Facebook Instagram   Linkedin YouTube Search Notifications & Alerts iPlacentia App Notify Me Placentia TV CA WEEKLY Home About Placentia Military Banner Recognition Program Military Banner Recognition Program On July 24, 2018 the Placentia City Council approved the Placentia Military Banner Recognition Program. The program, known as Placentia "Hometown Heroes," honors and recognizes both active-duty service members in the United States Armed Forces, and Honorably Discharged service members, including those honored posthumously. Branches of service include the Air Force, Army, Coast Guard, Marines, National Guard, Navy, and Space Force. Honorees must currently reside in Placentia, or ; be employed in Placentia (at least 1year), or ; enlisted in Placentia, or ; have an immediate family member (parent, spouse, son, daughter, grandparents) residing in Placentia. If the immediate family member resides in Placentia, this satisfies the local connection requirement, even if the honoree lives elsewhere. Applications may be submitted by the honoree or  by an immediate family member, friend, or representative applying on the honoree's behalf. Proof of military service (e.g., DD-214 or Statement of Service) is required. The banner will be installed along Kraemer Boulevard and Yorba Linda Boulevard based on the City's discretion and will remain on display for two years. The City will not be responsible for replacing banners that are stolen, damaged, destroyed due to sun, weather, vandalism or any act of nature beyond our control such as high winds. Upon the end of the display period, the banner will be given to the person named on the banner application. Application Process Applications will be accepted through October 21, allowing time for the banners to be ordered and installed during the month of November. The Fraud Hotline is designed to report suspected cases of fraud that have a financial impact on the City, including, but not limited to, theft, waste, or abuse of city resources. Examples include: Theft of City property, such as inventory or supplies Using City property for personal use Accepting a bribe, kickback, or illegal gratuity Bid-fixing Please note that the hotline is not a substitute for employment-related grievances or appeals, as existing City policy and procedures provide channels for resolving these issues. How to Report If you have an issue or concern to report, use our independent, third-party, anonymous, and confidential reporting service. You will need our Company ID (PLACENTIA) when reporting incidents or concerns. Here’s how to report: Online: reports can be submitted at www.fraudHL.com 24-hours a day using the secure online reporting form Phone or Fax: call or fax the toll-free number 1-855-FRAUD-HL We Take Action We review all tips and take appropriate action. For every tip we receive, we conduct preliminary fact-finding and case analysis to determine if there is sufficient basis to undertake an in-depth investigation. For every case, we can take one of the following courses of action: Conduct our own investigation: We investigate a case directly, depending on the severity and nature of the allegation and available resources. Request an audit by an independent third-party: Depending on the nature and severity of the allegation an independent third-party (e.g. audit firm, etc.) will investigate the allegation. Request an outside agency to review the tip: Allegations that do not fall within the City's jurisdiction are referred to a county, state, or federal agency for investigation. Examples include credit card fraud, welfare fraud, unemployment fraud, and mail fraud. Close a case if it lacks merit or sufficient information: Because cases can be made anonymously, we do not always receive sufficient information to thoroughly investigate. In other instances, the case may lack merit to continue. We will close such cases during the initial review. The primary mission of the Department is the delivery of life safety services. The department provides 24-hour emergency response to a wide variety of critical situations, including fires, medical emergencies, accidents, and miscellaneous public assistance requests. The Department strives to provide the highest level of life and property safety in a caring and cost-effective manner and to continually provide quality customer service for the community, the City, and one another.  The Fire Department is comprised of dedicated career firefighters.
