Diagnosis and Treatment If you think you might have a TMJ disorder, you should make sure to visit your dentist as soon as you can. At our offices, we can check the joint for any signs of the disorder. This will allow us to verify whether the symptoms you’ve been experiencing are the result of a TMJ disorder or not. If we discover that you do indeed have a problem with your temporo-mandibular joint, we’ll discuss treatment options with you. And not only do they look like normal teeth, but they act and react like your normal teeth as well. What Makes Composite Fillings Different? The outside of your tooth is made up of enamel, a hard, strong substance made up of calcium crystals bound tightly together. However, enamel is not made of living tissue. This means that porcelains used in dentistry can easily mimic the enamel of your teeth, both in appearance and in function. The inside of your tooth, meanwhile, is softer and more porous, and filled with deposits of calcium. This material is known as dentin, and it is both sensitive and alive. This allows your teeth to feel heat and cold and other sensations, sending impulses through the pulp and into the nerve. Traditional metal fillings could not imitate this behavior, but composite resins used in modern fillings can, because they are made of a material very similar to dentin. Further Advantages of Composite Fillings. Metal fillings also were unable to bond with the tooth. Instead, they simply rested in the cavity. It is capable of reducing the risk of tooth decay and cavities, as well as preventing plaque buildup on the surface of your teeth. When plaque and other forms of bacteria build up, your teeth will lose part of their enamel layer. Over time, this leaves them vulnerable to decay. However, fluoride can strengthen the enamel of your teeth by adding minerals back to the enamel layer, reducing the risk of tooth decay. In the early stages of tooth decay, it can even help reverse any decay that has taken place, though obviously fluoride cannot undo all tooth decay. How Is Fluoride Applied? To keep your teeth healthy, we can apply fluoride directly to your teeth. To do this, we use special rinses and toothpastes that contain a great deal of fluoride. You can purchase both fluoride toothpastes and mouth rinses over-the-counter, but only the weaker kind. The strongest and most effective fluoride rinses and toothpastes can only be obtained with a prescription. In our offices, we can also apply fluoride to your teeth in much higher doses. There are three main methods that we might use. First, we can apply a fluoride foam to a mouthguard and place it in your mouth for at least a minute (sometimes more, depending on the circumstances). Secondly, we can paint the fluoride onto your teeth with a special varnish. And finally, we can apply fluoride as a gel using either the painting process or a mouthguard. What Else Should I Know about the Process? Fluoride treatment will generally only take a few minutes. However, once the treatment is finished, we will ask you not to eat, drink, or rinse your mouth for at least half an hour. This is to allow your teeth to fully absorb the fluoride, as food and water could potentially interfere with this process. How often you receive fluoride treatment largely depends on your oral health. Most patients receive fluoride treatment once a year, though some need it every three or six months. Inlays are often safer and better alternatives to fillings, since fillings and other forms of direct restoration tend to shrink as they harden, potentially putting strain on the tooth or even leaving gaps between the tooth and the filling. Inlays, however, are hardened and shaped before they are placed in the tooth, meaning that they will fit perfectly and will not shrink, better protecting your remaining tooth from further damage down the road. Onlays If the cusp (or the edge) of your tooth is severely damaged by trauma or decay, then we’ll most likely suggest an onlay. Like inlays, onlays are fabricated outside of the mouth based on an impression of your tooth, and is therefore suited to your specific needs. Then, the onlay is placed over the existing tooth, effectively covering the damaged cusp, similar to a crown in many ways. Onlays will often require a temporary covering while the final onlay is being fabricated, meaning that you may need to make at least two appointments. In some cases, however, trauma to the tooth or consequences of certain types of restorations can damage the nerve sufficiently to require a root canal. If the nerve has been damaged, or if a cavity manages to reach the pulp, the pulp becomes infected and spreads the rot through the root and into the bone of your jaw or skull. The rot eats away at the bone and can cause serious damage if left untreated. This damage is called an abscess. Not only can it damage the bone of your skull or jaw, but this can also compromise your entire immune system. If the pulp of your tooth is infected, then it will be completely unable to heal on its own, so you need to come in for treatment by a root canal specialist at our offices. Your pulp being infected can be dangerous and extremely painful, and it’s always best to take care of the problem as quickly as possible. Symptoms If the pulp of one of your teeth has become infected, then you may experience: A bad taste in your mouth Increased sensitivity to hot and cold temperatures Increased sensitivity to sweets Swelling Tooth pain, especially when biting or chewing It is also possible, however, that you will experience no symptoms even if the pulp of your tooth has become infected. In these cases, you will be unaware of what is happening to your tooth until we detect it during your regular checkup. What Will Treatment Be Like? Once we find the infection of the pulp, we’ll let you know what’s going on and discuss when to schedule a root canal. At the start of the procedure, our root canal specialist will remove the infected pulp and clean out your tooth, making sure to disinfect it along the way to prevent further damage. When this is done, we’ll fill in the canals of your tooth to make sure it doesn’t get infected again. In most cases, we’ll recommend getting a crown and a core build-up to completely restore your tooth after root canal therapy. If, for any reason, you are uncomfortable with the idea of a root canal, or if the damage has spread too far, the only other option open to us would be to extract the tooth entirely.
