect you with the care you need. If you are unsure what specialty care you need, our staff can help you select a specialist and discuss referrals. Our areas of care include: Subspecialty programs and centers Airway Stenosis Program : Through the Airway Stenosis Program, we perform procedures to treat breathing problems due to airway narrowing. We use the latest techniques to improve patient outcomes and quality of life. Our multidisciplinary approach includes the expertise of specialists in head and neck surgery, thoracic surgery , pulmonology and more. Balance Program : This unique outpatient clinic combines the expertise of UCLA otolaryngologists with physical therapists. We use a true multidisciplinary approach to diagnose and treat dizziness and balance disorders. Our specialists use a range of techniques to create a unique, individualized treatment plan for every patient. Cochlear Implant Program : Since 1997, we have provided cochlear implants for more than 500 patients of all ages. We are one of the largest cochlear implant centers in Southern California and specialists around the country recognize us as experts. Endocrine Surgery Program : We use a team approach that includes the expertise of endocrinologists, surgeons, radiologists, and pathologists. Patients from all over the western United States come to us for treatment of thyroid and parathyroid disorders that require surgery. We collaborate with specialists in multiple areas across the hospital to ensure that each patient has access to any necessary specialty care. Head and Neck Cancer Program : Each year, we complete more than 50,000 patient visits, making us one of the most active head and neck cancer treatment centers in the nation. We use advanced diagnostic tools and treatments for head and neck cancers. Thanks to our active, nationally recognized research program, patients also have access to a range of clinical trials. Nasal and Sinus Disease Program : We use a comprehensive approach to care that incorporates the expertise of otolaryngologists, allergists, and surgeons. Since 1987, we have been using endoscopic approaches to sinus surgery, meaning many patients can have surgery with no facial incisions. We perform more than 400 operations annually, with more than 90% of our patients experiencing significant symptom relief. Sleep Surgery Program: We are recognized internationally as experts in the surgical evaluation and treatment of obstructive sleep apnea and snoring. Our experience and clinical research trials are based in our commitment to finding the right approach for each patient and delivering the best possible outcomes. Swallowing Disorders Program : In this program, we use a unique team approach that includes an otolaryngologist and a speech pathologist. At other medical facilities, these specialists work in separate locations, requiring patients to schedule multiple appointments to get the care they need. At UCLA Health, we coordinate your care in one convenient setting and quickly set up specialty referrals, if needed. Voice Center for Medicine and the Arts : We are at the forefront of clinical research, diagnosis, and treatment of voice disorders. The treatment team includes multiple subspecialists, such as laryngologists, voice scientists and speech pathologists. Many of our patients who have suffered from poor or lost voice have regained vocal skills through our center. Advanced surgery techniques and specialty areas Our head and neck specialists are experts in advanced head and neck surgery procedures. Facial plastic and reconstructive surgery: We take a refined, professional approach to facial plastic and aesthetic surgery. We act as your health care advocate and work with you and your family to provide individualized care that helps you live your healthiest life. Why choose UCLA Health for primary care? The primary care physicians (PCPs) at UCLA Health are here for you in all stages of your life. As the point person for your health care, your PCP should always be your first point of contact. Whether you’re sick, injured or have a general health concern, you’ll receive outstanding, individualized care. We offer: Comprehensive care: Our PCPs include doctors who are board certified in internal medicine, family medicine, pediatrics, or both internal medicine and pediatrics. This means we’re qualified to care for everyone in the family. We build strong relationships and deliver personalized, well-rounded care. Convenience and flexibility: We make it easy to find a skilled primary care doctor near you. Our network includes more than 50 UCLA Health primary care clinics throughout Southern California and more than two dozen clinics that provide primary care to children. Many practices offer extended hours, virtual visits, weekend appointments and flexible scheduling. We’re committed to helping you stay healthy, even when life gets busy. On-site services: Many of our primary care practices offer on-site imaging, blood draws and lab work, behavioral health services and pharmacy consultations. Some also have dermatology, cardiology and cardiac imaging, endocrinology and diabetes care, and other UCLA Health specialty physicians right in the same building. Having these services on-site saves you time and ensures that you have easy access to the care you need. World-class, coordinated care: If you need specialty treatments, you’ll have access to the full resources and support of the renowned UCLA Health network. Your primary care doctor will make referrals and oversee and coordinate your care with other specialists and programs. You and your family will receive exceptional, seamless care. Our areas of care Our primary care physicians deliver comprehensive care to you and your family. We offer everything from routine screenings and disease prevention to coordinated treatments for a wide range of health conditions. Our goal is to help you feel good and stay healthy. We offer: Behavioral health services Coordination of care for patients with chronic medical conditions and multiple specialist physicians Geriatric care Newborn care Palliative care Preventive care, including immunizations and routine screenings for cancer and other diseases Same-day sick appointments Travel medicine, including immunizations Well care for children and adults Well-woman care Services we offer Your primary care doctor will want to see you for your annual physical and whenever you are sick or injured. They’ll get to know your medical history and personal risk factors and work with you to make positive lifestyle changes that support your long-term health. Also called a general practitioner, your PCP: Coordinates your care and acts as the leader of your health care team Helps you manage chronic medical conditions, such as hypertension and diabetes Learns your values and preferences to help with shared decision making Manages your medications, adjusts dosages and orders refills Recommends preventive health screenings, vaccines and routine tests for cancer and other diseases Refers you to specialists as needed Treats common and acute medical conditions The difference between preventive care and follow-up or diagnostic care Let’s say you schedule an annual check-up with your primary care provider (PCP) where you get a flu shot and talk about a colonoscopy. At the appointment, you tell your doctor about new pain you’re having in your joints. This appointment may have started off as preventive care but turned into diagnostic care or treatment management. These two different types of care are billed separately. While most health insurances cover preventive care without a share of cost, they don’t always cover diagnostic care (for new symptoms) or follow-up care (for acute or chronic disease management) without a share of cost, such as a copay or deductible. Even for preventive care, there may be some charges for certain tests provided as a part of the preventive care evaluation. When scheduling an appointment with your PCP, it is always best to let the office know why you’re making the appointment. Tell the office if this is only for preventive care or if you also have medical conditions or concerns that you would like to talk about. Preventive care includes: Checking blood sugar, blood pressure and cholesterol based on screening guidelines Discussing age-appropriate cancer screening tests Evaluations for older adults (such as fall risk, hearing loss, brief assessment for memory loss, advance directives) Routine vaccinations (flu, COVID-19, pneumonia, etc.) Measuring height and weight Depression screening Preventive care does not include: Diagnosing, discussing or treating new symptoms Managing chronic diseases (such as obesity, diabetes, high blood pressure, high cholesterol, depression, anxiety, dementia, etc.) Following up on previously diagnosed conditions Ask your PCP if you have questions about the difference between preventive care and follow-up or diagnostic care so you’re not surprised by any costs. This means the information is securely transmitted and is treated the same way it would be if you were meeting with your doctor in person. I’m having technical problems during my video visit. What should I do? If you're having any issues with your video visit, turn your computer, smartphone or tablet completely off and back on again. If the problem continues, try turning off Wi-Fi. If you have audio problems, leave the meeting and try the following before rejoining: Ensure volume is turned up and phone audio is not turned off. Turn audio off and back on. I felt like I repeated my symptoms many times and was asked questions that I answered already upon describing my symptoms. It was frustrating and concerning because it made me feel like she was only half listening. For example, I explained my three main symptoms when the appointment first started and then she later inquired if I had two out of the three symptoms that I explained is the reason why I scheduled the appointment. 5.0 March 10, 2026 Dr Qwak is as usual very professional and caring. I appreciate her concerns for me and keeping up with my meds etc 5.0 March 8, 2026 Dr. Kwak always makes me feel comfortable, heard, and explains multiple solution options when needed. 5.0 March 6, 2026 Dr. Kwak is very personable and professional thorough and she makes a difference with positive results. 5.0 March 5, 2026 I was nervous about my upcoming gallbladder surgery and Dr. Kwak was so reassuring and kind. Also very knowledgeable and explained the anatomy of what I’m experiencing so well. I am grateful for Dr. Kwak. 5.0 March 3, 2026 Kind, knowledgeable and empathetic. 5.0 February 26, 2026 Dr Kwak was very kind and knowledgeable today at our visit.
