If you've lost a tooth (or a few teeth) due to injury or disease, dental implants can rejuvenate both your smile and your oral health. An implant is a synthetic tooth root in the shape of a post that is surgically placed into the jawbone. The “root” is usually made of titanium: the same material used in many replacement hips and knees, and a metal that is well-suited to pairing with human bone. A replacement tooth is then fixed to the post. The tooth can be either permanently attached or removable. Permanent teeth are more stable and feel more like natural teeth. The ideal candidate for implants is a non-smoker who has good oral health, including a sufficient amount of bone in the jaw, and healthy gums with no sign of gum disease. Single or Multiple Implants Implants are versatile. If you are only missing one tooth, one implant plus one replacement tooth will do the trick. If you are missing several teeth in a row, a few strategically placed implants can support a permanent bridge (a set of replacement teeth). Similarly, if you have lost all of your teeth, a full bridge or full denture can be permanently fixed in your mouth with a strategic number of implants. Advantages Over Dentures or Bridges Conventional bridges and dentures are not fixed to the bone, and can therefore be unstable. This can make it difficult to eat or smile with confidence. Implants not only look more natural, but feel and act more like normal teeth, with a stronger biting force. And because they don't directly rely on neighboring teeth for support, implants don't compromise the health of your natural teeth. In fact, bridges are only expected to last seven to ten years, even less with root canals, whereas implants will typically last a lifetime. Post-Treatment Care Consider your replacement teeth to be the same as natural teeth. They require the same daily brushing and flossing, and the same amount of regular checkups. Just like your natural teeth, the better you take care of your replacements, the longer they will last. Implant Services Our implant services include: Sinus Lift Augmentation: Often the sinuses will enlarge following loss of the upper back teeth. This can preclude placement of dental implants in the upper back areas of the mouth, since the sinus is an air-filled space and dental implants need to be surrounded by solid bone. A sinus lift augmentation/graft is a surgical procedure that restores the sinus cavity to its original size, and replaces the bone that has been lost as a result of the enlarged sinus. This new bone graft typically takes nine to 12 months to heal before the dental implants can be placed into the grafted bone. Autogenous Bone Grafting: Occasionally, the ideal areas for dental implants do not have sufficient bone quantity to support the implants. In these cases, it is often necessary to “borrow” bone from another area of your mouth and graft it into the deficient area. Often, the area of choice is from the chin or in the back of the jaw where your wisdom teeth used to be. Implant Site Development: When a tooth needs to be removed due to infection, it is not uncommon to have lost bone around the infected tooth. In these cases, various types of bone grafting can be done at the same time the tooth is removed to facilitate the placement of a dental implant at a later date. Computer-Enhanced Treatment Planning: We have special software on our computer that allows us to evaluate your dental CT scan in three dimensions for bone quality, bone quantity, and bone location, enabling the best possible placement of your dental implants. This also allows us to work closely and smoothly with your restorative dentist to ensure an ideal result where your dental implants are concerned. Implant Maintenance: Once your dental implant restoration is completed, it is important to keep the area clean and infection-free. Dental implants can accumulate plaque and tartar just as teeth can. Whether you have just one tooth replaced by an implant or several, a professional cleaning schedule will be necessary to keep your implants healthy. The time interval for these professional cleanings will be determined by your individual needs. Implant Restoration If you’ve lost a tooth, your dentist may have recommended getting a dental implant to fill the empty space and allow for optimal function of your teeth. The dental implant itself is only a replacement for the root of the lost tooth, so after the implant has been placed, you’ll still need to get a restoration, or replacement tooth. Implants provide a strong foundation for fixed (permanent) or removable replacement teeth that are made to match your natural teeth. To make the new tooth or teeth, your dentist will make an impression of your existing teeth, creating a model of your bite. The new restoration, most typically a crown, will be based on this model so it will blend in perfectly with the rest of your teeth. The crown is then attached to the connector point (abutment) on your implant. Replacing a lost tooth is essential to a healthy mouth and a healthy body. It will provide you with increased aesthetics of course, but will also improve your speech, ease of eating, and oral hygiene. It is estimated that more than 80 million people in North America snore while sleeping, which not only affects the quality of sleep of the person snoring, but also the quality of sleep of their loved ones and other family members. Luckily, there is a way to treat chronic snoring. What causes snoring? Snoring can be the result of several factors. Typically, snoring is caused by the relaxing of the muscles and soft tissues in the throat and mouth, making the air passage smaller. The decrease in space through the airway makes it harder for each breath to get through, and when it does get through, it moves over the soft tissues in the mouth and throat and causes a vibration that results in the snoring sound. Other causes of snoring may include: Excess body weight Alcohol consumption Certain sleep aides or sedatives Obstructive Sleep Apnea (OSA) What is a Mandibular Advancement Device? If you snore at night, then a mandibular advancement device (MAD) may represent a solution and a better night's sleep! The MAD is a specially designed dental device that gently helps keep the lower jaw, or mandible, in a forward position, increasing the space between the airway passage and helping you breathe better so you can get a full, quiet night's sleep. Some devices also stop the tongue from falling back over your windpipe. Your dentist will fit these special appliances to meet your individual condition. Is a MAD comfortable? The answer is simple — YES! A mandibular advancement device is made to be comfortable, so that you can sleep without even noticing you're wearing it. It does not prevent you from breathing with your mouth open and will even eliminate snoring for patients with sinus congestion or allergies. You may experience some slight stiffness of the jaw for the first few mornings after wearing the device, but this feeling is only temporary, and will go away after you wake up and remove the device. We always start with the least invasive options, which are non-surgical. However, in more serious cases, surgery may be necessary. Non-Surgical Treatment The first line of defense against gum disease is a unique type of cleaning called “scaling and root planing.” In this procedure, an ultrasonic cleaning device is used to remove plaque and tartar from your teeth where regular cleaning devices can't reach: under the gum line, on the tooth, and around the root. Then, the rough surface of the tooth and the root are smoothed out (planed). This provides a healthy, clean surface that makes it easier for the gum tissue to reattach to the tooth. If you address your gum disease before it becomes severe, scaling and root planing may be the only treatment you need. However, as with any dental procedure, after-care is vital. In order to keep your teeth in good shape and resist future occurrences of gum disease, you must brush and floss daily, eat a healthy diet, avoid tobacco use, and have regular dental checkups. Even after a successful scaling and root planing, if you don't attend to your teeth properly, it's quite likely that you'll develop gum disease again. Surgical Treatment Options If the tissue or bone surrounding your teeth is too damaged to be repaired with non-surgical treatment, several surgical procedures are available to prevent severe damage and to restore a healthy smile. We will recommend the procedure that is best suited to the condition of your teeth and gums. Following is a list of common types of periodontal surgery: Pocket Depth Reduction In a healthy mouth, the teeth are firmly surrounded by gum tissue and securely supported by the bones of the jaw. Periodontal disease damages these tissues and bones, leaving open spaces around the teeth that we call pockets. The larger these pockets are, the easier it is for bacteria to collect inside them, leading to more and more damage over time. Eventually the supportive structure degrades to the point that the tooth either falls out or needs to be removed. During pocket reduction procedures (also known as “flap surgery”), we fold back the gum tissue and remove the bacteria hiding underneath, as well as the hardened plaque and tartar that have collected. We may also remove any tissue that is too damaged to survive. We then sew the healthy tissue back into place. Now that the tooth and root are free of bacteria, plaque, and tartar, and the pockets have been reduced, the gums can reattach to the teeth. Regeneration When the bone and tissue supporting the teeth have been lost due to severe gum disease, we can restore these areas with a regeneration procedure. During this process, we begin by folding back the gum tissue and removing the bacteria, plaque, and tartar. Depending on your situation, we may then perform a bone graft to stimulate new bone growth, or we may apply a special kind of protein that stimulates tissue growth to repair the areas that have been destroyed by the disease. Soft-Tissue Graft A frequent symptom of gum disease is gum recession (also called gingival recession). As the gums recede, more of the roots are revealed. This can make teeth appear longer and can also create sensitivity to hot or cold liquids or food. It also exposes the tooth to increased damage from gum disease, as bacteria, plaque, and tartar attack the surface of the tooth and the root. During a soft-tissue graft, tissue from the top of your mouth or another source is sewed to the gum area, covering the roots and restoring the gum line to its original, healthy location. This procedure can also be performed for cosmetic reasons. The dental implant itself is only a replacement for the root of the lost tooth, so after the implant has been placed, you’ll still need to get a restoration, or replacement tooth. She is a Bay Area native, having grown up in San Francisco and later moving with her family to the East Bay. With a passion in enhancing people's lives with periodontal issues, Dr. Tiffany pursued her specialist training as a Periodontist in the UCSF Dental School Periodontal Department. Prior to this, she earned her undergraduate degree at University of California, San Diego, her Master of Science in Microbiology at Cal Poly SLO, and her Doctorate of Dental Medicine at Western University of Health Sciences in greater Los Angeles. She returned home to complete additional training with a focus on dental surgery in a general practice residency at VA Northern California. Here, she helped many local Bay Area veterans achieve their desired smiles through advanced dental technologies and procedures. Dr. Tiffany is deeply devoted to working closely with her patients in a relaxed atmosphere in order to provide them with the highest quality of care. Dr. Tiffany is a Board certified periodontist. Children are not born with a natural fear of the dentist, but they can fear the unknown. Our office makes a special effort to use pleasant, non-frightening, simple words to describe each treatment. We want you and your child to feel at ease from the moment your family arrives at our office. The more you and your child know about the first visit, the better you will feel. The American Academy of Pediatric Dentistry recommends... Children should visit the dentist by their first birthday. It is important that your child’s newly-erupted teeth (erupting at six and 12 months of age) receive proper dental care and benefit from proper oral hygiene habits right from the beginning. Getting to know your teeth is fun! Download our Dynamite Dental Fun Kit! When New Teeth Arrive Your child’s first primary or baby teeth will begin to erupt between the ages of six and 12 months, and will continue to erupt until about age three. During this time, your child’s gums may feel tender and sore. To help alleviate this discomfort, we recommend that you soothe the gums by rubbing a clean finger or a cool, wet cloth across them. You may also choose to make use of a teething ring. When your child has finished teething, you can expect a total of 20 primary teeth. Your child’s primary teeth are shed at various times throughout childhood. Permanent teeth begin erupting at age six, and continue until age 21. Adults have 28 permanent teeth (32, including wisdom teeth). Adopting Healthy Oral Hygiene Habits As your child’s teeth erupt, be sure to examine them every two weeks, looking for lines and discoloration that may be caused by decay. Remember that sugary foods and liquids can attack a new tooth, so take care that your child brushes after feeding or eating. We recommend brushing four times a day for optimal oral hygiene: after breakfast, after lunch, after dinner, and at bedtime. Brushing can be fun, and your child should brush as soon as the first tooth arrives. Fluoridated toothpaste is recommended for all children starting at tooth eruption, regardless of caries risk. A smear (the size of a grain of rice) of toothpaste should be used up to age three. After the third birthday, a pea-sized amount may be used. We recommend dispensing toothpaste for young children and supervising and assist with brushing. Flossing is also a part of good oral hygiene habits, and your doctor will discuss with you the right time to start flossing. If you notice signs of decay, contact your dentist immediately. Preventing Tooth Decay with Regular Checkups Tooth decay is caused by sugars left in your mouth that turn into an acid, which can break down your teeth. Children are at high risk for tooth decay for a simple reason: many children and adolescents do not practice regular, good oral hygiene habits. Proper brushing and flossing routines combined with regular dental visits help keep tooth decay away. Your child should visit the dentist every six months for regular dental cleanings and checkups. We recommend fluoride treatments twice a year along with cleanings to keep teeth their strongest. Tooth sealants are also recommended because they “seal” the deep grooves in your child’s teeth, preventing decay from forming in these hard-to-reach areas. Sealants last for several years, but will be monitored at your child's regular checkups. These teeth usually appear in late teens or early twenties but may become impacted (fail to erupt) due to lack of room in the jaw or angle of entry. The most common type of impacted wisdom tooth is “mesial”, meaning that the tooth is angled forward toward the front of your mouth. When a wisdom tooth is impacted, it may need to be removed. If it is not removed, you may develop gum tenderness, swelling, or even severe pain. Impacted wisdom teeth that are partially or fully erupted tend to be quite difficult to clean and are susceptible to tooth decay, recurring infections, and even gum disease. Each patient's situation is unique. Your dentist will usually take a panoramic X-ray to determine whether your wisdom teeth will need to be removed. If your dentist recommends removal of your wisdom teeth, it is best to have them removed sooner rather than later. Wisdom teeth are typically removed in the late teens or early twenties because there is a greater chance that the teeth's roots have not fully formed and the bone surrounding the teeth is less dense. These two factors can make extraction easier, as well as making the recovery time much shorter. To remove a wisdom tooth, your dentist first needs to numb the area around the tooth with a local anesthetic. Your dentist can use additional medication to safely sedate you during the extraction if you are feeling nervous about the procedure. Since the impacted tooth may still be under the gums and imbedded in your jaw bone, your dentist will need to remove a portion of the covering bone to extract the tooth. To minimize the amount of bone that is removed with the tooth, your dentist will often “section” your wisdom tooth so that each piece can be removed through a small opening in the bone. Once your wisdom teeth have been extracted, the healing process begins. Healing time varies depending on the degree of difficulty related to the extraction. Your dentist will let you know what to expect and will provide instructions for a comfortable, efficient healing process. We will ask you to fill out several forms that will get you acquainted with our office. Your initial exam will last approximately one hour. This visit will help us get acquainted with each other and learn about your goals and desires for your dental treatment. Generally, X-rays will be taken allowing your doctor to examine the jaw’s structure, teeth position and teeth decay. Oral hygiene instructions will also be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth on this visit and provide an evaluation that will outline your existing dental problems and proposed treatment. During your regular exam, we will: Check for any problems that you may not see or feel Look for cavities or any other signs of tooth decay Inspect your teeth and gums for gingivitis and signs of periodontal disease Perform a thorough teeth cleaning Your regular exam will take about 45 minutes. Each regular exam includes a detailed teeth cleaning, in which we will clean, polish, and rinse your teeth to remove any tartar and plaque that have built up on the tooth's surface. During dental bonding, a white filling is placed onto your tooth to improve its appearance. The filling “bonds” with your tooth, and because it comes in a variety of tooth-colored shades, it closely matches the appearance of your natural teeth. Tooth bonding can also be used for tooth fillings instead of silver amalgam. Many patients prefer bonded fillings because the white color is much less noticeable than silver. Bonded fillings can be used on front or back teeth, depending on the location and extent of tooth decay. Bonding is less expensive than other cosmetic treatments and can usually be completed in one visit to our office. However, bonding can stain and is easier to break than other cosmetic treatments, such as porcelain veneers. If it does break or chip, tell your doctor. The bonding can generally be easily patched or repaired in one visit. ↑ Back to top Bridges A bridge may be used to replace missing teeth, help maintain the shape of your face, and alleviate stress on your bite. A bridge replaces missing teeth with artificial teeth, looks great, and literally bridges the gap where one or more teeth may have been. Your bridge can be made from gold, alloys, porcelain, or a combination of these materials and is bonded onto surrounding teeth for support. The success of any bridge depends on its foundation: the other teeth, gums, or bone to which it is attached. Therefore, it’s very important to keep your existing teeth, gums, and jaw healthy and strong. ↑ Back to top Crowns Crowns are a restorative procedure used to improve your tooth’s shape or to strengthen a tooth. Crowns are most often used for teeth that are broken, worn, or have portions destroyed by tooth decay. A crown is a “cap” cemented onto an existing tooth that usually covers the portion of your tooth above the gum line. In effect, the crown becomes your tooth’s new outer surface. Crowns can be made of porcelain, metal, or both. Porcelain crowns are most often preferred because they mimic the translucency of natural teeth and are very strong. Crowns or onlays (partial crowns) are needed when there is insufficient tooth strength remaining to hold a filling. Unlike fillings, which apply the restorative material directly into your mouth, a crown is fabricated away from your mouth. Your crown is created in a lab from your unique tooth impression, which allows a dental laboratory technician to examine all aspects of your bite and jaw movements. Your crown is then sculpted just for you so that your bite and jaw movements function normally once the crown is placed. ↑ Back to top Dentures Dentures are natural-looking replacement teeth that are removable. There are two types of dentures: full and partial. Full dentures are given to patients when all of the natural teeth have been removed. Partial dentures are attached to a metal frame that is connected to your natural teeth and are used to fill in where permanent teeth have been removed. Just like natural teeth, dentures need to be properly cared for. Use a gentle cleanser to brush your dentures, always keep them moist when they’re not in use, and be sure to keep your tongue and gums clean as well. ↑ Back to top Extractions There are times when it is necessary to remove a tooth. Sometimes a baby tooth has misshapen or long roots that prevent it from falling out as it should, and the tooth must be removed to make way for the permanent tooth to erupt. At other times, a tooth may have so much decay that it puts the surrounding teeth at risk of decay, so your doctor may recommend removal and replacement with a bridge or implant. Infection, orthodontic correction, or problems with a wisdom tooth can also require removal of a tooth. When it is determined that a tooth needs to be removed, your dentist may extract the tooth during a regular checkup or may request another visit for this procedure. The root of each tooth is encased within your jawbone in a “tooth socket,” and your tooth is held in that socket by a ligament. In order to extract a tooth, your dentist must expand the socket and separate the tooth from the ligament holding it in place. While this procedure is typically very quick, it is important to share with your doctor any concerns or preferences for sedation. Often considered necessary for complete oral health, dentistry can have an impact on the health of your entire body. What is a dentist? A dentist is a specialist who works to diagnose, treat, and prevent oral health problems. Your dentist has completed at least eight years of schooling, and received either a DDS (Doctor of Dental Surgery) degree, or a DMD (Doctor of Dental Medicine) degree. If your doctor is a pediatric dentist, this means that he or she specializes in caring for children from infancy through their teen years. A pediatric dentist has received the proper education and training needed to work with young kids. Other specializations include: Endodontics (root canals) Oral and maxillofacial (including pathology, radiology, and surgery) Orthodontics and dentofacial orthopedics Periodontics (gum disease) Prosthodontics (implants) Why is visiting the dentist so important? Visiting the dentist regularly will not only help keep your teeth and mouth healthy, but will also help keep the rest of your body healthy. My teeth feel fine. Do I still need to see a dentist? Your teeth may feel fine, but it's still important to see the dentist regularly because problems can exist without you knowing. Your smile's appearance is important, and your dentist can help keep your smile healthy and looking beautiful. Today's dentists offer many treatment choices that can help you smile with confidence, including: Professional teeth whitening Fillings that mimic the appearance of natural teeth Tooth replacement and full smile makeovers What should I look for when choosing the right dentist for me? Choosing a dentist who “clicks” with you and your family is important, and you may wish to consider several dentists before making your final decision. During your first visit, you should be able to determine whether the dentist is right for you. During your appointment, consider the following: Is the appointment schedule convenient? Is the office easy to get to and close by? Does the office appear to be clean and orderly? Was your medical and dental history recorded and placed in a permanent file? Does the dentist explain techniques for good oral health? Is information about cost presented to you before treatment is scheduled? Is your dentist a member of the ADA (American Dental Association)? How can I take care of my teeth between dental checkups? ALWAYS remember to brush your teeth at least two times a day, and floss at least once! Make sure to use toothpaste that contains fluoride, and ask your dentist if you need a fluoride rinse. This will help prevent cavities. Avoid foods with a lot of sugar (sugar increases the amount of bacteria that grows in your mouth causing more plaque and possibly cavities) and avoid tobacco (this can stain your teeth, cause gum disease, and eventually lead to oral cancer). Don't be afraid to brush your tongue! By brushing your tongue, you will remove food particles and reduce the amount of plaque-causing bacteria. Tongue brushing also helps keep your breath fresh. Be sure to schedule your routine checkup. It is recommended that you visit the dentist every six months. At what age should I start taking my child to see the dentist? The American Academy of Pediatric Dentistry (AAPD) recommends that children first see a dentist as early as six months of age and no later than one year of age. During this time, your child's baby teeth will be coming in and your dentist can examine the health of your child's first few teeth. After the first visit, be sure to schedule regular checkups every six months. How often should I see the dentist? Children, teens, and adults should all see the dentist for a regular checkup at least once every six months. Patients who are at a greater risk for oral cancer or gum disease may be required to see the dentist more than just twice a year. Your doctor will help determine how often you should visit the dentist for regular checkups. What is a cavity? A cavity is a small hole that forms inside the tooth because of tooth decay. Cavities are formed when plaque buildup on the outside of the tooth combines with sugars and starches in the food you eat. This produces an acid that can eat away the enamel on your tooth. If a cavity is left untreated, it can lead to more serious oral health problems. Cavities can be prevented by remembering to brush your teeth at least two times a day and floss between teeth at least once. What is a filling? A filling is a synthetic material that your dentist uses to fill a cavity after all of the tooth decay has been removed. Fillings do not generally hurt because your dentist will numb your mouth with an anesthetic. Fillings are made from a variety of different materials, including composites, gold, or ceramic. If you need a filling, be sure to talk to your doctor about what type is best for you and your teeth. How often should I brush my teeth? According to your dentist and the American Dental Association, you should brush your teeth at least two times a day. Brushing keeps your teeth, gums, and mouth clean and healthy by removing bacteria-causing plaque. It is also recommended that you use a soft-bristled toothbrush and toothpaste that contains fluoride when you brush your teeth. You should spend at least a minute on the top teeth and a minute on the bottom teeth, and remember to brush your tongue; it will help keep your breath smelling fresh! When should I change my toothbrush? Your toothbrush will eventually wear out, especially if you are brushing your teeth twice a day for two to three minutes each time. Your dentist recommends that adults and children change their toothbrush every three months. If you are using an electric toothbrush, be sure to read the directions because you may not need to change toothbrush heads as frequently. Patients with gum disease are encouraged to change their toothbrush every four to six weeks to keep any bacteria from spreading. During dental bonding, a white filling is placed onto your tooth to improve its appearance. The filling “bonds” with your tooth, and because it comes in a variety of tooth-colored shades, it closely matches the appearance of your natural teeth. Tooth bonding can also be used for tooth fillings instead of silver amalgam. Many patients prefer bonded fillings because the white color is much less noticeable than silver. Bonded fillings can be used on front or back teeth, depending on the location and extent of tooth decay. Bonding is less expensive than other cosmetic treatments and can usually be completed in one visit to our office. The success of any bridge depends on its foundation: the other teeth, gums, or bone to which it is attached. Crowns are most often used for teeth that are broken, worn, or have portions destroyed by tooth decay. A crown is a “cap” cemented onto an existing tooth that usually covers the portion of your tooth above the gum line. In effect, the crown becomes your tooth’s new outer surface. Crowns can be made of porcelain, metal, or both. Porcelain crowns are most often preferred because they mimic the translucency of natural teeth and are very strong. Crowns or onlays (partial crowns) are needed when there is insufficient tooth strength remaining to hold a filling. Unlike fillings, which apply the restorative material directly into your mouth, a crown is fabricated away from your mouth. Your crown is created in a lab from your unique tooth impression, which allows a dental laboratory technician to examine all aspects of your bite and jaw movements. There are two types of dentures: full and partial. Full dentures are given to patients when all of the natural teeth have been removed. Just like natural teeth, dentures need to be properly cared for.
