In our office, scaling is usually done with a Piezo ultrasonic scaler, which uses high-frequency vibrations to break up and remove tartar below the gum-line. This is a gentle procedure, but if you feel any discomfort, please let your provider know. If it has been a while since your last cleaning, doing flossing twice a day a couple weeks before the cleaning will help the visit. After the teeth have been cleaned, they will be polished with a special toothpaste to remove any leftover plaque and tartar. The polishing will leave your teeth feeling smooth and looking shiny. Finally, a fluoride treatment may be applied in the form of a gel or foam to your teeth. Fluoride is important for the prevention of tooth decay and is effective in children and adults and especially important for patients with a history of tooth decay. Deep Scaling Procedures During a routine dental cleaning the ultrasonic devices or hand scalers will clean above the gum line and 1-2 mm below the gum line.  At times plaque and bacteria can grow 4mm to over 7mm below the gum line.  To clean these areas a deep scaling and root planning procedure will be done.  This typically utilizes local anesthesia with a topical anesthetic to allow for the procedure to clean deep below the gum line while providing comfort to the patient. During this procedure a debridement can, be done with an antibacterial rinse.  Many times, our office will also use a low-level intensity diode laser.  This allows for destruction of harmful bacterial and promotes re-attachment of the periodontal structures.  When used in this manner the laser is not painful and can even be used without a local anesthetic. After the Procedure After your dental cleaning, you should brush and floss your teeth as usual. Your teeth may be sensitive for a few days after the cleaning, but this should go away within a week. You may also be advised to use a special toothpaste or a mouth rinse to help with sensitivity. Dental Cleaning Frequency The American Dental Association (ADA) recommends that patients have a professional dental cleaning every six months . However, some patients may need to be seen more frequently depending on their oral health. For example, patients with gum disease may need to be seen every three to four months for deep cleanings.  Caring for Your Teeth Between Cleanings You can help prevent plaque and tartar buildup by brushing and flossing your teeth daily.  Use a soft-bristled toothbrush and toothpaste. Direct the brush to where the gums and teeth meet. Use gentle circular motions. Take your time and brush for at least two minutes. Floss at least once a day.  If possible, brush your teeth after every meal. Consider using an antibacterial mouth rinse to help control plaque and tartar buildup. Not all teeth that require a crown will need a root canal.  Your dentist will evaluate the condition of your teeth and develop a plan for you. A root canal is a procedure in which the dentist removes the damaged nerve tissue inside your tooth. Once the nerve tissue has been removed, the tooth is sealed and restored with a dental filling or crown. These crowns can be made on teeth and also on dental implants. Zirconia Veneers can be used in the esthetic zone for a beautiful conservative ceramic smile enhancement. EMAX Porcelain Crowns and Veneers EMAX Porcelain crowns are the most popular type of dental crown. They are made to match the color of your natural teeth and are very durable. These EMAX crowns are made by CAD/CAM technology and designed just for you.  These are beautiful restorations that can be utilized for crowns and veneers. Porcelain-Fused-to-Metal Crown (PFM) These crowns have a metal inner structure with porcelain fused over the top. They are also matched to the color of your natural teeth. These crowns were the main crown in dentistry from the 1950s until recently. With the advent of beautiful ceramic metal free restorations with excellent fit, these crowns are not used as much anymore. A PFM would typically use a precious Nobel metal. They are more expensive than other types of crowns but are durable and last a long time. Many people don’t like the esthetics of full gold crowns. These crowns can have a more conservative preparation to conserve tooth structure. They can be used on a patient’s second molars with a strong bite.  However, in many situations, a high density ceramic crown can be used. Stainless Steel Crown Stainless steel crowns are typically used for children’s primary teeth or as a temporary restoration. They are less expensive than other types of crowns but do not look as natural. For this reason, they are usually used for posterior teeth. Onlay Restorations Onlays and inlays are made from porcelain ceramic. These restorations cover the biting surface of a tooth. These can be more conservative that full coverage crowns and can be effective when half the tooth is damaged and the other half is in good condition. The only would restore the damaged area and the remainder of the tooth would remain intact. Gold or Ceramic can be used for these restorations. Our office would typically use a ceramic onlay. The Crown Procedure The procedure for getting a dental crown usually takes two visits to the dentist. During the first visit , the dentist will prepare the tooth by removing any decay and shaping it so the crown can be placed over it. A dental impression will be made of the tooth and sent to a dental lab where the crown will be made. A temporary crown will be placed over the prepared tooth until the permanent crown is ready. Our office will typically use a Trios Digital Scan to impress the tooth and wire the information to our dental laboratory. At times, a conventional “gooey” impression may be needed. (Note: If you have a temporary crown, avoid eating sticky and chewy foods. They can cause the crown to come off as the bonding agent is only temporary cement. Avoid crunchy or chewy foods that might break or dislodge the dental crown.) During the second visit , the dentist will replace the temporary crown with your permanent one. The permanent crown will be checked for fit and cemented into place with dental adhesive. Depending on the bonding material used, a curing light might be used to harden the cement. Any excess cement will be removed, and you will be given instructions on caring for your new dental crown. After the Procedure It is normal to have some sensitivity to hot and cold temperatures after the procedure. This should go away within a few days. This may be discomfort for a couple of seconds and then resolve. Be sure to brush and floss your teeth as you normally would. This will help keep your gums healthy. During a root canal, the damaged dental pulp (the soft tissue inside the tooth) is removed, and the inside is cleaned and sealed. Root canal therapy is necessary when the pulp becomes infected . This can happen due to decay, a deep cavity, cracked or chipped tooth, or an injury. If the pulp is damaged and not treated, it can become infected. The infected pulp can cause pain, swelling, and abscesses (pockets of pus). An abscess can damage the bone around the tooth and spread to other body parts if left untreated. A root canal procedure can save a tooth damaged by decay or injury and prevent the need for extraction. Signs You Need a Root Canal Procedure There are a few telltale signs that you may need treatment. If you have any of the following symptoms, please call our office right away for an appointment: Severe toothache pain that doesn’t go away Sensitivity to hot or cold temperatures that lingers after the stimulus has been removed Discoloration of the tooth Swelling or tenderness in the gums A small bump on the gums near the affected tooth The Root Canal Procedure During the consultation appointment, the dentist will evaluate the damage to your tooth and look for signs of infection. They will take x-rays to determine if a root canal is the best treatment option. If a root canal is recommended, the procedure can be completed in one or two appointments. First, the infected tooth will be numb with local anesthesia. If an alternate form of sedation is desired, your dentist can discuss that with you. Once the tooth is numb, a sheet of rubber called a rubber dam will be placed around the tooth to keep it isolated and clean during the procedure. A dental emergency is any type of dental problem that requires immediate treatment or threatens your oral health . This can be the result of an accident, sports injury, or fall.  Examples of dental emergencies include: Knocked out tooth Severe tooth pain An abscessed or infected tooth A broken tooth requiring a root canal Lost crown Jaw swelling or pain A foreign object stuck in gums or mouth How Do I Know if It’s Urgent? Not every dental problem requires immediate treatment. For example, a chipped tooth that is not causing pain or discomfort can usually wait to be seen during regular business hours. On the other hand, a dental emergency like a knocked-out tooth or severe toothache will require urgent care to avoid further damage or pain. If you are ever unsure whether or not you are experiencing a dental emergency, it is always best to err on the side of caution and call your dentist immediately to discuss treatment options. How To Deal With Dental Emergencies at Home There are some things you can do at home to help until you can get to the dentist. Knocked Out Tooth If you have a tooth that has been knocked out, it is important to remain calm and try and find the tooth. Once you have found the tooth, gently rinse it off with warm water. DO NOT scrub the tooth as this can damage it. If possible, try to re-insert the tooth into the socket. If you cannot do this, place the tooth in a cup of milk or water. It is important that you get to the dentist as soon as possible as there is a time limit on how long the tooth can be out of the mouth before it is no longer viable. Severe Toothache If you have a toothache, rinse your mouth with warm water and use dental floss to remove any food that may be stuck between your teeth. You can also take over-the-counter pain medication like ibuprofen to help with the pain. If the pain persists, call your dentist right away as this could be a sign of an infection. Abscessed Tooth If you have an abscessed tooth, rinse your mouth with warm salt water to help reduce the swelling. You can also put a cold compress or ice pack on your cheek to help with the pain and swelling. Seeing your dentist as soon as possible is important as abscesses can spread and become very serious. Foreign Object Stuck in Mouth If you have a foreign object stuck in your mouth or between your teeth, try to remove it with dental floss . DO NOT use a sharp object like a knife or scissors as this can cut or damage your gums. If you are unable to remove the object, call your dentist. Periodontal disease is an infection of the gums that can damage the bone around your teeth. It is caused by plaque, a sticky film of bacteria that forms on your teeth. Plaque produces toxins that irritate and inflame the gums. If plaque is not removed, it can harden into calculus (tartar). Calculus is more difficult to remove and can only be removed by a professional cleaning. If plaque and calculus are not removed, they will continue to damage the gums and bone. A bone graft can be used to regenerate bone on teeth and prepare for implants.   Bone Grafts are widely used in dentistry and medicine and are safe and predictable. Bone is also flexible and dynamic. During some of our procedures, we can use our patient’s own bone to augment the site. This is minimally invasive and allows for a predictable long-term outcome. Dental bone grafting is a procedure often used to help build up the bone in your jaw after tooth loss to support dental implants.
